To assist in the delivery of high standards of health and care to Heart Failure patients within their own homes, care homes, the traveling community and in clinics, by following prescribed packages of care. To assist in the service administration ensuring referrals are logged and records are maintained.

1. To contribute to the delivery of care packages for patients with Heart Failure, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.

2. To receive and process referrals, allocating and tending to a workload communicated through an electronic ledger system.

3. To work within the Heart Failure team and other agencies to prevent unnecessary admission or readmission to hospital.

4. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

5. To set up Telehealth monitoring for patients within their own homes, as identified by the Heart Failure Nurses

6. To assist in documenting data in SystmOne, received from patients on the telehealth monitoring system and feedback any variations outside of normal range to the Heart Failure Nurses.

Clinical

The post holder will:

· Apply competency based treatment techniques/care to patients with Heart Failure under the guidance of a qualified practitioner, following a prescribed treatment/care plan.

· Plan and prioritise own visits to delegated patients and ensure these are documented appropriately.

· Deliver care using prescribed methods and procedures. Including providing advice to patients on anxiety management, self-monitoring and other cardiac rehabilitation techniques.

· Receive, process and administer referrals and patient records. Regularly communicating with the rest of the Heart Failure Team to ensure the service’s patients are receiving a high quality of care and support.

· Set up telehealth monitoring in patients’ home, when patients are identified by the heart failure nurses. This includes teaching patients how to use the monitoring equipment and upload data via the appropriate system.

· Record data collected from telehealth monitoring is entered onto SystmOne on a daily basis, inform heart failure nurses of any variations in recordings outside of a normal range.

· Remove telehealth monitoring equipment from patients’ homes when no longer required. Return equipment to heart failure team for maintenance.

· Ensure data base of patients with telehealth monitoring equipment is kept up to date and any equipment retrieved and replaced if routine servicing required.

· Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis.

· Engage with family members and carers to provide the patient with the most comprehensive support possible.

· Make basic judgements on patients’ response to prescribed treatment/care plans, and report findings to a qualified practitioner.

· Liaise with other health care professionals as required.

· Overcome barriers to understanding, e.g. patients’ with hearing or cognitive impairment.

· Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.

· Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.

· Demonstrate dexterity and coordination when undertaking treatment of patients where accuracy is important, e.g. taking ECGs, venepuncture, supervising transfers, and passive exercises.

· Be responsible for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately.

· Contribute to the requisitioning of supplies and equipment through electronic ordering systems.

· Work predominantly independently on specified tasks, with regular clinical support and supervision by a qualified practitioner.

· Work with qualified practitioners with group work/clinics as required.

· Demonstrate an awareness and understanding of consent, and gain consent, and gain consent as appropriate following organisational policies.

· Share responsibility for maintaining store cupboards, cleaning specialist equipment and general housekeeping tasks.

· Share responsibility of indirect patient contact tasks, such as, answering phones, arranging appointments, processing referrals and inputting activity data.

· Undertake health promotion/education as required, e.g. smoking cessation, falls etc.

· Utilise universal precautions and adhere to infection control policies.
