Scope of Work

Supports projects throughout the organization including both clinical and non-clinical areas to accomplish measurable quality improvement. The Patient Safety & Quality Project Manager leads, manages, and supports system wide projects to improve quality outcomes and reduce safety events across the Bronson System.  The Project Manager must be experienced in leading teams and able to facilitate system-wide transformation and culture change to produce optimal outcomes.  This individual works with multidisciplinary teams, including physicians and executives to evaluate, assess and plan for improvement.  The Project Manager also measures and evaluates the data to determine effectiveness of change.

• Bachelor's degree (BSN or BS) and three to five years of experience including clinical nursing experience in an acute care setting.
• 1-2 years of experience specific to leading teams is required.

1. Processes, sorts, assembles, and distributes contaminated and non-contaminated instruments and equipment to construct clean accurate sets which are in good repair by using special techniques and procedures of Universal Barrier Precautions.

• Follows procedures for manual cleaning process.

• Follows procedures for automated cleaning process.

• Proper use of chemicals and knowledge of Material Safety Data Sheet (MSDS) forms.

• Practices Universal precautions, Infection Control policies and observes safe work practices.

• Communicates to appropriate personnel regarding sharps, punctures and other potential risks.

• Operates decontam machinery in strict accordance with departmental procedures.

• Adheres to using personal protective equipment (PPE) while working in decontam.

2. Distributes instruments sets to ensure receipt by departments in a timely manner as directed by supervisory staff, as requested by customers and as deemed necessary by technician.

• Deliver sterile cart to Operating Room (OR)/Outpatient Surgery (OPS) in a timely manner.

• Accurately delivers sterile items in the correct location as designated in Sterile Processing Department (SPD).

• Responds to Operating Room (OR)/Outpatient Surgery (OPS)'s requests in a timely manner.

3. Prepares surgical instruments and related supplies to ensure surgical instrument sets are assembled accurately and completely by following recipe card.

• Precisely follows individual recipe sheets for each set/tray/item being assembled.

• Replace lost or damaged instruments as required to complete the set/tray.

• Conducts quality checks on instruments for cleanliness, function, defects, breakage and overall condition.

• Flushes all instruments with lumens or cannulated.

4. Keeps accurate records of sterilization and of all equipment in the department to comply with federal and other external regulations by following guidelines in department policy and procedure manual.

• Maintains Sterile Processing Department (SPD)'s equipment service log.

• Maintains accurate sterilization records/documentation.

• Follows protocol for scanning bar coded items.

• Scanning items to the proper stations and locations.

• Sterilization records entered accurately into the sterilization module.

• Consistently records accurate load control numbers.

• Consistently verifies and initials all records according to policy.

5. Performs sterile linen processing to ensure availability of sterile linen, by following established guidelines.

• Correctly assembles and wrap linen packs according to guidelines.

• Keeps sterile linen stocked to the set par levels.

• Proper wrapping techniques used when using non-woven wrapper.

• Proper packaging techniques used when using paper peel pouches.

• Proper techniques and supplies used when using rigid containers.

6. Loads and unloads sterilization carts to ensure smooth and timely processing and delivery of sterile instruments and supplies by carefully and properly handling items on the cart as outlined in department procedure manual.

• Operate sterilizers in strict accordance with departmental procedures.

• Followings policy and procedures for Ethylene Oxide sterilization.

8. Performs all extra assigned duties for quality assurance. Checking and auditing sets prepared in assembly for  accuracy, cleanliness, and that instruments are in good working order, by auditing Utilizes the SPD audit sheet to track accuracy and performance.

• Responsible for communicating and applying quality assurance measures for the Central Service department while conducting surgical tray audits.

• Collects and analyzes statistical data as it pertains to sterile procedures and assists in corrective action based on analysis.

• Supports process improvement. Evaluates current and new regulatory criteria for compliance as it applies to industry standards

Job Description

Essential Functions:

• Demonstrates leadership and guidance to staff to ensure that the quality assurance plan, policies, processes, and procedures are established, current, and followed by all laboratory staff.
• Coordinates a program of auditing to find deviations, procedural drifts and non-conformance to policies and operating procedures. Successfully tracks and trends product/service non-conformance and its possible consequences. Works with supervisors/ section leads to develop improvement plans if required. Implement tools to ensure sustained improvement is occurring, audits those tools to ensure sustained improvement. Establish and maintain improvement plans as necessary.  Participate in planning, development, implementation, and maintenance of departmental monitoring tools (i.e. dashboards, productivity). Prepares and reports all findings to the Laboratory leadership team and committees as required.
• Prepare reports and oversee departmental documentation in accordance with various regulatory agencies, including but not limited to AABB, CAP, SDDOH, TJC and CMS. Maintains procedures to ensure appropriate and immediate response to and resolution of all issues and concerns. Responsible to investigate and respond to concerns brought forward (complaints, errors, accidents, or concerns) as appropriate. Monitor occurrence/incident reports and report findings to laboratory management. Oversee plans for improving processes and workflow to ensure patient safety and quality of services provided.
• Participate in cost control activities by seeking the most effective method of delivering high quality services and maintains an awareness of the cost impact of decision and actions. Ensures that operational controls are in place to assure the provision of high quality and cost-effective services. Will proactively identify opportunities for, and participate in, ongoing performance and process improvement activities relating to business performance. Assists senior laboratory leadership with technical aspects of goals, objectives, tactics, and long-range planning of the laboratory.

