Main Duties include: · Lead on PCN screening improvement plan, with support of practices. · Help to improve PCN cancer screening uptake and reduction of DNAs. · Help to improve PCN childhood immunisations uptake by various measures including, but not limited to, directly calling patients, booking appointments and providing information to patients. · Work with local system partners including Public Health, Cancer Alliance, PPGs, ICN, etc. · Specific actions to engage with groups with low participation locally in screening and immunisations.

An exciting opportunity has arisen for a clinic receptionist to join the team in Cardiology at Croydon University Hospital.

The post holder’s duties will include making follow up appointments for various outpatients clinics, dealing with telephone and face to face queries and preparing documentation for clinics. The post holder will liaise with patients, GPs, consultants and other departments in the hospital.

The post holder will contribute to ensuring that an efficient and effective outpatient services is delivered to meet the needs of the patients.

Access to pre-operative anaesthetic assessment clinics

General anaesthesia with experience of elective and emergency surgery for ASA 1-4 patients,

Paediatric experience for children aged five and over and experience of TIVA

Regional anaesthesia with training in ultrasound guided regional techniques

Obstetric anaesthesia with on call experience in obstetrics including management of ASA 3 parturients

Resuscitation and transfer as part of the medical emergency team and involvement in intra-hospital transfers

Pain with access to pain management clinics and acute pain ward rounds

Intensive Care with three months experience in adult intensive care unit for those trainees with only three months completed at core level

Doctors in these posts will also be allocated named educational supervisors, ability to attend teaching sessions and departmental quality improvement meetings. They will be helped in preparing for the Primary or Final FRCA exam. The SLAP (South London Anaesthetic Primary) course exclusively run at Croydon has to date achieved very high pass rates.

JOB DESCRIPTION FOR THE POST OF HEALTH & WELL BEING COACH HEALTH CARE ASSISTANT One Thornton Heath Primary Care Network REPORTS TO: PCN Clinical Directors and Business Lead Job Summary: The role involves coaching and motivating patients in managing their own health and wellbeing, to live independently and improve their health outcomes. The post holder will work with individual and/or groups of patients and liaise with other healthcare providers. Working for One Thornton Heath PCN across six GP practices as part of their multidisciplinary healthcare professional teams, support and supervision will be provided by a GP Supervisor and/or a line manager. This is a key position within our developing PCN and appropriate training will be given. Responsibilities: · Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan. · Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare. · Empower patients to take more control in managing their own health and wellbeing, to live independently and improve their health outcomes through interventions such as: o self-management education and peer support o establishing and attaining goals set by the person based on what is important to them o working with the social prescribing service to connect them to community-based activities that support their health and wellbeing. · Provide support to local community groups and work with other health, social care and voluntary sector providers to support the patients’ health and wellbeing holistically. · Work with people with lower activation to understand their level of knowledge, skills and confidence (their “Activation” level) when engaging with their health and wellbeing. Main Duties include: · Help to improve the PCN Diabetes & Hypertension challenges and other local health problems as agreed by the PCN, with appropriate training/support. · Support group consultations (virtual as necessary) to support patients to better understand their condition, how to manage it and agree self-management goals. · Diabetes & Hypertension reviews (including bloods/immunisations) for housebound patients. · Liaise with other health care providers as necessary including community diabetes service, ICN, etc.

We have an exciting opportunity for a highly motivated and enthusiastic receptionist to join our friendly and busy team of health professionals in the Endoscopy Unit at Croydon University Hospital.

The successful candidate will be the first point of contact for patients and visitors to the department and will be expected to provide comprehensive administrative/clerical and reception services. We are looking for a candidate with excellent communication skills, the ability to work as part of a team and to prioritise/organise their own workload.

The right candidate will be sympathetic and sensitive to the requirements of staff, patients and other service users, whether communicating by telephone or face-to-face, including dealing with difficult or demanding situations.
