The Community Nursing Home (CNH) Coordinator Registered Nurse (RN) focus is on individual growth and development in practice with increased levels of self-direction. This nurse brings principles and theory with limited application of critical thinking skills. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. The CNH Coordinator assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. Coordinates care with multiple providers across all levels and sites of care. Coordination includes assuring services interface within the Veterans Affairs (VA) and CNH programs to provide continuity of care to patients.
Duties include but not limited to:
The nurse promotes the maintenance of health and prevention of illness and injury based on nursing processes, including systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through services such as case finding, health teaching, health counseling and provision of care supportive to or restorative of life and well-being. The nurse's primary commitment is to the patient whether an individual, family group, or group, or community. The registered nurse, as a licensed professional, will be responsible and accountable for their practice.
- Carries out evaluations with assessment of patient findings.
- Oversees patient plan of care with concurrence from the Provider and interdisciplinary team.
- Provides psych-social support to the patient and his/her family.
- Oversees treatment within parameters indicated in Veteran's treatment plan.
- Engages in interdependent decision making with the Provider and treatment team regarding the needs and treatment of Veteran for continuity of care.
- Evaluates treatment plans for effectiveness and institutes appropriate adjustments or corrections.
- Collaborates with CNH colleagues and Veteran's Primary Care Provider.
- Evaluates the patient's physical and emotional response to medications and treatments.
- Recommends appropriate measures regarding physical and social factors in the environment, which may affect patient care.
- Assists the patient and family in identifying and utilizing resources which lead to the best possible state of health for the patient.
- Provides coverage for other CNH nurses as necessary during vacation or sick leave.
- Demonstrates the skill and knowledge necessary to provide care appropriate to the age of the patients served, to obtain and interpret information to identify patient needs, and provide care.
- Maintains/updates current knowledge on home care nursing through continuing education activities.
- Work hours: as stipulated through policy.
- Adheres to hospital policy regarding ADP Security.
- Proficiency in the eight High Performance Developmental Model (HPDM) core competencies.
- Ensures residents are well cared for, safe and given appropriate care.
- Possesses the knowledge and skills necessary to interact appropriately and courteously with internal and external customers. Maintains confidentiality of patient information electronically, verbally, and written. Work and function within a safe manner including infection control and operation of equipment.
- Utilize nursing research and literature in daily practice.
- Possesses knowledge and skills necessary to provide care appropriate to age-related needs of the patient served, including understanding of the changes associated with aging and principles of growth and development relevant to the adult and geriatric population.
