FUNCTIONS/SCOPE OF DUTIES:

A. Medication Management:

1. Reads and interprets prescriptions, obtains proper medication or material in the proper strength and dosage form; determines proper amount and counts, pours, or measures the medications; affixes proper label and auxiliary labels; and records required data on electronically or via the Pharmacy's copy of prescription/order.
2. Replenishes inventory for emergency carts, kits, and trays. Collects drug use evaluation data for quality improvements purposes.
3. Compounds and/or reconstitutes oral or external medications (non-sterile) without a second review.
4. After prescription/order(s) are reviewed by a pharmacist for therapeutic propriety, reviews orders for technical accuracy and ensures that the item dispensed corresponds to the drug and dose ordered.
5. Answers questions and calls from pharmacy and non-pharmacy personnel regarding drug distribution.
6. Monitors refrigerator temperature daily in assigned areas.
7. Perform monthly inspections to assigned areas to ensure proper storage of medications and soon to expired drugs are removed.
8. Maintains, verifies, stocks and replaces outdates in patient care area automated dispensing machines throughout the facility (i.e., floors and clinics).
9. Properly handles medications by ensuring high alert, hazardous, refrigerated and controlled substance medications are handled/stored appropriately and legally.
10. Prepares medications that requires special handling and documentation.
11. Assures that each individual prescription order is filled correctly with right drug/supply item, strength, dosage form, quantity and with all necessary auxiliary labels attached.

B. Outpatient Pharmacy Activities (when assigned):

1. Processes and fills outpatient prescriptions in compliance with regulations, policies, and procedures.
2. Utilizes the automated dispensing machine (i.e. Scriptpro) for packaging and dispensing medications.
3. Assembles prescriptions properly with the correct drug, strength, quantity, auxiliary labels, and prescription labels.
4. Assures that the correct medication is filled by using bar code scanning on stock bottles.
5. Appropriately assigns and prints label for completed prescriptions notating if it's window, mail, UPS, refrigerated or non-refrigerated item.
6. Supports automation by manually completing prescriptions not processed through the automation dispensing machine.
7. Assures the timely completion of all prescriptions.
8. Verifies the patient's identification following the Medical Center's policies. Ensures two patient identifiers are utilized when dispensing medications.
9. Verifies that the patient receives the correct prescriptions by using VA software (i.e., VISTA and barcode scanning)
10. Verifies that the patient has received all prescriptions (i.e., refrigerated, controlled substances, bulk medications, etc.)
11. Ensure the patient signature is obtained at pick-up electronically. Only uses manual signature during contingency.

C. Inpatient Pharmacy Activities (when assigned)

1. Performs review of medications prepared by another pharmacy employee to be dispensed to hospital areas.
2. Inventories and replenishes emergency trays/carts/kits.
3. Operates and maintains automated dispensing unit dose systems such as Pyxis and Parata.
4. Inspects packaged items for accuracy and suitability before submitting for final check, prior to distribution to hospital areas. Ensures correct number of tablets are present in each locally unit dosed package prior to dispensing to floors and/or automated devices.
5. Utilizes the Parata Perl inspection system after medications repackaged.
6. Assures repackaged dosage forms are labeled according to USP and VA standards and attains a final review by a pharmacist.
7. Maintains repackaging records that assures all regulatory and review standards are met.
8. Trouble-shoots minor mechanical problems of all automated systems as they occur and coordinates resolution of more involved mechanical or software problems with Supervisor, vendor help desk, ADPAC, etc. as appropriate.
9. Fills medication orders to fulfill pre-exchange doses, missing dose reports, Automated Dispensing Machines (ADMs), loads, restocks and IV orders with accuracy.
10. Correctly inventories, assesses, and determines the need for floor stock and external medication.
11. Delivers pre-exchange doses, missing dose reports, Automated Dispensing Machines (ADMs), loads, restocks and IV orders with accuracy.

The primary purpose of this position is to perform numerous tasks which are dissimilar in kind, but which have in common the purpose of assisting the work of one or more persons in an organization. The incumbent is responsible for aiding with a variety of problems and inquiries which are received in person, telephonically, or by referral from other secretaries, receptionists or support staff in the Executive Suite. Maintaining Appointments, Calendars and Schedules

Communications and Relationships 1. To communicate with the multidisciplinary healthcare team (doctors, nurses, dietitians, physiotherapists, occupational therapists, ward managers, and site practitioners), to intermediate care, general practice support and community pharmacists in the provision of medicine-related information and assessment of patients treatment. 2. To liaise with external bodies including local CCGs, regional pharmacy services, and necessary pharmacy networks. 3. We have an exciting opportunity to recruit a Band 7 Clinical Nurse Specialist in the Asthma Service. This is for a one -year fixed term contract.

The role will involve supporting the asthma service, and helping to develop and deliver pathways which result in excellent patient care and reduced waiting times.

There will be a focus on introducing an app- based tool to support patients with asthma management.

The potholder will support the wider asthma service in a variety of ways including, providing rapid access clinics, case finding, pre assessment telephone clinics, face to face clinics, community link and liaison between primary and secondary care. There will be an education component to the role, primarily for internal staff but also for our local community referrers. There will also be some audit and research opportunities. The potholder will also be responsible for mentoring junior members of the team.

The successful candidate will have at least two years’ experience working with patients who have asthma. This could be in a ward, community or outpatient setting. They must be able to practice autonomously and be self-directed, but also be able to operate efficiently as part of a team. They will demonstrate an eagerness to learn and develop and have a passion for education.

We have an exciting opportunity to join our recently expanded specialist in-house Apheresis team at Imperial College Healthcare NHS Trust (ICHNT) as a band 6 Charge Nurse.

The Clinical Haematology service runs one of the largest blood and marrow transplant programmes (allogeneic and autologous) in Europe, successfully achieving and maintaining JACIE accreditation status, while operating as specialist tertiary centre. We are a large unit with a total of 58 inpatient beds across three wards that include designated haematopoietic stem cell transplant (HSCT) beds. Our Apheresis service is currently located within day care and will be moving to upgraded facilities next year. We take pride in the fact that we provide a full programme of specialist Apheresis services 24/7 with on-call provision that extends to Critical Care and an outreach TPE Neurology service.
• Therapeutic apheresis is a highly specialist technology that provides a comprehensive range of treatment modalities that can involve the separation, removal and replacement of blood components and constituents. More recently, there has been an expansion in the application of advanced therapeutic medicinal products (ATMPS) for which apheresis plays a vital role in the collection of cellular products, enabling a wider range of diseases to be treated.
• Another area of growth in apheresis procedures has been that of automated red cell exchanges (RCE), which has become the NICE approved gold standard treatment for patients with sickle cell disease for which ICHNT operates as the haemoglobinopathy coordinating centre (HCC) for West London , providing an important therapeutic RCE programme with provision expected to grow across the North West London network.
• This is an exciting opportunity for a motivated haematology nurse who is keen to develop specialist Apheresis skills across a wide range of procedures, to increase awareness of regulatory requirements/standards, to further develop vascular access skills and to participate in research/clinical trials, as Apheresis services are shaped for the future.
