Clinical Dementia Advisor
Are you enthusiastic and passionate about providing holistic care and support to people affected by Dementia, if so then this is the post for you.
This is an exciting opportunity to join a new dynamic service called Mindset where RDaSH and Alzheimer’s Society are working together to deliver high quality care and support for people affected with Dementia and their families.
You will be employed by RDaSH and work jointly with the Dementia Advisors from the Alzheimer’s Society. By joining a newly developing service the post holder will have a unique opportunity to be involved in the ongoing development of the service. The Clinical Dementia Advisor will be instrumental in establishing the services role in the RDaSH footprint and the wider health and social care community, they will be liaising with colleagues in Primary Care and third partner agencies.
The Clinical Dementia Advisor will be locality based in line with the Doncaster Place Plan. They will be an integrated part of the CMHT and will have full access to the multi-disciplinary team and managerial structure.
We have 1 x full time post for Clinical Dementia Advisor, this post is open to RMN and RGN or Registered Healthcare Professionals with experience in working with people living with Dementia.
• To be responsible for the further assessment, planning implementation and evaluation of the evidence-based care required, including health promotion for an identified group of patients, who
have already been diagnosed as having Dementia.
• Responsible for the appropriate clinical risk assessment of patients contacting the team as appropriate.
• Providing an effective advice, guidance and decision-making process culminating in the coordination of any reviewed plan of care, within MDT support.
• Responsible for the admission, case conference/reviews and discharge in collaboration with members of the multi-disciplinary team.
• Responsible for coordinating the care and assessment of a designated caseload. This includes the comprehensive care planning for maintaining stability, contingency and crisis.
• Active participation in the development of the service is an expectation, setting of quality standards, including the auditing, monitoring, and reviewing in line with current clinical guidance practice and policy.
