The duties of this occupation include but are not limited to the following:

• Utilizes standard medical evaluation procedures and East Asian Medicine evaluation to formulate a differential diagnosis to develop an acupuncture treatment plan.
• Incorporates the various training techniques taught within East Asian Medicine and acupuncture, and may apply safe and effective advanced acupuncture techniques.
• Educates patients on health lifestyle changes including nutrition, exercise, sleeping habits, stress management, mind body techniques and other matters related to acupuncture care.
• Provides group acupuncture as determined.
• Documents and codes information related to patient care in the VA's electronic health care record on a timely basis following VHA policies.
• Communicates with other health care providers as appropriate.
• Maintains up-to-date VHA and professional trainings and attends required meetings.

Major duties include, but are not limited to:

• Incumbent is responsible to the Supervisor Laundry Operations Specialist for assuring that the work assignments of the other employees of the section are carried out.

The Clinical Pharmacist Practitioner has responsibility to develop, organize, and manage the Clinical Pharmacist Specialty Program within the assigned Chronic Pain medication management teams. The CPP has full responsibility to develop necessary services such as physician consultation, patient monitoring, patient counseling and medication management within the Chronic Pain management framework. The CPP is responsible to serve as preceptor for, Doctor of Pharmacy candidates, Post Graduate Year 1 Pharmacy residents and Post Graduate Year 2 Pharmacy residents in pain and mental health. The CPP must be able to demonstrate the knowledge and skills necessary to complete the duties of the position, including the necessary skills to provide care appropriate to the population of the patients served.

Primary duties of this position include but are not limited to:

Clinical

• Works independently under a scope of practice, providing medication management for those patients who are seen in the Chronic Pain clinic and inpatient areas.
• Serves as a mid-level provider who functions to initiate, modify, or discontinue medications (excluding controlled substances) as clinically indicated in the treatment of chronic pain management and disease states including but not limited to neuropathic pain and fibromyalgia etc.
• Interviews patients in assigned area, conducting medication history, formulating and implementing pharmaceutical care plan and documenting this information in the patient's electronic record in the Computerized Patient Record System (CPRS).
• Documents a pharmacy education note in the patient's electronic record in CPRS after educating/counseling has taken place.
• Designs a therapeutic regimen or plan, based on patient-specific information that includes establishing a therapeutic endpoint, electing an appropriate drug and related therapy, and establishing an appropriate dosage regimen.
• Interfaces with providers, including physicians, physician's assistants (PA), and Advanced Registered Nurse Practitioners (ARNP) and other healthcare providers in the design of the veteran's treatment regimen to achieve optimization of rational and cost-effective drug therapy.
• Establishes and expands the patient data base through interpreting the drug history, medical history, physical examination and ongoing patient specific monitoring of therapy, with documentation in the formal patient medical record.
• Monitors and assesses therapeutic and adverse effects of drug therapy, including drug interactions, through selection and evaluation of physical and laboratory parameters.
• Orders lab tests as indicated for appropriate patient monitoring.
• Provides consultation on selection of appropriate therapeutic regimens as a follow-up of non-therapeutic drug concentrations obtained from ambulatory care patients.
• Provides physicians and other providers a focal resource for discussion of medical problems requiring a pharmaceutical intervention.
• Participates in pain management meetings as assigned.
• Provides clinical telehealth services (such as VA Video Connect) for appropriate patients.
• Implements Medication Use Committee policies including therapeutic interchanges and automatic substitutions to include appropriate patient and provider education.

• Participates in Medication Use Committee activities when requested.
• Reviews medication usage in patients receiving pain medications as part of the overall quality assurance program and prepares necessary reports as required.
• Designs, implements and supervises the chronic pain clinical education and training program for Doctor of Pharmacy candidates and Post Graduate Pharmacy residents.
• Participates in the medical center Ongoing Professional Practice Evaluation (OPPE).
• Registers in Electronic Florida Online Reporting of Controlled Substances Evaluation (E-FORCSE) to participate in the Florida Prescription Drug Monitoring Program.
• May serve as a director or associate director for a post graduate pharmacy residency program based on qualifications.

• Independently designs, and conducts clinical projects within the scope of pharmacy practice.

MAJOR DUTIES & RESPONSIBILITIES

1. Provides total care of the Veterans' medical needs by performing history and physical exam, ordering appropriate lab tests, radiology studies, screening studies. This includes evaluating those tests, developing a treatment and follow-up plan to include medication, medical equipment, and home based interventions within the guidelines laid down by the supervising physician. This care will also include the outpatient follow up visits, post discharge follow-up and managing care with specialty and sub-specialty medical providers. The patient care will be provided through face to face visits and/or other modalities (group clinics, telephone clinics, secure messaging, etc).
2. Assists in the preparation of the patient and/or his/her family to include, but not be limited to, teaching (to include health promotion and disease prevention activities), accumulating data and medical problems, and presenting patient centered discussion of medical problems for appraisal and scheduling patients with the appropriate Primary care department services.
3. Responsible for completing patient encounters, notes, and correctly coding visits in accordance with coding guidelines.
4. Acts as the focal point of the designated outpatient clinic by being the contact person for all other consultant services regarding patient care, and providing consults for Primary Care department to other services.
5. Provides assistance to the Primary Care Department with regard to duties to assist the service that may be requested. These may include, but not be limited to, assisting in data assimilation for VA reports, quality assurance programs, or others as determined by the Chief of Primary Careor Chief of Staff.
6. Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in Sioux Falls VAMC Primary Care Department. The individual must demonstrate the knowledge of the changes associated with aging and possess the ability to provide care based upon physical, psycho/social, cultural, safety and other age related factors as noted in the competencies described in departmental and/or unit specific policies and procedures.
7. Maintain skills, proficiencies, and professional knowledge required for licensing and clinical scope of practice.
8. Provides instruction/preceptor-ship for physician assistant students, physician assistant residents and Nurse practitioner students from various schools across the region, within her Primary Care Department clinic. Provides physical/history and medical instruction to pharmacy students, within her Primary Care Department clinic. The incumbent serves as a Program Analyst (Health Informatics) located in Charleston, SC.
