The Role
We are looking to recruit enthusiastic and supportive Community Care officers within our Neighbourhood teams. This is a great opportunity to join Tameside MBC whilst we are undergoing a vast period of change.
The role ensures that and includes the following;
- Service users have access to approachable, responsive information and advice which is proportionate to their needs, often undertaken following a home visit.
- The service offers advice, information and redirection to sub-threshold services whilst incorporating the well-being principle of The Care Act with a focus on Early Intervention and Prevention.
- To identify eligible care needs under the Care Act and identify if a joint social care or health assessment is required from a Social Worker, Physiotherapist, Occupational Therapist or Nurse. Supporting with Hoarding and management of tenancy where required and/or required.
- To manage a caseload of vulnerable adults, prioritising need depending on urgency of referral.
- To support with individual tenancies, and support with service users’ financial management
This is not a domiciliary or personal care role; this is supporting people in a person-centred support role.
You will receive regular supervision, support and training to ensure your continuous professional and personal development.
About You
Experience of working with adults and previous employment in a similar position would be desirable.
It is essential that you hold a current valid driving licence and have access to a vehicle for work use.
