• The technician is trained to work in a strictly operator dependent modality (sonography).

The Lead MIT works closely with cardiologist, clinicians, technologists, nurses, clerical staff, logistics, leadership, biomedical engineering, venders and IT to assure quality control, efficient coordination of services, high quality diagnostic exam results and patient satisfaction.
The services performed by the Lead MIT include the performance of routine and emergency 12-lead electrocardiograms (ECG), and treadmill exercise testing with 12-lead electrocardiographic monitoring. Duties also include placing cardiac rhythm recorders on patients which provide a continuous magnetic tape of patient's ECG recordings (Holter Monitoring), scanning these recordings into the electronic medical record, analyzing data using CardioDay software and providing physicians with a report regarding ECG abnormalities/events and corresponding symptoms documented by the patient in the patient log/diary
The Lead MIT ECG, Holter, Exercise Laboratory is also responsible for assigning ECG tracings to physicians for reading (MUSE application), entering interpretation edits into the MUSE application (for ECGs read on paper) finalizing the edits made in a timely manner. The duties also include correcting end user ECG demographic errors, eliminating duplicates and providing detailed reports to Cardiology Nurse Manager/Section Chief pertaining to MUSE issues, early identification of "back-log" issues and concrete feedback with regards end user issues. The Lead MIT at this level provides direct staff education and training as it pertains to obtaining a 12 lead ECG and Exercise Stress Tests.

Electrocardiograms:
1. Processes request for electrocardiograms as needed in clinic setting.
2. Able to perform and instruct others in the performance of 12-lead electrocardiograms. Must be familiar with common technical problems which require adjustment to the equipment to ensure a satisfactory quality tracing
3. Able to recognizes machine malfunction and able to make appropriate adjustment and minor repairs.
4. Able to assist with the editing and processing of routine tracings.
5. Uploading tracings with physician edits to VistA Imaging and assigning ECGs to physician MUSE in-box for reading/interpretation.

Holter Monitor Recording and Analysis, and Cardiac Event Monitoring
1. Able to apply the leads for the tape recorder which records electrocardiograms for predetermined amount of time as ordered by physician. Able to provide instruction the patient on the monitoring devices use and maintenance including of the patient diary during the recording period.
2. Able to analyze the tape recording using a sophisticated computerized scanner and software. The individual must have sufficient electrocardiographic knowledge to recognize arrhythmias, confirm or correct the computer diagnosis and select for printing appropriate portions of the electrocardiogram for analysis by the Cardiologist.

Exercise Testing:
1. The Lead MIT can evaluate the consult/request for test and the patient's medical record and determine appropriateness of non-thallium exercise stress test request. For requests that are not appropriate, the Lead MIT is expected to contact the provider requesting exercise testing and explain the issue. If there is a question of test appropriateness that is not clear the technician will consult an attending cardiologist.
2. The laboratory performs bicycle ergometer and treadmill exercise testing with or without thallium scanning. The test is performed with continuous ECG (electrocardiograph) monitoring. The Lead MIT applies the leads to the patient and operates the equipment.
3. The Lead MIT can operate the treadmill, the bicycle ergometer and monitoring equipment using a variety of protocols according to the request of the Provider supervising the test. He/she must be able to vary the testing protocol according to the clinical condition of the patient as the test proceeds.

