Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.
Duties include but not limited to:
- Organize and participate in an urban human immunodeficiency virus (HIV) clinic.
- Assist clinical staff to organize patient care visits.
- Provide orientation visit for new patients to clinic.
- Track patients who are high-risk for noncompliance with medical follow up due to medical and psychosocial reasons such as substance abuse and homelessness, older age and geriatric comorbidities.
- Coordinate with infectious disease physicians, medical trainees, social workers, nurse practitioners, clinical nurses, pharmacists, clinical psychologists, dieticians, psychiatrist and community case management organizations who provide multidisciplinary care to HIV infected patients.
- Responsible with coordination of patient for general infectious disease and travel consults.
- Review antibiotic consults for Medical Center.
- Conduct chart reviews and telephone calls to patients for pre-visit preparation or post visit follow up.
- Coordinate HIV registry if applicable, collaborate with public health department, order test under RN policy, oversight of clinic flow and administrative and clinic assignments to ensure smooth clinic operation and maintain compliance with standards.
