This new role is a great opportunity for a candidate who wants to be part of a new service development and take on a clinical leadership role working with a diverse group of colleagues along with working clinically with people who present with varied and complex mental health needs. The Doncaster Primary Mental Health Hub has been growing since February 2023 and now has a rich mix of roles, including mental health practitioners, CAPs, MHWP working together with an integrated peer support and care navigation offer from voluntary sector organisations.
The post holder will provide clinical leadership to mental health roles across the Primary Care Networks (PCNs) and hubs. Within the primary care setting, the role holder will work in a collaborative and multi-agency way. They will work closely with secondary care colleagues too to develop and sustain a ‘No Wrong Door’ approach and access to the right service at the right time.
The post holder will work closely with the team manager and psychological therapies lead to further develop this team to deliver innovative ways of supporting people to improve their mental health and wellbeing and support recovery.
The post will facilitate the interface between Primary Care and Secondary Mental Health Services to provide expert clinical leadership and knowledge to the local Primary Care Teams, for patients and carers. The role will offer expertise in networking and navigation to promote ease of access to a range of specialist mental health support services.
The post holder will be an effective member of the multi-disciplinary team. They will be professionally accountable and responsible for patient care, clinical supervision and leadership of mental health staff. They will interpret policy and best practice guidance and apply it to the clinical based services, ensuring that they remain modern and fit for purpose.
