Enhanced Clinical Practitioner (Community)
We currently have an opportunity for a dynamic, highly motivated, innovative, and forward-thinking, fully qualified Enhanced Clinical Practitioner (ECP) to join our newly developed Acute Home Visiting Service which forms part of the Team Up Derbyshire partnership. This is an exciting time to join our workforce based in the High Peak Derbyshire.
This permanent position would appeal to enthusiastic, motivated, and proactive individuals who are keen to further develop their skills and be an integral part of a developing primary care workforce.
The post holder will be an integral part of the Ageing Well team, working as and when required across High Peak & Buxton.
We are seeking practitioners with enhanced skills . It is essential that you have clinical decision modules as well as being a non medical prescriber (V300).
We are particularly interested to hear from practitioners with a focussed interest in the areas of neuro-diversity, (including autism/learning disabilities/ mental health) or palliative care.
Full-time, 37.5hours

The High Peak PCN is working collaboratively with Team Up in our area and this is a really exciting time to join a new initiative with opportunity to learn and develop new skills.
You will work as a key member of the multi-disciplinary team, working alongside General Practitioners, Operational Lead, Advanced Nurse Practitioners, Trainee Advanced Nurse Practitioners, Paramedics, Trainee First contact Practitioners and Adult Liaison Officers, to provide holistic, pro-active care planning.
We want to recruit clinicians who are passionate in supporting and empowering patients and their carers to manage their complex health needs by optimising health and accessing support when needed.
Working as part of an MDT, the post holder will support GPs by making urgent visits and providing support to housebound patients in the High Peak.
They will initiate appropriate care and effective treatment to patients in a pre-hospital environment, selecting and applying appropriate skills and equipment safely and (within the scope of their practice) support the progression of care plans of patients with long term medical conditions, working to improve the health and wellbeing of the people of the High Peak.
The job holder will be required to meet the travel requirements for this post and travel across the High Peak.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
