An exciting opportunity has arisen for an experienced and enthusiastic pharmacist to join our friendly and forward-thinking team at Chesterfield Royal Hospital NHS Foundation Trust. We are seeking a pharmacist who is highly motivated, innovative and dynamic to lead the pharmacy clinical services within the Diabetes and Endocrine speciality.

Pharmacological management of diabetes is a continually evolving field and this is the perfect opportunity for the post holder to develop an advanced practice role within the MDT to improve medicines safety within this speciality.
• You will undertake a lead pharmacist role working closely with the endocrine multidisciplinary team. You will work clinically on the endocrine ward using prescribing skills in this specialist area and develop your own portfolio of advanced practice.
• You will support the education and training agenda in upskilling trust staff in their knowledge and practice relating to safe use of insulins.
• You will be involved in developing and modifying the Trusts new electronic prescribing system to support safe governance of prescribing in this high risk area.
• You will also be expected to work with partners within Joined up Care Derbyshire in expanding the safety agenda relating to diabetes for patients as they leave the hospital.
• You will also proactively contribute to the wider medicines safety agenda.

We are very committed to the development of pharmacist prescribing and have one of the highest numbers of active pharmacist prescribers in the country.

These are the key areas that support the purpose of the job

1. To lead the provision and development of clinical pharmacy services within diabetes and endocrine speciality and support the Principal pharmacists in the provision of the wider divisional clinical pharmacy services

2. The Pharmacist should:

• Lead the processing of requests for new medicines within the speciality and care unit

• Advise on the review of the Medicines Formulary as it applies to the speciality

• Facilitate the development and implementation of medicine-related guidelines

• Undertake the Pharmacy aspects of the induction of new medical staff in the specialty

• Lead the auditing of medicines in the specialty

• Attend appropriate specialty meetings to ensure that pharmaceutical issues are considered this includes care unit governance

• Lead the review of medicines related incidents within the speciality and Care Unit

• Monitor and investigate Controlled Drugs (CDs) governance issues within the Care Unit

• Lead pharmaceutical developments within the Care Unit

• Monitor interventions in treatment to identify training and information need

• Provide training to Care Unit/ speciality staff

3. To provide expert advice and information on new drugs and new treatment strategies in area of responsibility by evaluating and interpreting published evidence

4. To monitor information on drug expenditure/prescribing patterns and advise consultants, senior pharmacy managers and general managers on clinical economy within area of responsibility

5. To monitor compliance and advise on the implementation of National Guidance e.g. NICE

6. To advise the speciality management team on service and product developments and to contribute to the planning and implementation of any changes in practice

7. To undertake prescribing within a defined speciality, and as part of wider medicines optimisationinitiatives undertaken by the clinical pharmacy service.

8. To undertake clinical pharmacy practice at an advanced level.

9. To train and counsel patients in the self-administration of medicines in defined circumstances

10. To provide training to Trainee Pharmacists and Clinical Pharmacists, including tutoring for the MSc/Diploma in clinical pharmacy and being a Designated Supervisor

11. To participate in clinical audit and other initiatives aimed at assuring high quality prescribing and pharmacy practice, within the context of Quality Governance

12. To provide training and education to other healthcare professionals

13. To provide professional supervision to technical and supporting staff and to undertake the role of Responsible Pharmacist when required

14. To contribute to the delivery and development of clinical pharmacy services across the Trust

15. Actively participate within the multi-disciplinary and clinical pharmacy team

Discharge: Take a proactive approach to the management of ward discharges, to include prescribing of discharge medicines in line with local guidance Work collaboratively with medical and nursing teams and Trust Operations Team to facilitate and expedite discharge Ensure high quality patient counselling is delivered by the MDT during admission and at discharge Support communication to GPs , community pharmacy and other primary care services Medicines optimisation: Undertake medicine optimisation activities in line with clinical specialty Provide clinical pharmacy support to consultant ward rounds, MDT meetings and ward operational meetings Work with prescribers and nursing teams to ensure high standards of medicines use including development and review of relevant guidelines Review & investigate medicines incidents and liaise with the ward team to adopt changes in practice where necessary Take a lead role in managing ward based medicines expenditure, to include: Ensuring multidisciplinary ward team take a thorough approach to obtaining patients own medicines from home Ensure the ordering and supply of medicines receives appropriate scrutiny Provide and present reports to the MDT , ward team and pharmacy in relation to medicines expenditure Contribute to the pharmacy clinical economy group agenda To undertake independent prescribing practice to patients within the specialty in accordance with agreed objectives / guidelines To contribute to the delivery and development of clinical pharmacy services across the Trust Actively participate within the multi-disciplinary and clinical pharmacy team To act as a source of expert advice on prescribing and medicines optimisation to medical and nursing staff within a defined clinical area To provide supervision, training and tuition of pre-registration students and pharmacists, including tutoring for the MSc/Diploma in clinical pharmacy. To participate in clinical audit and other initiatives aimed at assuring high quality prescribing and pharmacy practice, within the context of Quality Governance To provide training and education to other healthcare professionals. To provide professional supervision to technical and supporting staff and to undertake the role of Responsible Pharmacist when required
