Community Care RN CoordinatorNurse II handles the process of organizing and coordinating resources and services in response to individual healthcare needs along the illness and care continuum and in multiple settings. The Ambulatory Surgery Unit Registered Nurse (ASU RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The incumbent will administer medications and procedures per established policies and guidelines, and influence care outcomes by collaborating with members of the interdisciplinary team.

The ASU RNcoordinates care prior to and on the day of procedure/surgery. The incumbent provides direct patient care to patients in a pre-operative clinic preparing patients for upcoming care, and on the day of care - to patients undergoing invasive and surgical procedures during the pre-operative/pre-procedural phase and during the post-operative/post-procedural phase of recovery.

The incumbent provides pre-operative/pre-procedural care, including (but not limited to) the nursing assessment of patients, consent, and history and physical validations, the starting of intravenous fluids including venipuncture, assuring all lab work and other pre-operative/pre-procedural testing is completed and communicated to team members, confirms surgical sites are prepared and marked, and administers medications as ordered. The ASU RN monitors post-operative/procedural patient's level of consciousness after the recovery of anesthesia/sedation and closely monitors patients for signs of side effect of residual anesthesia medications.

This position is located in the Mental Health Care Line, VA Hudson Valley Health Care System. Controls and Oversight: Percent Designs studies, training programs and organizational strategies for identifying opportunities to improve performance and ensure adequate internal controls are in-place. Analyzes key steps in transaction process or business practice to identify potential material weaknesses. On a periodic basis assists the Integrity Compliance Officer in performing local health care system-wide reviews of internal controls effectiveness. Assists managers and staff to develop and implement internal control measures and monitors if there is a high risk of operational failure.

Development of Criteria: Page 4 of 8 Develops reporting procedures, sampling techniques, and measurements at various points of the revenue program or business cycle to identify problem areas or inherent weaknesses in work processes, procedures, information systems and content. Develops indicators and monitors to provide objective evidence of the effectiveness of quality procedures and controls. Performs in depth analysis of stable work processes or service delivery practices to identify reasonable and efficient quality measurements tailored to the activity. Monitors quality program implementation and indicators designed to prevent erroneous transactions and ineffective work practices and to verify adherence to quality plans and requirements.

Program Implementation: Investigates and analyzes adverse quality trends or conditions and recommends corrective action. Consults with technical experts to examine and analyze unusual or problem situations indicated by changes in program quality indicators. Performs in-depth research, analyzes operating contingencies and special situations. Assists managers and staff in developing process solutions.

• Incumbent is charged with full responsibility, both technical and administrative, for the daily operation of the HIM department which consists of all functions of health record activities related to coding, health record documentation analysis, transcription/speech recognition, release of information, file room/scanning programs, and health information exchange.
• Is a full assistant to the Chief of Health Information Management (CHIM). Providing expertise, guidance and direction and has responsibilities relative to providing supervision, administrative support, coordination, and technical assistance in the planning, development and maintenance of an effective, comprehensive, and integrated HIM Program at this Medical Center.
• This position assists the CHIM in the planning, directing, policy making, organizing and controlling the activities of this department and serves as acting CHIM in the absence of the CHIM.
• The incumbent shares responsibility with the CHIM for collecting and processing data for various reports, surveys, and research projects; for preparing reports and recommendations for review and consideration by the Chief, Health Information Management (CHIM) and upper management concerning the immediate needs as well as the long range goals of the department; for recommending equipment and staffing needs; and for preparing and keeping current procedures and operation manuals.
• Responsible for assisting physicians and other health professionals in obtaining health record information to be used in research projects.
• Advises on what material is available, how it may be obtained and the uses that can be made of the information while protecting the confidentiality of the record.
• The incumbent aids in a variety of projects and is responsible for screening, abstracting and tallying data from records and computer-generated reports.
• May serve as a consultant to provide guidance and advise researchers on methods for gathering data.
• Assists in selected miscellaneous retrospective health record review/data abstraction projects to support medical center Quality Management data collection activities.
• Provides effective audits and monitors for all areas of responsibility, assuring that accurate and timely data is processed by all employees.
• Reviews work accomplished and assesses quality, quantity, accuracy and timeliness of work produced by each employee.
• Ascertains problems encountered and takes corrective action.
• Develops performance standards and conducts performance evaluations. Develops and conducts competency assessments.

The NM is responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance. They function as a role model, educator, practitioner, and

Preferred Experience: 2-5 years of Medical Intensive Care Unit (MICU)/Surgical Intensive Care Unit (SICU) experience and 2-3 years of progressive MICU/SICU nursing leadership experience in a level 1 facility.
Preferred Education: Master of Science in Nursing (MSN)

Note: This position is not approved for telework and staff is required to be on onsite in the clinical area of assignment. This position supports all 3 campuses and may require rotation as needed.
