KEY RESULT AREAS Carry out medicines administration to selected patent in accordance with prescription requirements To undertake competency based training in the preparation and administration of medicines To be competent in the administration of prescribed medicines to allocated inpatients against an inpatient prescription following the Trusts Medicines Management policy To undertake a range of checks on the preparation and administration of medicines including injectables, and Controlled Drugs in accordance with the Medicines Management policy To accurately record the administration of medicines on the inpatient prescription and promote the safe and timely administration of medicines. Offer information and advice to patients when administering medicines. To report any Adverse Drug Reaction(s) to a senior nurse or medical staff Support patients wishing to self-administer their own medicines To contribute to handover of patient related information to medical and nursing teams To work with nursing and medical staff to improve the quality of care provided to inpatients To identify and resolve, by referring to the prescriber, pharmacist or patients notes where necessary, problems and errors in prescribing of a technical nature To work with ward and dispensary based pharmacy teams to improve the quality and efficiency of medicines management to hospital inpatients, ensuring compliance with national standards (e.g. CQC, RPSGB) To dispense medicines in accordance with professional, legal and ethical standards and in agreement with departmental performance standards To provide a source of advice and expertise on matters relating to the provision of medicines and medicines information at discharge. To collect data relating to the activity of the post, undertake audits work and assessment of patient experience To use the electronic prescribing and medicines administration system (EPMA), ensuring that patient data is updated in a timely manner. To provide necessary training for medical and nursing staff, with regular updates if changes are made To support ward / department nursing staff in meeting the required standards for the Safe and Secure handling of medicines, including participation in Audit initiatives
