The Suicide Prevention program serves to facilitate case management services to Veterans identified as high-risk on the facility High-Risk List at the Mann-Grandstaff Medical Center and Community-Based Outpatient Clinics (CBOC). The primary components of the program are to provide case management services designed to improve/maintain Veteran's health and mental health, enhancing their ability to remain stable, housed, and community-integrated.

• Weekly clinical contact as needed for high-risk Veterans discharged from inpatient status for the first thirty days -post-discharge while identified as high-risk, in collaboration with other treatment team members;
• Ongoing clinical contact as needed for Veterans deemed at high risk for suicide following national guidelines, and those Veterans placed on the high-risk list;
• Preparation and updates in conjunction with the primary mental health team of the suicide specific assessment, treatment plan, safety plan, and follow-up notes including treatment plan updates;
• Provision of minimum monthly clinical case management contact with all high-risk Veterans in the program;
• Coordination of all elements of mental health and primary care including active communication with all providers involved in the provision of comprehensive care to high-risk Veterans;
• Provision of evidence-based treatment to high-risk Veterans (i.e., group, individual Cognitive Behavioral Therapy) and completion of specialized training and/or certification in evidence based treatment for high-risk behavior (e.g. Dialectical Behavior Therapy);
• Review/provision of comprehensive discharge planning and social service referrals to highrisk Veterans and families;
• Provides case management services while utilizing professional skill, objectivity, insight, advanced clinical training, and experience to interpret data identify viable treatment options, and recognize potential high-risk factors, acuity, and needs for services;
• Obtains any relevant HIPAA clearances to allow transmittal of facility data on suicide attempts and individuals at risk and transmitting the data following cyber security requirements as applicable for this sensitive data;
• Ensuring that identified high-risk Veterans have a 24-hour resource number to call and ways to get assistance if an immediate crisis should occur;
• Respond to consults from the National Veterans Crisis Line and internal facility calls regarding suicidal Veterans;

• Tracking and reporting on Veterans who have attempted suicide or are at high risk for suicide
• Responding to referrals from the Veterans Crisis Line (VCL) and self-referrals from other sources

• Working with providers to ensure that monitoring and treatment is intensified for high-risk patients while making sure that those who are at elevated risk receive education and support about approaches to reduce risks
• Reporting to OMHSP on Veterans who attempted or died by suicide and sending OMHSP requested data, which is used to determine characteristics and risks associated with these groups of Veterans
• Ensuring that medical centers have established a High-Risk for Suicide List (HRSL) of Veterans and a process for establishing Patient Record Flag Category 1 - High Risk for Suicide (HRS-PRFs).
• Thorough knowledge of the program objectives to ensure that actions taken are appropriate.
• Disseminate resources and implement effective, evidence based strategies for reducing incidence of suicide and suicide behavior.

The Substance Abuse Treatment Program Supervisor holds administrative and clinical responsibility for the Substance Abuse Treatment Program (SATP) at the Mann-Grandstaff VAMC. The principal clinical role of the Licensed Professional Mental Health Counselor (LPMHC) is to provide integrated mental health and substance abuse evaluation and treatment for Veterans with comorbid disorders who receive care in the SATP and other programs within the Behavioral Health Service (BHS) or Mann-Grandstaff Veterans Administration Medical Center (MGMC).

Functions and Scope:

• Directs the day-to-day clinical and administrative activities of the SATP team. Coordinates assignments and work duties. Clinically supervises work of addiction therapists.
• Monitors program operations and outcomes, making changes to clinical interventions, documentation of care, program materials, team work assignments, schedules, outreach activities, and other program elements in order to maintain outcomes of the highest quality.
• Responsible for all areas of performance of the SATP Programs.
• Responsible to see that the SATP Program meets the data monitoring and program reporting requirements of the various oversight bodies.

Clinical Assessment

• Evaluates each Veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion including an assessment of vulnerability and priority for admission. Assesses at-risk factors and develops a preliminary plan, involving the Veteran and family or significant others, and performs a thorough assessment of serious and complicated cases involving psychiatric illness.
• Verifies the Veteran's eligibility for VA medical care, and eligibility of program participation. They also help family members determine eligibility for indicated community programs.

• Responsible for providing case management for a complex caseload of homeless Veterans in the HUD-VASH programs in rural and remote regions of Central Washington.
• Performs on-site visits of HUD-VASH apartments/housing units in rural and remotely separated communities to ensure that Veterans reside in safe environments in compliance with local housing codes.
• Coordinates care including serving as an advocate on behalf of the Veterans and their family members to ensure comprehensive service delivery and linking and referring Veterans and families to the VA Medical Center, VA Regional Offices and/or community agencies. Must have extensive knowledge of programs and services providers in communities throughout the Central Washington Region.

• Responsible for program oversight, monitoring and administration, serving as the contact with local Public Housing Authorities (PHA's) and providing clinical case-management to Veterans enrolled HUD-VASH. Responsible to review the agreements between Section 8 landlords, PHAs, and the VA to establish appropriate referral sites for placements.
• Implements and maintains referral and screening procedures for potential HUD-VASH, Program Veteran participants that meet national policy as well as the needs of medical center mental health services and residential programs.
• Works closely with the VA medical center's homeless programs, residential and community care programs.
• Responsible for collecting and submitting HUD-VASH Program participant data as outlined by the Northeast Program Evaluation Center (NEPEC) evaluation procedures.
• Fulfills ethical responsibilities to Veterans, colleagues, and VA in accordance with relevant discipline Code of Ethics, VHA Code of Ethics, and local Ethics policies.
• Documents in the patient's medical record in accordance with medical center policies.

• Regularly conducts and participates in outreach activities including field interviews, assessments and referrals for homeless Veterans contacted in the community, shelters, and "on the streets," as well as those who are referred by VA residential programs, medical centers, or outpatient clinics.

Clinical Assessment

• Evaluates each Veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion including an assessment of vulnerability and priority for admission. Assesses at-risk factors and develops a preliminary plan, involving the Veteran and family or significant others, and performs a thorough assessment of serious and complicated cases involving psychiatric illness.
• Verifies the Veteran's eligibility for VA medical care, and eligibility of program participation. They also help family members determine eligibility for indicated community programs.

• Responsible for providing case management for a complex caseload of homeless Veterans in the HUD-VASH programs in rural and remote regions of Central Washington.
• Performs on-site visits of HUD-VASH apartments/housing units in rural and remotely separated communities to ensure that Veterans reside in safe environments in compliance with local housing codes.
• Coordinates care including serving as an advocate on behalf of the Veterans and their family members to ensure comprehensive service delivery and linking and referring Veterans and families to the VA Medical Center, VA Regional Offices and/or community agencies. Must have extensive knowledge of programs and services providers in communities throughout the Central Washington Region.
• Implements the Housing First model within the HUD-VASH program.
• Provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs.
