About the Role As a Specialist Clinical Pharmacist, you will lead the delivery of clinical pharmacy and medicines management services to our Specialty Medicine wards, ensuring the safe, effective, and evidence-based use of medicines. Working as an integral part of the multidisciplinary team, you will use your advanced clinical skills and specialist knowledge to provide expert advice on pharmaceutical care. You will also play a critical role in improving the management of medicines-related issues, reducing iatrogenic admissions, and enhancing patient outcomes. Additionally, you will have the opportunity to shape prescribing practices by contributing to policy development, clinical audits, and education initiatives. Key Responsibilities Specialist Clinical Services: Deliver specialist pharmacy services to the Specialty Medicine Directorate, providing expert advice to consultants, medical staff, and nurses on all aspects of medicines use. Independent Prescribing: Undertake an independent prescriber qualification (where applicable) and practice as an independent prescriber for a defined cohort of patients. Patient Care: Construct and implement individual pharmaceutical care plans, ensuring patients receive optimal treatment. Monitor therapeutic progress, recommend treatment adjustments, and counsel patients with complex needs. Education & Leadership: Provide training and mentorship to junior pharmacists, pharmacy technicians, pre-registration pharmacy graduates, and other healthcare professionals. Medicines Optimisation: Lead medicines reconciliation, assess medication histories, and proactively identify and address medicines-related issues. Policy Compliance: Ensure adherence to national and local guidelines, medicines legislation, and the Trusts Medicines Policy. Audit & Research: Conduct clinical audits and contribute to research initiatives to improve practice and support innovation within the department. Why Join Us? Skills: Advanced clinical knowledge, strong problem-solving abilities, and excellent communication and interpersonal skills. Leadership Qualities: Experience in training, mentoring, or managing staff is desirable.
