The duties and responsibilities to be undertaken by members of the practice reception team may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Reception Manager/Practice Manager, dependent on current and evolving practice workload and staffing levels: Opening up/locking up of practice premises and maintaining security in accordance with practice protocols Undertaking a variety of Reception tasks following induction and training Maintaining and monitoring the practice appointments system Processing personal and telephone requests for appointments, visits and telephone consultations, and ensuring callers are directed to the appropriate healthcare professional Processing and distributing incoming (and outgoing) mail Processing direct patient communications received through website and email channels, scanning, summarising and coding. Taking messages and passing on information Filing and retrieving paperwork Processing repeat prescriptions in accordance with practice guidelines Computer data entry/data allocation and collation; processing and recording information in accordance with practice procedures Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers Providing clerical assistance to practice staff as required, including word/data processing, filing, photocopying and scanning

Clinical: 1. To provide a specialist systemic assessment of referrals of a significant and complex nature to formulate and implement plans for the treatment and/or management of a childs or adolescents mental health problems, based upon an appropriate conceptual framework and employing methods based upon evidence. To use routine outcome measures as laid down by the Trust. 2. To assess children, adolescents and their families as part of a risk assessment. 3. To consult to parents/carers (including foster parents) as part of a treatment plan. 4. To work in ways which are sensitive to and appropriate for the needs of families from a wide range of racial, cultural and religious backgrounds. This includes abilities to understand and offer therapeutic interventions to a variety of family forms including single parents, same sex couples and their wider family networks, accommodated children and their network of care, children with learning difficulties, and children who are on the Child Protection Register, and parents with mental health difficulties. Competence is required in undertaking systemic therapy with families using an interpreter where English is not their first language. 5. To provide specialist systemic psychotherapy with clinical autonomy drawing on a range of models and wherever possible brief interventions. To practice in a way which is inclusive and considerate of the needs of each individual in the system including family members in their varying developmental stages and current emotional state. 6. To make highly skilled evaluations and decisions about treatment options considering highly complex factors concerning historical and developmental processes that have shaped the child, family or group, and their difficulties. 7. To be responsible for implementing a range of systemic psychotherapeutic interventions for children, adolescents, and their families, including couples and groups, drawing upon different explanatory models, and maintaining several provisional hypotheses in reaching a formulation and treatment plan. Interventions may include family psychotherapy, individual work, chairing professionals, and network meetings, liaising with other agencies, observations of young people in different settings, and the use of a range of systemic models. 8. To exercise full autonomous professional responsibility for the assessment, treatment, and discharge of clients, and formulate effective care plans in collaboration with the client and family. To undertake risk assessment and risk management relevant to individual patients, including protective and risk factors present in the network of significant relationships, and to do this using the knowledge of the multi professional context that is a mental health service for the locality. 9. To work jointly and collaboratively with other team members to enhance and develop work with families by providing specialist knowledge and skills.

This role is an exciting opportunity to develop and implement new services within the psychosis pathway across three busy South London Boroughs - Bexley, Bromley and Greenwich. The post holder will be a member of a multi-disciplinary team providing support to clients in care homes with 1:1 care and their carers/ families. The aim of the service is to help individuals with Dementia and other Mental Health Diagnosis to be supported in their place of residence (own home, care/nursing homes, extra care) by the people with whom they are familiar plus the input of the DISS/OP CRHT for as long as clinically appropriate. The key aim of the support worker is to work alongside the multi-disciplinary team to 1. Reduce the number of people relocated to care homes. 2. Reduce numbers admitted to hospital A&E. 3. Reduce out of area placements. 4. Reduce the level of NHS Continuing Healthcare (CHC) one to one support for those assessed as requiring a level of care need because of illness or disability. 5. Training care home staff to better care for residents with Dementia. 6. To help those with Dementia live an increased quality of life. 7. To support older people in a Mental Health Crisis. Be responsible for the Childrens Continuing Care assessment of children and young people with Mental Health/ LD needs and Physical Health needs. The post is with the Generic Team in Greenwich CAMHS.The Generic Team is a highly experienced multidisciplinary team comprised of psychiatry, family therapy, art therapy, psychotherapy, nursing, social work, childrens wellbeing practitioners, and psychology at various levels of seniority.This Senior CBT Therapist role is to provide assessment, formulation and evidence-based CBT interventions for children and young people with complex mental health problems and their families. The role involves delivery of individual, family and group interventions. The role may also involve specialist consultation, support, and training to colleagues within and external to the service. The successful candidate would contribute to a busy and large MDT, working within the overall framework of the teams policies and procedures. As a Senior CBT Therapist you will have a balanced job plan which meets both the needs of the service and your own developmental needs and interests. The job plan will include contributing to initial assessments which are robustly supported through post assessment multi-disciplinary case discussions.