Job Summary: The Behavioral Health Quality Manager is responsible for the organization's performance improvement activities related to the quality of patient care and safety. Manages regulatory agency compliance functions consistent with administrative, medical staff and board approved plans. Ensures data collection and reporting meets state, federal and regulatory agency guidelines. Manages policy and procedure development to support staff training and educational activities to achieve the organization's quality of patient care standards and compliance with regulatory standards.

Minimum Education: Bachelor's degree required. Master's degree in Social Work or other clinical area preferred.

Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License, Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW), preferred.

Work Experience: 5 years of leadership experience in healthcare field, preferably in behavioral health.

Knowledge, Skills and Abilities: Ability to utilize assertiveness, political intuitiveness, and be self- motivated. Effective interpersonal, written, and oral communication skills. Ability to analyze complex issues and solve them creatively. Strong leadership skills relative to motivating and developing multi- disciplinary teams to be successful. Demonstrated training in performance improvement techniques and methods. Ability to organize and prioritize work in an effective and efficient manner.

Essential Functions and Responsibilities: Collaborate with physicians and organization leaders to design programs to improve quality and safety of patient care. Direct implementation, administration, and analysis of clinical outcomes and service data trends; compare performance to established standards; identify variances and undesirable performance; recommend and monitor strategies for improvement; report findings and make recommendations to appropriate oversight bodies, including medical executive committee, quality improvement committee, and patient safety committee. Oversee core measure abstraction and ensure timely submission. Develop, implement, and monitor hospital-wide continued readiness activities for regulatory survey preparation, follow-up, tracking and reporting to assure continual compliance with Joint Commission standards, as well as federal and state regulations such as CMS, OK DHS, OK DHS Office of Juvenile Oversight, OK Dept of Health, and ODMHSAS. Maintain accurate documentation and ensure timely submission of information as required by regulatory agencies. Oversee annual Joint Commission self-assessment process (i.e. FSA, ICM). Manage process to ensure entity specific policies and procedures are kept current and in compliance with relevant standards of care and regulations. Interfaces with Epic IT team to develop documentation and workflows to monitor and evaluate the quality and appropriateness of care furnished. This includes: intake/admissions screening, psychosocial assessment/reassessment, high social risk case finding, continuity of care, discharge planning, and exchange of appropriate information with sources outside the hospital. Collaborates closely with manager of nursing and other clinical managers to ensure staff training and educational activities ensure organization's quality of patient care standards and compliance with regulatory standards. Manages the variance reporting system in Verge, including grievances and complaints, to investigate and implement corrective action plans to improve quality. Coordinates the physician peer review process in conjunction with the executive manager and medical executive committee. Manages human and material resources. Budget plans are completed in a timely fashion with analysis of cost reduction. Continuously monitors overall utilization of resources and Manages corrective action related to fiscal management. Collaborate with entity clinical leadership and provide consultative services to other system behavioral units regarding state and federal regulations, data collection, submission and reporting. Conduct regular visits to offsite locations to ensure safety and quality processes are established. Interface with the credentialing coordinators at Warren Clinic and Saint Francis Health System to ensure hospital privileging and provider enrollment is conducted in a timely and accurate manner to ensure all regulations and provider contract requirements are met. Serve as the LPCH administrative point of contact for the LPCH Medical Executive Committee and Credentials Committee.

This Department drives Quality, Safety, and Experience (QSE) initiatives for their local hospitals set forth by regional and systemwide QSE initiative and goals.  This department works with Executive teams, clinical teams, Directors, Managers, and Medical staff to analyze data and outcomes in order to drive quality improvement work that is aligned with our QSE goals.  The QSE department implements and operationalizes best practices and quality improvement methodology to achieve desired Quality and Safety Outcomes. Hybrid with 3 days/week on site minimum. Travel to facilities in Metro Detroit is REQUIRED.

Scope of work

• Provides guidance and feedback to improvement teams and leaders on skill development and team effectiveness. Actively participates and contributes to System-wide training courses on quality, safety, and experience.
• Creating, maintaining, communicating, and executing program goals, strategy, and roadmap. Liaisons with executive team within the regional location. Ability to evaluate performance of location in the context of the overall Corewell Health performance in QSE. Can bring insights and best practice to advance the performance of the regional location.
• Maintain effective business partnerships to ensure goals and understood and work is prioritized appropriately to achieve those goals. Use of data and influence to highlight strengths and success as well as opportunities for improved performance. Ability to coach towards excellence and overcome barriers to achieve outcomes.
• Coordinates and integrates work of improvement teams members throughout their hospital or other designated area while maintaining a visible leadership presence locally and within the corporate team, as appropriate. Connects with regional and system functional leadership to stays abreast of new developments in healthcare quality, safety, and experience and provides updates and reference material to the Spectrum Health Quality Committees including board subcommittees as required.
• Participates with system staff to implement clinical outcome measures for quality improvement, patient and staff safety, patient/family experience, cost and complication reduction, benchmarking, and the implementation of evidence-based medicine.

Qualifications

• Required Bachelor's Degree in healthcare administration, business administration, nursing or related field
• Preferred Master's Degree MPH, MBA or other related area
• 10 years of relevant experience Performance management, quality improvement, patient safety, patient experience and outcome reporting Required
• 5 years of relevant experience leadership role in a healthcare environment. Required
• Creating, communicating, and executing on vision, strategy, and roadmaps, building relationships with diverse stakeholders, and leading and influencing cross-functional teams in creating successful initiatives, without formal authority. Required
