Job Responsibilities and key duties: Clinical practice: To deliver a high standard of patient care as an advanced clinical practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base. Use expertise and decision-making skills to inform clinical reasoning approaches when patients present with differentiated and undifferentiated presentations. Making use of skills in history taking, holistic assessment, identifying risk factors, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. Working in partnership with individuals, families and carers. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports. Initiate, evaluate and modify a range of interventions which may include lifestyle advice and care through the use of health promotion, health education and screening. Where the post holder is an independent prescriber to prescribe safe, effective and appropriate medication as defined by current legislative framework. To prioritise health problems and intervene appropriately, including initiation of effective emergency care. Recognise and manage anaphylaxis within current UK Resuscitation Council guidelines. Perform practical basic life support procedures and techniques within current UK Resuscitation Council guidelines. Work collaboratively with multi-agency and inter-professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings. Act as a clinical role model for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies and networks. To clearly communicate the health status of the patient using appropriate technology and format. To work within the practice guidelines, policies and protocols. Leadership and Management: Proactively initiate and develop effective working relationships, fostering clarity of roles within teams, to encourage productive working. Evaluate own practice, and participate in multi-disciplinary service and team evaluation, demonstrating the impact of advanced clinical practice on service outcomes. Actively engage in peer review to inform own and others practice, formulating and implementing strategies to act on learning and make improvements. Actively seek feedback and involvement from individuals, families, carers, communities and colleagues in the co-production of service improvements. Demonstrate team leadership and resilience when managing situations that are unfamiliar or unpredictable. Continually develop practice in response to changing population, health need and promote the sharing and adoption of best practice. Participate in the education and training of students of all disciplines. Participate in the weekly Practice meetings with clinical colleagues and management.
