An exciting opportunity has arisen for an enthusiastic and motivated senior doctor in training (above ST4 or equivalent) with an interest in leadership, management and quality improvement to join the Department of Acute Medicine, Whipps Cross University Hospital, as a chief registrar.


The chief registrar role provides senior trainees with 40-50% protected time to develop skills and confidence in leadership, management and quality improvement. Learning that chief registrars gain from the Royal College of Physicians’ (RCP’s) bespoke development programme will be put into practice in a supported environment that provides autonomy, flexibility and support to develop and support projects that address key local challenges and priorities. This may include service improvement, engagement and morale, education and training, workforce, and sustainability.

The chief registrar role is a leadership role for senior specialty trainees. The role provides 40-50% protected time to develop and implement local initiatives focusing on, for example, service improvement, engagement and morale, education and training, workforce and sustainability.

Chief registrars benefit from access to a bespoke taught development programme provided by the RCP, which runs from September 2024 to June 2025 and comprises five 2-day modules that chief registrars are expected to attend.

The chief registrar will ideally report to and be mentored by the medical director, a nominated deputy, or other senior clinical leader. The mentor role requires a minimum time commitment of monthly 1 hour meetings with the chief registrar, and this commitment should be taken into account when nominating a suitable mentor.

The chief registrar will also have a named educational supervisor for their clinical role, who may or may not be the same as their clinical supervisor.

You will spend 6 months working as a senior fellow at Newham University Hospital, where you will be a core member of the medical team, responsible for providing skilled care to critically unwell patients across the hospital, and you will spend 6 months working with London’s Adult Critical Care Emergency Support Service, ACCESS, which provides for the transfer and retrieval of critical care patients across London.

ACCESS
• Join the dedicated, multi-disciplinary transfer team as the transfer doctor to

complete clinical, capacity and repatriation moves for critically unwell patients.
• Take clinical leadership of the team and undertake safe inter-hospital

transfers with distant supervision (after an appropriate period of supervision

and sign-off)
• Ensure thorough and accurate record keeping during transfers
• Perform effective clinical handover when handing over to receiving team
• Carry out appropriate equipment checks at the beginning and end of the day
• Attend daily pan-London briefings
• Carry out clinical transfers to the level appropriate to training

ICU:As a member of the team the post holder will be responsible for the evaluation and management of patients referred to critical care as well as the provision of care to patients on the unit. The post holder will ensure that evidence-based care is delivered to the patients and will have responsibility for communication with other members of the multi-disciplinary team and with family members. The service is fully supported by the ICU consultant who has ultimate responsibility for the management of the patient. All significant decisions and plans, including decisions to admit, discharge and transfer, need to be discussed with the duty consultant and the outcome of these discussions need to be accurately documented in the patient record.



ACCESS:The senior fellow will be responsible for assessing patients suitability for transfer, working with the clinical teams and the ACCESS consultant to ensure that patients are able to receive the care they need. Following a successful assessment the Fellow will provide the clinical support during the transfer, delivering interventions as necessary to maintain the patient’s safety. At the receiving unit the fellow will handover the patient to the receiving team, and then work to ensure the team can green up for the next deployment.



Clinical Governance

The appointee will be expected to input into the Clinical Governance activities of the Trust to maintain the quality standards of clinical services. There is a monthly Governance meeting and the hospital holds a weekly Serious Incidents meeting. The post holder will be expected to actively participate in all aspects of governance activity to ensure that excellence of care will flourish.



Research and Development



The department has an active research focus, led by Dr Juan Martin. There have been four publications in the last two years along with poster presentations at international conferences. Newham has contributed hugely to the Recovery trial, recruiting over 200 of the 737 patients recruited by the whole Trust.

Time permitting, the post-holder will be encouraged to initiate and to participate in research programmes under the guidance of the critical care research lead, and the clinical lead in accordance with Trust policy. Extensive research facilities exist at Barts Health NHS Trust where collaborative and translational research will be actively encouraged. Research interests include Peri-Operative Optimisation, microcirculatory blood flow, acute kidney injury, genomics, Covid, VAP and critical care follow-up.













Teaching and training

Are you dynamic Band 6 or 7 Physiotherapist who is passionate about delivering high quality physiotherapy services to within the field of Surgery & Critical Care? Following recent successful business cases, we are looking to recruit additional staff into the Critical Care and Surgery Team at Wexham Park Hospital. We are very excited to have the opportunity to further develop the senior leadership within the Critical care and Surgical team.

This Team Lead post offers the exciting prospect of leading the team working in Critical Care & Surgery alongside a peer. This will include inpatient work in Critical Care, Surgical Acute Dependency Unit, General Surgery, plastics & ENT. There will also be opportunities to contribute & collaborate with our award-winning surgical prehabilitation service.

You will be line managed by our Principal Physiotherapist for ICU, Surgery & Orthopaedics & have peer support with your Band 7 colleague within team. You will also work closely with our cross-site APP for Critical Care & Critical Care Rehabilitation & Follow-up service to ensure our critical care patients experience an excellent level of rehabilitation from critical care admission to discharge into the community.

This post holder will lead, develop and evaluate surgical & critical care physiotherapy services at Wexham Park Hospital.

They will provide therapeutic clinical expertise, high quality advice and support to the multi-disciplinary team. Clinical supervision, appraisal and teaching will be key components of this role.

They will demonstrate excellent communication and collaborative working, along with other clinical specialists across the Trust, in order to provide a high quality, efficient service.

In conjunction with the Principal, APP and peer they will ensure clinical governance targets are met, including the development of clinical standards and protocols, specialist evidence based practice, audit, research, clinical and non-clinical risk management.

Job Responsibilities*

Primary Responsibilities and Duties (80%)

• To develop and implement appropriate psychotherapy treatment for inpatients and outpatients.
• To contribute, as part of an inter-disciplinary team, in the overall care plan for the patient.
• As part of an inter-disciplinary tea, to facilitate transition to adult-care.
• To develop or improve on workflow for better patient care.
• For the Senior Psychologist, to initiate and implement new psychological programmes with guidance.

Secondary Responsibilities and Duties (20%)

• To assist in the development of materials for teaching and training.
• Provide in-service training for department staff as needed.

Requirements*
