Main Duties: Clinical: To provide specialist psychological therapy assessments and treatments with clients referred to the NHS Talking Therapies service. Job responsibilities Clinical Practice Work as part of a multi-disciplinary team to deliver safe and high quality care. Carries out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care. Delivery of care in line with NSF, NICE guidelines and evidence-based care The post holder will work within their clinical competencies to provide specialist pharmaceutical support to all practices of the PCN including working with members of the wider healthcare team to improve care and support safe and effective prescribing. Moving between two practices, the role will have a patient facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews. This will also include the management and review of patients with polypharmacy. As a Pharmacy Technician, you will be providing a medicines management service to patients within the prison with the professional support of a prison service pharmacist. You will administer medicines to patients against a valid prescription, ensuring this is taken correctly, provide advice to patients about the appropriate use of medicines and promote healthy lifestyles to aid their rehabilitation.

You will dispense prescriptions and stock items, whilst supervising assistants when in the dispensary. There will also be elements of stock control involved in your role, such as undertaking stock top-up services and reviewing expiry dates and storage facilities for all medicines, reporting any deficiencies to the prison service pharmacist.

As a Mental Health Practitioner you will be providing specialist mental healthcare to offenders and working as part of the Mental Health In-Reach Team, plus wider mental health services. You will be required to work in a psychologically minded way with offenders in achieving their agreed goals and quality health outcomes.

Our Mental Health Team provide specialist interventions at primary and secondary care level including; referral management, screening assessment, triage, evidence-based interventions, care planning and risk assessing, plus one-to-one and group-work facilitation.

Our Mental Health Practitioners manage a mixed and challenging caseload and are required to perform robust assessment, screening and interventions to offenders with learning disabilities and mental health conditions. You will contribute to alternatives to inpatient admission and assisting with early discharge through the implementation of high intensity interventions and complex case management arrangements.

One of our key aims is to ensure continuity of care for an offender on ordinary location and reduce the length of stay, both in relation to prison inpatient services and external NHS/independent in-patient services. You will work closely with community mental health teams to ensure appropriate sharing of information, continuity of care and the Care Programme Approach for all offenders as necessary.