A SUPPLY TECHNICIAN, GS-2005-7, - Responsible for all phases of DLR and CLIX repair/repairable items procurement and returns program. Forecasts, justifies, prepares, submits, and administers the annual DLR budget request. Responsible to acquire needed DLR and CLIX repair parts. Verifies availability and orders DLR parts as needed. Determines proper fund coding, prepares requisitions and forwards to source of supply. Insures part availability, receives part, stores, prepares Material Release Order (MRO), and forwards part to maintenance shops. Recovers damaged/unserviceable part, initiates turn-in procedure, prepares for shipment, packages and ships to depot, Aviation Classification Repair Army Depot (AVCRAD), or other appropriate activity. Maintains Document Control Register and ship-out log. Verifies that appropriate activity has received returned part.
-- Assists/performs the Property Book operations in the area of inventory and storage management, and equipment coordination. Ensures the property book sections periodically perform a visual inventory of all items. Maintains property book accountability utilizing an automated or manual supply accounting system. Establishes hand receipt files and maintains accountability by the use of hand receipt listings. Assures that all authorized property is on hand or has been requisitioned. Coordinates the assignment and transfer of inventory.
-- Analyzes supply transactions or provides customer service for urgent critical shortage items and items requiring special handling when established procedures are not applicable. Codes and classifies requisitions, shipping orders, and other documents; extracts requests to other sources of supply or refers them to inventory control point when stock is unavailable or cannot be shipped.
-- Within funding limitations determines stock replenishment levels for centrally controlled items. Assures that supplies and monies are not wasted through excess accumulation.
-- Establishes procedures for management of calibration, testing of TMDE, and monitors tool room operations, to include accounting for, issuing and replacing tools.
-- May oversee subordinates, when organization and workload does not support a full time supervisor, that are performing material and resource tracking (i.e., DLR, CLIX, and related budgets), special TDME/tool transactions and hazardous material accountability.

Exploitation, Maintenance et Continuité de service

• Elaborer le calendrier de maintenance préventive des infrastructures, mettre ce dernier en œuvre avec l’ensemble des prestataires concernés, superviser de façon rapprochée les opérations avec le souci permanent de la minimisation des risques lors des interventions, et veiller au reporting de cette activité

• Exécuter les contrôles et diagnostics techniques réguliers des équipements d’infrastructure

• Effectuer les consignations électriques, mécaniques et hydrauliques

• Définir, constituer et inventorier des stocks de pièces de rechange critiques

• Organiser, superviser et valider les travaux des entreprises extérieures

• Être le garant, du bon fonctionnement des systèmes de gestion technique centralisée et d’alarmes techniques

• Piloter les prestataires de maintenance

PURPOSE AND SCOPE:

Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Patient Related

Education:

• Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.

Treatment:

• Welcome assigned patients and inquire as to their wellbeing since their last treatment.

• Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.

• Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment

• Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).         

• Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.   

• Monitor patients’ response to dialysis therapy. 

• Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.       

• Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure

• Obtain Hemostasis and apply appropriate dressings.   

• Evaluate the patient prior to discharge.

• Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification        

• Responsible for calculating and entry of individual patients’ dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.          

• Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.    

• Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification      

• Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.         

• Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.

• Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.

• Perform and document any intervention for unusual patient status and document patients' response to intervention.

• Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.

• Obtain all prescribed laboratory testing and prepare specimens for collection.      

• Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.       

• Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.       

• Monitors patients performing self-care under the supervision of RN.

Staff Related

• Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.

Education/Communication:

• Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices

• Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.

• Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.

• Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).

• Initiate Solution Delivery System (SDS) system.

RECORD KEEPING:

• Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.  

• Enters all treatment data into the designated clinical application in an accurate and timely manner.           

• Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure

omitted entries are completed or corrected by appropriate staff.

• Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.

• Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.      

• Collect, label, appropriately prepare and store lab samples according required laboratory specifications.     

• Ensure collection of lab specimens by appropriate lab courier.

         

INFECTION CONTROL:

• Assists in collecting  information for infection control audits.       

• Supports staff and patient Adherence to infection control practices.         

• Follows infection control Policies and Procedures          

• Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.

         

TRANSITIONAL CARE UNIT:

• Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care 

• Reinforces all education and care related matters as it relates to the new patient           as allowed by state law

• Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.      

• Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.

• Sets and preforms a treatment on all machines used in the transitional care unit.

• Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.

• Assists with coordination of transition to patient’s modality choice.         

• Assists with supply ordering and inventory for TCU

The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force.
