The purpose of this role will be to ensure safe medicines management across our organisation and partnering hospital and a hospice. The successful candidate will focus on medicines reconciliation, ordering stock and service user own medications, including TTOs. They will oversee destruction of medicines and work closely with other professionals to reduce waste and optimise treatment options. They will also be completing a range of audits to ensure the medicines management standards are met across the board. CLINICAL Accept referrals via agreed protocols within the service (including protocols for managing self referrals). Assess clients for suitability for CBT or other Talking Therapy. Make decisions on suitability of new referrals, adhering to the departments referral protocols, and refer unsuitable clients on to the relevant service or back to the referral agent as necessary. Formulate, implement and evaluate therapy programmes for clients. Adhere to an agreed activity contract relating to the number of client contacts offered, and clinical sessions carried out per week in order to minimise waiting times and ensure treatment delivery remains accessible and convenient. Attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate. Complete all requirements relating to data collection within the service. We meet monthly as a whole team alternating virtual and face to face meetings.


To support the provision of a community service to individuals with a learning disability in a defined area of Lancashire, in order that health care needs are met and contractual agreements fulfilled.

A registered learning disability nurse, with knowledge of health policy and legislation applicable to people with a learning disability and/or Autism. Adaptability, good communication skills and self–motivation are essential, alongside skills in working with people who have complex health needs, challenging behaviour and/or Autism, including demonstration that practice is evidence based and innovative.
