Duties of the RN HCBS Case Manager include, but are not limited to:

• Uses knowledge of the home and community-based care program and VA Program to organize delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning.
• Applies nursing clinical knowledge/judgement to promote staff involvement in planning, decision-making, and evaluating Veteran needs and outcomes
• Coordinates care for Veterans receiving both internal VA and community-based home and community-based services including but not limited to skilled home care, non-skilled homecare, adult day health care and respite care. • Evaluates, systematically, current practice and formulates outcomes for groups of patients, and/or organizational processes within area of expertise.-Works with inter professional team members at VISN and facility levels to ensure services and eligibility determinations are delivered objectively with strong, consistent reliability with documentation and communication of outcomes in a direct and coordinated manner.
• Conducts comprehensive clinical review of patient records to identify the presence of Veteran medical necessity for services.
• Evaluates and documents Veteran progress and changing needs.
• Executes position responsibilities that demonstrate leadership, experiences, and creating approaches to management of complex patient care.

Major duties include, but are not limited to:

• Incumbent is responsible to the Supervisor Laundry Operations Specialist for assuring that the work assignments of the other employees of the section are carried out.

The Community Care Referral Case Manager (RCM) Registered Nurse duties include, but are not limited to, the following:

• Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.

The Nurse Manager of Fort Meade VA Community Based Outpatient Clinic is a Registered Nurse with 24/7 accountability for nursing care by licensed and unlicensed staff. S/he is responsible for the day-to-day operations ensuring patient and staff safety in compliance with VHA, VISN, and VAMHCS policies/procedures/nursing protocols, accepted best practices, and professional standards of care. S/he evaluates performance, provides annual competency training, and continuing education for direct reports. The Nurse Manager performs timecard certification, verifying staff schedules and hours worked. S/he practices fiscal responsibility within the budgetary guidelines. S/he is responsible for the implementation of the organization's Vision, Mission, and Values. The Nurse Manager consults with and reports to the Chief Nurse of Ambulatory and Emergency Care Clinical Center. The Nurse Manager is an essential leader in the clinic overseeing the clinic operations as well as ensuring safe and quality patient care and excellent customer service. The Patient Safety Program Manger (PSM) Registered Nurse (RN) is responsible for trending and analyzing incidents across the facility; participating in the Patient Safety Committee; interpreting VA handbooks and directives regarding Patient Safety; and monitoring of the quality and progress of root cause analyses and follow-up actions to ensure implementation of system changes. The Patient Safety Program Nurse develops strategies to disseminate "lessons learned" and will be the content expert for the VA Maryland Health Care System. He/she participates in the development of innovative approaches to the establishment, enhancement, modification, and /or improvement of care with a focus on Patient Safety.

The PSM coordinates and implements patient safety policies and activities developed by the National Center for Patient Safety (NCPS) and is the facility's key person to help build and implement patient safety systems and promote the culture of safety within Veterans Health Administration (VHA) facilities and for Veteran patients in all healthcare settings.

The PSM has primary responsibility for training clinicians, management, and all relevant support personnel within their facilities, about the patient safety program and patient safety analyses processes.
