Key Responsibilities: Clinical To provide a comprehensive Speech and Language Therapy service to clients referred with speech, language and communication difficulties within mainstream schools and special school in the borough To deliver appropriate formal and informal assessment, make differential diagnosis on the basis of this assessment, and communicate complex condition related information from assessment to children, carers, families and members of the multi-disciplinary team/other professions. To maintain sensitivity at all times to the emotional needs of clients/carers when imparting complex and distressing information regarding the nature of the clients difficulties and the impact of this on individuals and their families. Following assessment and differential diagnosis, to design and develop evidence based treatment and management programmes to be implemented within the multi-disciplinary teams available, and within the childs family. To adapt and facilitate treatment programmes and the use of augmentative communication systems and/or communication aids according to perceived clients needs. To impart principles and establish the use of language enrichment techniques by parents/carers and fellow practitioners To support children with identified special educational needs within mainstream and special schools. To work closely with school staff to embed inclusive speech, language and communication practice within the classroom. To work closely with clients, carers and families, agreeing decision making relevant to the client management, and providing the family with sufficient information to ensure meaningful choices that promote dignity, independence and quality of life. To demonstrate empathy with clients, carers and families and colleagues, ensuring that effective communication is achieved, particularly where barriers to understanding exist. To demonstrate skills in motivating clients and/or carers to engage in the therapeutic process. To maintain a high level of concentration in all aspects of patient care at all times, in order to monitor auditory, visual and kinaesthetic aspects of communication adaptations, and to facilitate treatment programmes and the use of augmentative communication systems communication aids according to perceived need, with regard to cultural and linguistic differences. To be able to initiate referrals to other health professionals, specialist services and agencies. To be computer literate and ensure accurate recording of actions, and updating patients records, maintaining confidentiality at all times. Research and Development To ensure practice is supported by research, evidence based practice, literature and peer review. To undertake Clinical Governance and Audit projects within the local service To participate in departmental research projects and collect research and outcome data as required. Education and Development To demonstrate professional knowledge as evidenced by 1st degree level qualification or equivalent. To act as an increasingly autonomous Speech and Language Therapist who is legally and professionally accountable for own actions as guided by the professional code of conduct and Trust guidelines and protocols To demonstrate the ability to reflect on own practice with peers and clinical supervisors. To work independently accessing clinical supervision and appraisal within an Individual Performance Framework at pre-determined intervals to promote and develop own clinical skills, whilst ensuring that the objectives set reflect the Service and Trust plans. To identify personal and professional development evidenced by the development of a Personal Development Plan and professional portfolio of continuing professional development, developed within an appraisal framework. To keep up to date with new techniques and developments for the promotion and maintenance of good practice in paediatrics. To attend relevant training courses, meetings and special interest groups in order to maintain skills and knowledge required of a therapist working in the field of paediatrics. Information Resources To maintain up to date and accurate case notes in line with RCSLT professional standards and local trust policies. To share information with others as appropriate, observing data protection guidelines. To undertake duties in compliance with the administrative systems at Trust and departmental level. Clinical Care The post holder will: Put the patient at the centre of everything they do. Assess and plan the delivery of patient care and take responsibility for implementing and evaluating that care, using an appropriate nursing model Caring for level 2 and level 3. Will be expected to undertake the introduction to Critical care work base module which if successful will gain 30 credits at level 6 (degree level) level 7 ( MSc level). Carry out nursing responsibilities with high standards of care according to Trust Policies, Practice and Clinical Guidelines and National Critical Care guidance. Ensure that documentation is comprehensive, up to date and reflects the nursing care/ interventions given to the patients. The post holder will embrace the philosophy of the multi-disciplinary team and take steps to ensure such working relationships are fostered at every opportunity. Staff will be expected to undertake internal rotation. Practice evidence-based clinical nursing taking steps to ensure that all nursing interventions are justified. The post holder will undertake to learn methods of literature searching, gathering evidence and critical appraisal to underpin nursing practice. Ensure that all equipment is maintained in safe working order and demonstrate competence of how to use the equipment safely and effectively with training and support. Promote psychological care and rehabilitation of critically ill patients. This will include use of patients diaries and increased awareness of critical care follow up. Report all incidents / accidents involving patients, visitors or staff to Matron or the shift co-ordinator, in the first instance. The post holder will ensure that the correct documentation is fully completed and forwarded to the appropriate personnel in accordance with Health and Safety requirements. Report all complaints immediately to the shift co-ordinator and Lead of Critical Care in the first instance. In cases where investigation is required, verbal or written statements and any other evidence, such as copies of documentation, must be provided promptly and professionally. Team Working and Communication The post holder will: Communicate daily with patients, relatives and carers to ensure that they are informed of nursing goals and medical objectives and are involved in decision-making. This requires a high level of interpersonal skill. The post holder will arrange for patients, relatives and carers to see senior nursing or medical staff, as required. Provide support for relatives and patients in a sensitive and respectful manner. Exhibit a thorough awareness of the patient activity in the entire unit regardless of the patient allocation system employed on a shift basis. The post holder will inform the shift co-ordinator of any activity which affects patient care. Actively seek out opportunities to participate in the education of learners allocated to the ward. Supervise a range of clinical skills and conduct teaching. At all times exhibit a nurturing attitude to the students to maximise their learning experience during their allocation. Assist in the induction of new staff to the unit under the guidance of more senior staff. Maintain a dynamic learning environment within the ward, distributing up to date information of nursing practice and developments, and compiling teaching programmes for the ward. The post holder will be aware of the requirements of a quality review of practice learning environments. Promote effective communication within the multidisciplinary team. Act as an effective role model and to attend departmental meetings as required. Professional Development The post holder will: Be responsible for their own professional development and updating by reading the nursing press, attending relevant courses both in-house and external, in line with the NMC requirements. Through the Appraisal and Development System, the post holder will construct a personal development plan in conjunction with the Line Manager that reflects individual and organisational needs. Ensure all professional performance is underpinned by NMC documents, e.g.) Code of Professional Conduct and Revalidation. Acquire new skills and knowledge to expand nursing practice. Will be familiar with local policies, protocols and guidelines. When representing the department, trust or profession, the post holder will ensure that this is conducted in a professional manner. Other Responsibilities The post holder will: Wear uniform at all times, in accordance with the uniform policy. Interact with patients, visitors to the ward, other members of the nursing team and managers in positive, supportive and co-operative ways. Be responsible for reporting their sick leave directly to the shift co-ordinator or CCU Lead, giving as much notice as possible. Study leave must be negotiated with the CCU Lead/Line manager and must reflect the organisation and individual requirements of the personal development plan. All learning should make a positive contribution to the unit and nurses are expected to share their learning with their colleagues after attending a course or study day.

The post-holder will be enrolled in a graduate certificate or postgraduate certificate training delivering the national curriculum for Mental Health and Wellbeing Practitioners. Once successfully enrolled, the post-holder will attend all elements of the programme of study and progress successfully through assessments of competence and attainment.
