The Chief, Care Management and Social Work Service has full responsibility for provision of complex social work services requiring advanced practice skills as well as administrative responsibility for numerous programs such as Community Nursing Home, Respite, Contract Adult Day Healthcare, Veteran Directed Home Care, Advance Care Planning, Caregiver Support, Intimate Partner Violence Assistance Program, and Post 9/11 Military to VA. Additional program areas with coordination responsibilities lying within Care management and Social Work include the Visual Impairment Services Team, Polytrauma/TBI, and Spinal Cord Injury and Diseases, including Multiple Sclerosis and ALS.

The Chief is responsible for defining the mission and vision of the Service, developing the scope of practice for health care system social workers, designing a service business plan, and assuring implementation of quality improvement measures. The incumbent is responsible for developing, planning, clarifying, administering and managing Care Management and Social Work Services for all areas of the Marion VA Health Care System (HCS). The incumbent designates service priorities and objectives, ensures the supervision and evaluation of all service staff, and is responsible for supervising and evaluating the performance of all direct reports, including program managers, program coordinators, and social workers. The Chief makes independent professional decisions and recommendations. The incumbent serves as the liaison to other Services or care-lines, other facilities, VISN offices and VHA Central Office. This position also serves as liaison with Veterans Benefits Administration Offices, other government agencies, and the community. The incumbent is responsible for advising management of specific clinical or administrative issues within the Care Management and Social Work Service and within the Marion VA HCS. The position serves as a consultant, mentor, and advisor to subordinates, peers and administration. The incumbent is responsible for Joint Commission, CARF, OIG and other compliance for the Care Management and Social Work services at the facility.

The Chief of Care Management and Social Work Service serves as the facility's Social Work Executive and has professional and clinical responsibility for approximately 100 facility social workers, including one GS 14, eight GS-13 and twenty GS-12 social workers. This includes provision of program specific consultation related to social work, social work hiring and human resources actions, social work productivity measurement, staffing management, risk management and coverage, management of the social work trainee program, strategic direction for social work, and professional education.

This employee travels to the various sites and to other VA facilities, Veterans Integrated Service Network (VISN), or VACO as necessary.

Preferred Experience: One year of experience equivalent to the GS-13 grade level. Candidate must have strong leadership skills as well as evidence of supervisory and management skills. Individual may have certification or other post-masters degree training from a nationally recognized professional organization or university that includes defined curriculum/course of study and internship, or equivalent supervised professional experience.

This position serves as the Assistant Medical Center Director located at the Jesse Brown VA Medical Center within the Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Veterans Integrated Service Network (VISN), VA Medical Center (VAMC). The Assistant Medical Center Director exercises line authority for multiple administrative and allied health services and sections and is directly responsible for facility operations.

The Assistant Director shares with the Executive Leadership Team a level of responsibility for the direction and management of the Medical Center and is directly responsible for facilities operations. The Assistant Director plans, coordinates, supervises, and directs various administrative and health related services. The Assistant Director participates on a day-to-day basis in top level Executive discussions, decisions, policy making and actions of all administrative functions and indirectly of all clinical matters impacting patient care.

Duties include, but may not be limited to the following:

• Budget and Fiscal Management for the facility services assigned such as resource management and FTEE control within the assigned services.

The Clinical Resource Hub (CRH) is a VISN-based repository of clinical and administrative staff that serve Veterans Affairs facilities within and outside of a Veterans Integrated System Network (VISN). Staff within the CRH are expected to provide inter-professional services to VISN facilities that are underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Position is Telework Eligible.

Duties include, but not limited to:

• Develops policies and guidelines related to the practice and administration of CRH Tele-Pain Medicine.
• Oversees the administration of telehealth pain medicine and opioid safety review board electronic consultation services.
• Member of the local, regional and National Pain Management Committees.
• Reviews and reports to CRH leadership team regarding Opioid Safety Updates and collaborates with CRH Service Line Chiefs to address Opioid Safety Initiatives.
• Coordinates with the Service Line Chief to ensure that Telehealth equipment and supplies that are necessary for the efficient operation of the Tele-Pain Medicine Section are acquired and maintained per VA standards and protocols.

• Collaborates with the Service Line Chief to develop yearly Tele-Pain Medicine Section budget projections.

• Provides telehealth, face to face evaluations, and interventional pain medicine procedures in designated VA facilities as part of a balanced, data­ driven, multi-disciplinary treatment plan, in accordance to current licensing and standards of care.

The Associate Chief Nurse (ACN) provides senior leadership and oversight for daily clinical and administrative operations within nursing as directed by the Associate Director for Patient Care Services
(ADPCS.) The ACN supports the integration and collaboration of nursing with other professional
disciplines and functional areas in the mutual achievement of patient-centered and organizational goals. They champion a High Reliability Organization (HRO) with a culture of safety founded on a systems approach to developing evidence-based health care solutions based on prevention, not punishment, with collective mindfulness.

The Associate Chief of Staff, Geriatrics and Extended Care (ACOS/GEC) is responsible for providing a full range of geriatric services for a diverse group of aging Veterans within the Hines VA Hospital. These duties include but are not limited to referral consultation covering all issues of Geriatrics. The ACOS/GEC will perform as a consultant to our inpatient service, Emergency Medicine service, and others as appropriate. In addition, the ACOS/GEC will perform Geriatric Evaluation and Management and will develop treatment plans for our residents in the Community Living Center. In addition to excellent leadership skills, the ACOS/GEC will be knowledgeable and proficient with VA Performance Measures and the principles of System Redesign, demonstrate leadership qualities, as well as strategic planning skills, dedication to patient-centered care, and a commitment to organizational stewardship. Duties and Responsibilities of the Associate Chief of Staff, Geriatrics and Extended Care (ACOS GEC) will include but are not limited to

• Ensures clinical care is delivered in accordance with hospital and VHA policy, meeting or exceeding community standards and VHA national benchmarks.
• Responsible for the fiscal oversight of the Geriatrics and Extended Care Service
• Chairs the Hines Hospital GEC Committee.
• Serve as a member of the Medical Executive Board (MEB) and is responsible for advising staff members of the recommendations and implications of Medical Executive Board action as it concerns the service and Medical Center.
• Serves on hospital committees and task forces, both standing and ad hoc, as appropriate. Participates in local, VISN, and national level committees.
• Assist the Chief of Staff in the enforcement of the policies of the Medical Center Bylaws, and Rules and Regulations of the staff.
• Provides direct clinical and administrative oversight for medical students and Internal Medicine residents, and fellows from Loyola University Medical Center (LUMC).
• Develops performance metrics for all provider classification to drive performance and to direct desired behavior.
• Is in compliance with VHA Scheduling Directives to enhance the management of access.
