This position is located the Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Mid-Atlantic Health Care Network (VISN 6), Fayetteville NC VA Health Care System (FNCVAHCS). The successful candidate will serve as a Neurologist under the Medicine Service Line. The incumbent will ensure the quality and timeliness of the services provided, customer satisfaction, as well as compliance with the standards of accrediting bodies such as the Joint Commission and the Inspector General's Combined Assessment Program. The incumbent will ensure appropriate use
of the available resources and will be responsible for the state-of-the-art standards of care. The incumbent collaborates with other services as appropriate.

The Neurologist is accountable to the Chief, Medicine Service, for assuring the most effective delivery of care in Neurology while meeting and exceeding compliance standards. The Neurologist will provide expert consultation and treatment to patients referred to the outpatient and inpatient Neurology Service and provide feedback and consultation to other Neurology Practitioners within the Neurology section. In this role, the incumbent will also provide feedback and patient education to the patients and their families and will demonstrate excellence in all facets of patient care and meets or
exceeds compliance standards.

The Neurologist is responsible for patients in the outpatient general neurology consult and follow-up clinics and providing inpatient consultation to the acute medical and intensive care units. The incumbent will provide consultative evaluations and treatments to patients referred from sections of the hospital and/or affiliated VA outpatient clinics and perform administrative functions and other tasks as assigned by the Chief of the Medicine
Service. The outpatient clinics will consist of both in-person and virtual clinics. Patients seen in the Neurology Consult Clinic are evaluated and treated by the Neurologist for disorders such as Headache, Peripheral Neuropathy, Carpal Tunnel Syndrome, Neck/Back Pain, seizures and Tremor. The Neurologist will perform neurologic procedures, such as NCS/EMG; EEG interpretation; and/or Botox administration, commensurate to his/her individual training and expertise, in accordance with local VA credentialing and privileging requirements.

• Responsible for entering accurate, complete, timely progress notes for each episode of care in the patient's electronic record. This is accomplished by utilizing the Electronic Health Record (EHR).

The Office of Mental Health and Suicide Prevention (OMHSP) in the Veterans Health Administration (VHA) is responsible for providing strategic guidance directed toward improving the quality of mental health care for over one and a half million Veterans nation-wide. The Suicide Prevention Program (SPP) is focused on the full implementation of VA's National Strategy for Preventing Veteran Suicide (2018) and the Department of Veterans Affairs and Department of Defense (VA/DoD) Clinical Practice Guideline for Assessment and Management of Patients at Risk for Suicide. Suicide Prevention (SP 2.0) Community Engagement and Partnerships Coordinator (CEPC) positions focus on connecting, developing, and coordinating suicide prevention efforts at the Veterans Integrated Service Network (VISN), state, local, and federal level.

• The Pharmacist recommends appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics, monitoring for efficacy, side effects and clinical outcome; and after review with a Clinical Pharmacist, advises prescribers as appropriate.
• In conjunction with Clinical Pharmacist review, provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendation is to other health care providers in charge of the patient both orally and in writing. Monitoring will include prospective review and intervention in:

• Performs continuous evaluation of prescribed medications to assure optimal drug therapy.
• Provides medication counseling to patients.
• In conjunction with a Clinical Pharmacist, serves as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients and caregivers.
• Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS reporting program.
• Recommends formulary alternatives as appropriate.
• In conjunction with a Clinical Pharmacist, provides pharmacokinetic consultation, including dosing vancomycin and aminoglycosides, in accordance with local policy.
• Participates in quality assurance programs through submission of medication errors.

• Routinely reviews par levels in automated dispensing cabinets to ensure appropriate stocking and mitigate stock-outs.
• Works with Automation Dispensing Machine Pharmacy Technicians to ensure medications are appropriately stocked in outpatient clinic locations per PBM approvals.
• Assist with medication addition request to automated dispensing machines for inpatient and outpatient use (to include CBOCs). Highlighting in the process if medication request is a high alert medication and ensuring appropriate transportation of high alert medication is followed.
• Takes ownership to ensure contingencies are in place for routine and scheduled power and system outages that impact pharmacy automation equipment to include medication storage refrigerators.
• Assist with monitoring the facility temperature monitoring system, and addressing any temperature excursions per policy.
• Assist with reviewing discrepancy, override, and null transaction reports for automated dispensing machine and addresses issues with area nurse managers and pharmacy supervisors as required by local policy.
• Assist with adding and removing users into the Automated Dispensing Machines per policy.

The primary goal of HUD-VASH is to move veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve veteran's health and mental health, enhancing their ability to remain stable, housed and community integrated. The Social Worker is responsible for providing case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA Health Care System, community health or mental health providers, and agencies that serve homeless populations. The Social Worker independently provides clinical psychosocial and case management services at an advanced practice level for eligible veterans and their family members in the program. Veterans eligible for HUD/VASH come from a wide geographic area. Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving. Services are provided in healthcare settings and in the community. The HUD/VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies.

Duties include but are not limited to:

Program Administration and Organization- The Social Worker is responsible for data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements. Additionally, the Social Worker develops working relationships and agreements with other organizations and directly coordinates with the designated PHA regarding housing voucher application and award process for each veteran. The Social Worker is responsible to review the agreements between Section 8 property owners, PHAs, and the VA to establish appropriate referral sites for placements.

Clinical Assessment- The Social Worker evaluates the veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion including an assessment of vulnerability and prioritization for admission. The Social Worker assesses at-risk factors and develops a preliminary plan, involving the veteran and family or significant others, and performs an insightful assessment of serious and complicated cases involving psychiatric illness which may also include, catastrophic medical condition, dementia, traumatic brain injuries, and other high risks diagnoses.

Referral, Rehabilitation, and Program Admission- The Social Worker provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. If appropriate, the Social Worker provides referrals or consults, or in some cases direct admission of the veterans prior to HUD/VASH participation, for other VA programs including community-based residential treatment such as Grant and Per Diem (GPD) Programs, Health Care for Homeless Veterans (HCHV) contract residential treatment, Domiciliary or Psychiatric Residential Rehabilitation and Treatment Programs (PRRTP), or Compensated Work Therapy (CWT) and Compensated Work Therapy Transnational Residence (CWT/TR)Programs.

Comprehensive Case Management- The Social Worker develops psychosocial treatment plans, actively involving the veteran and their family or significant others, in coordination with interdisciplinary team members, based on the psychosocial assessments, including goals for clinical treatment. Using known resources, the incumbent makes initial and continuing decisions coordinating the veteran's care through linking and referral to VA medical facilities or Regional Offices and other VA and non-VA services and provides independent consultation and makes recommendation to the interdisciplinary team on course of action.
