The Hospital Discharge team services to all Surrey residents who meet social care criteria in line with the Care Act. We are leading the way in implementing new ways of working in personalisation and a recovery-focused strength-based approach. Our goal is for our service-users to be able to make informed choices about the services they use, be more in control of their care, and to be active citizens within their community.

About the Role
This is an excellent opportunity for a Social Worker looking to develop their experience in a busy specialist service. We are looking for a Social Worker who is keen to work in a proactive, dynamic, friendly adult social care countywide team that undertakes adult social care assessments and carers assessments, s.42 safeguarding enquiries and Mental Capacity Assessments. We work collaboratively with our local Mental Health Trust and other stakeholders to facilitate safe and timely discharge from hospital for our residents.

We are committed to creating opportunities for service users to develop interpersonal and social skills through engagement in community activities. We work in a preventive capacity to reduce social isolation and seek to promote opportunities to enable re-integration and the development of independent living skills. We are developing expertise in MCA within the area of Substance Misuse and are supportive of Social Work staff developing their careers as BIAs, AMHPs or Practice Educators.

Ongoing training and supervision will be provided as per the needs of the service and Surrey County Council policies, including the application of the Care Act and MCA within the substance misuse arena. We are co-located and work closely with our health partners, and you must be able to demonstrate ability to form effective relationships with partners, stakeholders, service users and their families'.

Shortlisting Criteria

• Professional qualification in Social Work and current registration with Social Work England

Your new role

Break fix server engineer working on HP, Dell and IBM servers, Server provisioning, Rack, stack and configure servers.Support Trainee DC Engineers when required.Troubleshoot hardware faults and arrange replacements.Install and decommission redundant equipment.Provide expert hardware support.Responsible for proactive updating of customers and resolution of service.Co-operate and work closely with all members of the support team to ensure efficient, productive and high quality technical support and customer service to customers within the live Data Centre.Liaise with onsite support engineers and within other sites.Manage all incoming calls, emails to the data centre support function.Liaise with third party suppliers to report and escalate customer problems.Respond to customers within agreed SLA.Follow escalation processes; tickets to support management and other teams as required.Escalate tickets to 2nd line against agreed timescales and continue to monitor these tickets to resolution.

Early - 07:00 to 14:30; Late - 14:30 to 22:00; Week night - 22:00-07:00; Weekend day - 07:00-19:00; Weekend night - 19:00-07:00

Secretarial: To prepare and distribute documents by using the patient administration system (Systmone) and Microsoft office software (letters, memos, reports, discharge summaries, etc) ensuring that all policies and procedures are adhered to. To be responsible for responding to requests for patient records including tracking and reporting of activity to Trust HQ and the team manager in line with information governance procedures. To assist in carrying out a quarterly or annual audit of client case notes and act in accordance with the Data Protection Act. Administration and Management of Resources: To be responsible for the day to day management of the office and the development, implementation and maintenance of office practices and procedures. I am currently assisting a market leading brand in their search for a Planner to join their Team. This is an opportunity to be site based, as well remote working. As a Planner, you will benefit from working with the existing construction team and have full support from a Planning Manager and Construction Director.
