Job Description Nurse Reports to Clinical Team Leader Job Summary The post involves working as a member of a multi-disciplinary team providing specialist palliative care and support for children and young people from 0-35 years. The post holder will be expected to work without direct supervision, providing a high standard of holistic care to children and young people with a variety of complex health needs. The Maximising Independence Service comprises of community Frailty, Falls Prevention and uni disciplinary physiotherapy pathways. The services include community centre based Frailty MDT Clinic, Falls Prevention Groups and home visits.

The successful candidates will be a ‘generalist with a special interest model’ of placing ward based acute and sub-acute ward based general internal medicine as a core function while maintaining a specialty interest. The candidate will be welcomed into a well organised department providing a wide range of services in ward based General Internal Medicine and specialty and sub-specialty level services in Diabetes and Endocrinology. We work collaboratively and supportively together and are always looking to improve the care for our patients.
1. To provide a safe, effective and efficient service to enable the best possible care to patient
2. To perform administrative duties including those associated with the care of patients, the day-to-day running of the department, coding and mandatory returns, as well as the collection of other data as required by the Commissioners for performance management.
3. To participate in the development of the service, working to the service specifications and providing input into strategy and planning to the Service Director and management team as required.
4. To carry out all duties at the highest possible standard and in accordance with current quality initiatives within that area of work.
5. To cover for colleagues’ annual leave and other authorised absence, on a reciprocal basis.
6. Any other duties that may be required from time to time

About the Role

Our Guildford Locality Team is a varied, allowing those who wish to do so, to specialise in their area of interest. Alternatively, you could move between the sub-teams and develop a whole range of specialist knowledge! We are a forward thinking, motivational and diverse team incorporating multi-agency and collaborative working as an ever expanding ambition for the team to promote opportunities for team members and customers.

On a daily basis, you will be meeting with clients to understand their care and needs, how they can be supported and to provide the best outcomes for them.
