Senior Clinical Fellow in Emergency Medicine (ST4+) –80:20Wellness

The senior clinical fellow posts include 80% time working in the adult emergency department and 20% paid time to develop in a specialist interest of the successful candidate’s choice. The posts provide an ideal opportunity for the successful candidates to develop their own specialist interests while working in a busy inner city university hospital, with teaching and research responsibilities and are based in the adult side of the department, serving the Bristol Royal Infirmary (BRI).

The post will be a combination of service provision including out of hours and paid time to develop a specialist interest. Clinical

The reception, diagnosis and emergency treatment of patients presenting to the adult Emergency Department. The successful candidate will be given supervision and support from senior medical staff to develop competency in the autonomous clinical management of these patients whilst ensuring patient safety. This post is based in the adult Emergency Department and no sessions will be undertaken in the paediatric Emergency Department.

There is close liaison between ED and ICU at BRI. Patients referred to Specialist Units within the hospital are the responsibility of that specialist team, who will see the patient in the Emergency Department and arrange appropriate care. However, medical and nursing staff in the ED are responsible for any emergency care necessary. This shared care is seamless and collaborative.

Managerial

The management responsibility of the post holder will be to the Clinical Director who is responsible to the Chief Executive and Trust Board.

The post holder must be aware of clinical governance and clinical risk management and take an active part in their implementation, including audit. Clinical audit is established at the BRI. They will participate in at least one supervised audit project during the 12 month post and will be encouraged to attend interdisciplinary clinical governance meetings

On-Call Commitment

In ED the roster is a full shift system. Shifts are a balanced mix of earlies, mids, lates, and nights. The post’s pay will be calculated by HR and will be compliant with the new regulations set in Aug 2017.

Medical Examiner Service

Ensuring accuracy of the MCCD, discussion of care with the next of kin, and liaison with HM Coroner are our main functions, during which any governance issues arising are fed back constructively to the organisation concerned. Subsequent follow-up ensures our recommendations are established in future practice.

Emergency Department 80:20% Clinical Fellow Post – Simulation

Bristol Royal Infirmary Emergency Department offers Junior and Senior Simulation based clinical fellow jobs, 80% time working in the Adult ED and 20% SIM.

The SIM position of the post is directly managed and supervised by the Bristol Medical Simulation Centre allowing the post holder to undergo any basic or advanced Simulation training needed. The links to Simulation Services also allows for a really varied experience within the Trust, not only within the Emergency Department.

Within the Emergency Department, the successful candidate will join an enthusiastic and multidisciplinary teaching team with a wide range of clinical teaching and supervision skills.

The Emergency Department runs an active SIM programme with regular MDT, and monthly team centred training as part of the ‘in house’ Emergency Department programme. There also frequent MDT BMSC days which will resume as soon as Covid allows. Links to Regional Training and SW HEE Sim programmes would allow for experience there if desired.

While there is as much supervision as wanted, there is also plenty of time scope within this role on your own projects and ideas. There is ample time to aim for a publication, QUIP or other SIM based project.

Emergency Department 60:40% Clinical Fellow Post (ST4+) – Paediatric Blast Injury

Founded in 2017 this is a collaboration between Save the Children UK, Imperial College London, the World Innovation Summit for Health, Bristol Royal Infirmary and Bristol Royal Hospital for Children emergency departments. This was in response to the global crisis that places millions of children worldwide at daily risk of conflict injury and the lack of paediatric specific experience and training in these areas. In 2018 the Partnership released the Paediatric Blast Injury Field Manual, a clinical resource for first responders and clinicians dealing with injured children in conflict zones.
Successful applicants will be appointed to a 60/40 clinical post at Bristol Royal Infirmary with 40% funded time to work on any of the following projects.

1. Update of the Paediatric Blast Injury Field Manual and preparation of material for digital conversion to a web based app to allow great worldwide access
2. Creation of a field pocket action card and haemorrhage control pack for first responders dealing with injured children
3. Non-pharmacological paediatric pain management research project with an aim to produce clinical resource material for care providers

Clinical

The post holder must be aware of clinical governance and clinical risk management and take an active part in their implementation, including audit. Clinical audit is established at the BRI. The post holder will work at Bristol Eye Hospital and will be responsible for supervising a team of validators to carry out RTT validation and data quality checks on over 7000 RTT pathways. This will include the need to process patient pathway activity against the patient access policy and the supporting standard operating system guidance. The Senior RTT Validator and Trainer will be an expert in RTT national guidance and will support the more junior validators in the team and provide training to other departments across the hospital site. We operate a 7 day level of working in some teams and a 6 day level of working in other teams at the BRI meaning that you will contribute to 1:4 Saturday &/or Sundays. You will contribute to the weekend working and on-call respiratory rota.
