The DCQM provides executive leadership and direct supervision over the following areas: Accreditation, Risk Management, External Peer Review, and Patient Safety. Works towards achievement of high quality, safety, and cost effective evidence based outcomes on a continual journey of a high reliability organization. Collaborates with both administrative and clinical services to monitor quality and safety within the service and medical center. Ensures processes are in place for continued accreditation readiness and participates in rounds to survey clinical departments. Functions as facilitator in implementing action plans and process improvement strategies at services and medical center for follow up to include regular progress reports. Reviews quality data for trends and early intervention. Applies clinical performance improvement methods, collects outcome data related to policy, procedure, and regulatory compliance and in collaboration with management, initiates performance improvement monitoring projects. Utilizes data and comparative analytical processes in recommending design and implementation of innovative programs to improve service excellence, customer satisfaction, and compliance to patient care and regulatory standards. Major duties and responsibilities include, but are not limited to:

1. Provides executive input in the strategic planning, budget, mission, operational planning, and policy development for local and network efforts.
2. Utilizes current trends and relevant theories in an analytical framework to design/modify, implement, and measure integrated programs and activities to achieve clinical and administrative goals of the medical center and/or network. Evaluates outcomes against strategic priorities.

4. Consultant to medical center staff and greater community in areas of expertise.

The Chief Nurse executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines, influence organizational mission and health care. The Nurse IV is responsible for administrative functions of their assigned nursing units/programs. Functions as an administrator, educator, and consultant utilizing management theory in collaboration with VISN, Chief of Staff, Patient Care Services and other services. Must establish and maintain effective relationships with all levels of medical center personnel and relate effectively with customers, families, and the community as well as program and service leaders at the local and network levels. Key relationships with staff are needed to (1) Assist nurse managers and advanced practitioners to carry out the functions and activities expected of them, (2) Contribute ideas and recommendations for the establishment of standards of care, policies, and objectives for the enhancement of nursing organization-wide, (3) Assist in policy-making activities as related to customer service and the overall functioning of administrative and clinical programs, and (4) Collaborate with other health care staff to establish and maintain programs that cross service and/or discipline lines and influence organizational mission and health care.

Specific programs include Medical Practice, Women's Health, Anticoagulation Clinic, Infectious and Disease Clinic, and the CBOC's (Eureka, Ukiah, Clearlake, Santa Rosa, Oakland, Downtown, San Bruno). She/he assumes accountability for administrative functions for these areas and is responsible for planning, developing, implementing, directing, and evaluating the overall program and functions therein including staffing and budget accountability. As Chief Nurse in Primary Care, she/ he is responsible for all processes, performance measures and other metrics that support Quality Care and Access. She/he supports and facilitates outreach that provides access to exemplary care that is fully integrated across the continuum. She/he is fully committed to continually improving Primary Care and related services. This Chief Nurse partners with Nurse and Physician Colleagues, Clinical and Administrative Chiefs of Service, and Administrative and Clinical Directors to achieve the goals and performance measures of VHA, VISN 21, and SFVAHCS by ensuring the highest quality care and expert Nursing Practice is provided in all areas. She/he develops and maintains effective relationships with all levels of internal and external customers by interacting and relating effectively with veteran patients and families, community partners, local program and service leaders, VISN Program leaders and National Healthcare Leaders within VHA and the larger Community. The Nurse IV Deputy Chief of Quality Management for Performance Improvement executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines, influence organizational mission and health care. The DCQMPI initiates, develops, leads, and directs the facility systems redesign, performance measures management, data analytics, High Reliability Organization, and Pathways to Excellence/Evidence Based Practice initiatives and programs. The DCQMPI works collaboratively with the Clinical Service Chiefs, Administrative Service Chiefs, and Nursing Leadership to achieve VA Northern California Health Care System, In the absence of the Chief of Quality Management (CQM), the incumbent acts as a member of the Executive Leadership Team (ELT) and has executive decision-making authority over areas of responsibility within Quality Management Service as well as the accreditation, risk management, and patient safety programs.
