To liaise with external bodies including local CCGs, regional pharmacy services, and necessary pharmacy networks. 3. The Joint Emergency Team aims to prevent inappropriate hospital admissions and facilitate early discharges from hospital by acting as the gateway to the Integrated Community Services within the borough of Greenwich. The team responds to referrals requiring intervention within 2 hours which includes the falls pick up service. The virtual ward is an integral part of this team and enables treatment and monitoring within the patients own home for a period of up to 14 days.

The team provides a multi-professional assessment of health and social care needs to support the immediate crisis, identifying specialist assessment and rehabilitation requirements. The team is able to monitor and provide intervention to support patients through an acute period of illness. The team is able to provide equipment necessary to maintain patients / clients at home.

JET is a seven day service that operates from 08:00 to 20:30, including bank holidays.

The Occupational Therapist will work as part of a multidisciplinary team, including health and social care colleagues to meet the needs of clients referred to the Joint emergency Team, ensuring the highest possible standards of practice.
• To conduct specialist and generic assessments with the MDT for clients referred to Joint Emergency Team.

Be able to work alone in the community in a variety of settings, independent of multi- disciplinary support whilst in direct contact with clients and their carers. Be professionally and legally responsible and accountable for all professional activities including occupational therapy assessment, diagnosis and treatment of clients referred to the team as an autonomous practitioner and in partnership with the client/carer; obtaining consent in accordance with professional guidelines and local team. Demonstrate and apply theoretical knowledge of acute, multiple, complex and long-term pathologies and impairments in the assessment and management of clients. Use a range of highly specialist clinical assessments and skills to ensure effective management of clients Recommend appropriate interventions to promote independence and safety, utilising effective clinical reasoning skills and evidence-based practice. Carry out clinical risk assessment and complete risk management plans relevant to the client, including risk assessments for equipment provided. Ensure management of clinical risk identified. Complete an individual care plan for clients who require input from integrated community services (bed-based or home based input). Engage in joint patient assessment/ treatment with more junior staff as need dictates. Work in a co-ordinated manner, communicating effectively with other team members and the wider clinical network to ensure that clients needs are met. The post is with the Generic Team in Greenwich CAMHS.The Generic Team is a highly experienced multidisciplinary team comprised of psychiatry, family therapy, art therapy, psychotherapy, nursing, social work, childrens wellbeing practitioners, and psychology at various levels of seniority.This Senior CBT Therapist role is to provide assessment, formulation and evidence-based CBT interventions for children and young people with complex mental health problems and their families. The role involves delivery of individual, family and group interventions. The role may also involve specialist consultation, support, and training to colleagues within and external to the service. The successful candidate would contribute to a busy and large MDT, working within the overall framework of the teams policies and procedures. As a Senior CBT Therapist you will have a balanced job plan which meets both the needs of the service and your own developmental needs and interests. The job plan will include contributing to initial assessments which are robustly supported through post assessment multi-disciplinary case discussions. In addition, you will be providing formulation driven, evidence-based CBT treatment for a wide range of complex presentations, including different anxiety disorders, OCD and depression. You will also be care co-ordinator for some young people, involving working with the family and wider professional network. In addition, you may contribute to the supervision of more junior clinicians. Specialist Occupational Therapist, Band 6

We are looking to appoint a committed, highly motivated, enthusiastic and skilled Band 6 Specialist Occupational Therapist to our dynamic, integrated multi- disciplinary Rapid Response Team(RRT)/ Urgent Community Response Service (UCR).

This is an exciting opportunity to join and develop our integrated collaborative working between Oxleas NHS Foundation Trust and Bexley Adult Social Services. Would you like to help us continue to improve our provision of health, social care and community support to the residents of Bexley.

RRT has been operational since 2013, since has gown and developed, RRT was one of the 7 accelerator sites involved in the introduction and successful roll out of the NHSE Urgent Community Response (UCR) initiative. RRT/UCR is a multi- professional team consisting of Nurses, PT's, OT's and Social Workers. The team aims to respond to crisis referrals within 2 hours and avoid unnecessary presentation to hospital and residential care.

The team provide holistic health and social care assessments for people over the age 18 years, who are registered with a Bexley GP and live in the Borough of Bexley. The team are based at QEH Woolwich and operates 7 days a week, on an 8am-8pm basis, providing urgent community response to vulnerable people in the community.

Mixed shift pattern 8-4, 12-8

Long days at the weekend 8-8

The post holder will support in the leadership of Rapid Response Team, deputising in the absence of the Band 7 lead Therapist/Nurses for the clinical management of the care of patients referred to the team. The post holder will assist with and directly triage urgent patient referrals with varied and complex needs in acute hospital and community setting acting as an expert physiotherapy clinical resource to RRT members by facilitating high quality evidence based practise.

The post holder may be required to work across hospitals sites on a planned basis and in community when responding to urgent community referrals. Post holder will undertake delegated line management of junior staff, both informal and formal basis.

The Rapid Response Team aims to prevent inappropriate admissions to acute hospital wards, unnecessary Emergency Department attendances from the community and facilitate early discharges from hospital by acting as the gateway to Community Services within the Borough of Bexley. The post holder will conduct specialist Occupational Therapy assessments for patients in the community with some in reach into Secondary care as required . The team provides an integrated multi-professional assessment of health and social care needs.

Demonstrate specialist theoretical knowledge of complex acute and long-term pathologies and impairments in the assessment of clients.

Work closely with other health professionals and social services and provide specialist Occupational Therapy advice in meeting the needs of the clients.

Conduct a thorough holistic specialist Occupational Therapy assessment of clients, obtaining consent in accordance with professional guidelines and local team procedures.

Recommend appropriate interventions to promote functional independence and safety, utilising effective clinical reasoning skills and evidence-based practice.

Complete individual plans of care for clients who require input from integrated community services bed-based or home-based rehabilitation input.

Maintain effective multidisciplinary communication with other members of the team and the wider clinical network to ensure that client’s needs are met.

Provide specialist Occupational Therapy advice and support to client, carers and relatives based on clinical evidence and reasoning.

Ensure safe, appropriate and effective provision, installation, education and demonstration of equipment.

Participate in and lead MDT meetings and complex family conferences.

Ensure management of clinical risk with complex patients.
