All duties of the Clinical Pharmacy Technician will be performed under the direct supervision of the assigned CPP/CP. All clinical decision making will first be reviewed by the CPP/CP. A. Anticoagulation Clinic Support:
1. Performs medication reconciliation to include reviewing medication history, changes (including herbals, dietary supplements, and over-the-counter medications), adherence, and complications of therapy and report findings to CPP/CP.
2. Obtains and assess information on compliance, medication dosages, medication changes, alcohol consumption, smoking, bleeding symptoms, dietary habits, and general state of health. The information obtained will then be communicated to the CPS/CP.
3. Assess patient compliance and attains renewals for Direct Oral Anticoagulants (DOACs).
4. Routinely monitors laboratory data as appropriate (to include, but not limited to CBC, UA, Creatinine, INR, A1C, CMP, FLP) and assist with scheduling future lab orders and follow up appointments.
5. Prints daily schedules and "no shows/lost to follow up" reports. Review both reports and patients' electronic health record (EHR) to identify which patients have and have not been followed up within the last 60 days.
6. Telephone support involves appointment reminders, rescheduling patients who have missed appointments, phone interviews of patients who have obtained INRs from outside labs, and triaging incoming phone calls from patients, their family members or other health providers.
7. Completes electronic progress notes, reviews all encounters daily, and notify CPP for completion.

B. Medication Management:
1. Interviews patients, family members and/or caregivers, to develop an accurate list of medications, drug strengths, dosages, routes, and instructions the patient has been taking.
2. Obtains relevant patient information from patient interviews, assisted living facilities (ALF's), and nursing homes using joint legacy viewer (JLV) with our department of defense (DOD) partners.
3. Compares the compiled list with the active medication profile in the computerized medical record of the patient.
4. Documents the compiled list in the electronic health record, noting any discrepancies or omissions within 24 hours of patient encounter.
5. Updates and clarifies allergy information in patient's records in an electronic health record (EHR)
6. Improves medication safety by collecting up-to-date and prior-to admission medication lists for patients admitted to the hospital.

C. Hospital Discharges:
1. Assists with reviewing the provider discharge medication reconciliation note and notifies the pharmacist of discrepancies that need to be addressed with the provider.
2. Prepares Pharmacy Home Medication List after discharge medications have been processed by the Pharmacist and makes an addendum to the Provider's note stating the medication changes that occurred at the time of discharge.
3. Prepares discharge medications for prescription/order (s) are reviewed by a pharmacist for therapeutic propriety, reviews orders for technical accuracy and converts the order, if necessary, so that the item dispensed corresponds to the drug and dose ordered.

The Office of Mental Health and Suicide Prevention (OMHSP) in the Veterans Health Administration (VHA) is responsible for providing strategic guidance directed toward improving the quality of mental health care for over one and a half million Veterans nation-wide. The Suicide Prevention Program (SPP) is focused on the full implementation of VA's National Strategy for Preventing Veteran Suicide (2018) and the Department of Veterans Affairs and Department of Defense (VA/DoD) Clinical Practice Guideline for Assessment and Management of Patients at Risk for Suicide. Suicide Prevention (SP 2.0) Community Engagement and Partnerships Coordinator (CEPC) positions focus on connecting, developing, and coordinating suicide prevention efforts at the Veterans Integrated Service Network (VISN), state, local, and federal level.

The Ambulatory Care Nurse Manager Nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The registered nurse, as a licensed professional, will be responsible and accountable for his/her practice.

The Ambulatory Care Nurse Manager demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication, and coordination of resources. The Ambulatory Care Nurse Manager actively develops and manages collaborative partnerships with physicians and other members of the multi-disciplinary team as a means of providing high-quality, cost effective, and customer focused health care. He/she is ultimately responsible for ensuring suitable and timely training, appraisal/performance development, counseling, scheduling of the unit, coaches staff, effectively manages employee conduct and performance issues, and completes recurrent reports and performance evaluations in a timely manner. The Ambulatory Care Nurse Manager participates in service and medical center committees and task forces to improve nursing standards of practice and patient care, while identifying educational needs to staff and ensuring that those needs are aptly addressed.

• The incumbent support the Chief Financial Officer (CFO), Deputy CFO (DCFO), and Associate CFO (ACFO), with accounting and financial management, planning and monitoring of all appropriations and National Level accounts for all VHA programs.
• Provides data management expertise, technical guidance, and recommendations for specific actions needed to develop and maintain consistent and reliable budget data repository in SQL for data analysis and monthly reporting evaluating the effects of program operating plans and budget issues.
• Compiles and analyzes data to provide timely and efficient input to various reports on operating plan and budget execution statuses. This will include reviewing historical data funds utilization within the administered programs.
• Enters budgetary data into a variety of forms, schedules, and reports for executive level decision making. This will include recommending and making adjustments when required.
• Serve as an expert level Budget Analyst and will ensure the budget system is effectively utilized as an evaluating tool in measuring program performance and effectiveness; ensure funds are used and controlled to maximize the utility and availability of resources needed to provide quality health care to Veterans including staffing, facilities and supplies; provide advice on developing, coordinating, and carrying-out financial policies, procedures and plans; and reviews, analyzes, evaluates and reports program accomplishments in financial terms.
• Assists with execution oversight of funding maintenance of all programs/activities, review the appropriateness of expenditures, and ensure fiscal integrity of the Program Office's financial records.
• Evaluates variances between operating plan and actual obligation results overseeing VHA's budget execution processes. This includes daily monitoring of the execution of VHA's budget, including VHA Finance's four appropriations (Medical Services, Medical Support and Compliance, Medical Community Care and Medical Facilities); and providing budget execution instructions processes to budget official during budget calls.
• Compares current spending information and trends against the operating plan for complex and diverse programs administered within the network of sites and provide advice on developing, coordinating, and carrying-out financial policies, procedures, and plans.
• Prepares and provides financial statements to program officials for informational and financial planning purposes. This will include preparing a variety of routine financial reports for VHA leadership review.
