You will be providing occupational therapy leadership to the wider Living Well Centre and the occupational therapist within the focused support team. You will be managing a limited caseload of service users to assess, plan, implement and evaluate occupational therapy interventions. You will be providing occupational therapy leadership for students and/or junior staff, through supervision and appraisal. You will be participating in the planning, development and evaluation of occupational therapy and multi-disciplinary team (MDT) services, holding responsibility for defined projects.

You will be selecting and applying occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity (particularly paid employment) and leisure.

You will be working collaboratively with service users to identify occupational therapy goals as part of the overall care plan working within the care programme approach process.

You will plan and implement individual and/or group interventions, in collaboration with the service user, their family and friends and supporters and colleagues from across the Lambeth Alliance as well as the wider health networks.

RESPONSIBILITIES / ACCOUNTABILITIES: Working with the Clinical Nurse Specialists to support and maintain a clinically managed network of multidisciplinary professionals, enhancing the care of people with Heart Failure at local level and promoting evidence-based practice to ensure efficient and effective use of expert resources. Work closely with all members of the multi-disciplinary team across primary, community, secondary, social care, hospices, voluntary and wellbeing sector to promote the development of a seamless service ensuring good communication at all times. Be responsible for the assessment, planning, implementation, and evaluation of individualised nursing care to patients living in the community, always maintaining patient privacy and dignity. The service is provided five days per week. Deliver care in health centres, GP surgeries and community venues as required. To use a range of verbal and non-verbal communication tools to communicate effectively with patients and carers, to ensure an understanding of their condition so that patients are supported in the short- and long-term self -management of their condition, including gaining consent and discharge plans. Undertake clinical procedures within own scope of competency, utilising a variety of clinical instruments and equipment safely and precisely. Initiate, facilitate and participate in appropriate health enhancing activities in order to meet the needs of patients, promoting self-care. Communicate with a range of people in a form that is appropriate to them and the situation including the exchange of factual information with patients and carers, using persuasion and reassurance, effectively managing any barriers, dealing professionally, sensitively and in a non-judgemental manner whilst respecting patients dignity, wishes and beliefs and obtaining their consent for specific activities. Participate in managing and co-ordinating the work of the junior staff, offering support and development as appropriate. We are looking for a Occupational therapist to join our forward thinking and innovative team to continue to provide excellent rehabilitation to patients within their home environment.


The post holder has the opportunity to join the discharge home to assess service.

Discharge home to assess, assesses patient within the first 72 hours of discharge from hospital and provide up to 21 days rehabilitation.

The therapist will work alongside physiotherapists and therapy assistants, social workers, community nurses, GPs and care staff. Staff meet regularly for professional development and peer support, the post holder will have the opportunity to supervise and delegate tasks to junior staff and offer placements to occupational therapy students.
