To file/retrieve Health and Social Care Records on a daily basis and in accordance with agreed Trust Policies and Procedures. 2. To assist in maintaining the Health and Social Care Records in a tidy order while transferring between departments. 3. To ensure that the incoming/outgoing post is in a tidy order ready for despatch and on occasions deliver Health & Social Care Records to other areas. 4. • Working with patients who have undergone emergency or elective reconstructive surgery for trauma related injuries, cancer, burns and other minor/major injuries
• Being responsible for coordinating and providing complete care to allocated patients and work as part of the team
• Participating in or carrying out small to complex dressing changes in wound management
• Working closely with members of the multidisciplinary team, including Therapists and Psychologists, Doctors, and speciality Nurses
• Being actively involved in your own development and supporting junior members of the team in their development

You will be an excellent communicator all levels, always maintaining a people centred approach with the ability to build effective relationships and quickly establish your knowledge and credibility in this role. You will be fully versed in H&S legislation and have a NEBOSH and British Safety Council Diploma. You will be confident in influencing and advising stakeholders at varying levels.

The post holder is an experienced nurse, who is responsible for ensuring the delivery of safe and effective patient care to the practice population using autonomous clinical skills and a broad in depth theoretical knowledge base. The post holder will manage a diverse clinical case load, dealing with the presenting patients needs within a primary care setting. They will contribute to developing the nursing services offered by the practice. The practice nurse works independently and contributes to audit, research and supporting, implementing and evaluating local standards, policies, guidelines and care pathways. NMC requirements will be met, and the individual will work within the NMC Code of Conduct to agreed practice standards and protocols. Job responsibilities The post holder is accountable for his/her own practice and will work independently managing their own clinical workload responding effectively to patient needs and ensuring patient choice and ease of access to services. To provide input and advice on evidence-based practice and care interventions within their sphere of knowledge. Clinical Role Undertake procedures including but not limited to venepuncture, BP management, urinalysis, new patient health checks, ECGs, stitch/clip removal, and wound management. Administer child and adult immunisations and vaccinations in accordance with national and local programmes. Competent in anaphylaxis and resuscitation techniques. Ensuring safe storage, rotation and disposals of vaccines and drugs and that adequate stock levels are maintained Provide support and chaperoning to the doctors for minor surgery and other procedures Make professionally autonomous decisions for which he/she is accountable and provide safe, evidence based, cost effective, individualised patient care Assess, plan, develop, implement and evaluate treatment programmes for patients that promote health and well-being Make professionally autonomous decisions for which he/she is accountable and provide safe, evidence based, cost effective, individualised patient care. Help the practice develop and deliver specialist primary nurse led services, including chronic disease management (including for all areas of QOF). Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions. Maintains disease registers in liaison with administrative staff. Undertakes annual reviews of patients understanding and ability to self manage. Make appropriate referrals to other members of the primary, community and secondary care teams. Promote health and well being, giving general education advice on diet, contraception, smoking chronic disease management, exercise etc... Contribute to the practice achieving its GMS contract obligations including quality, organisational and access targets. Maintain accurate records according to NMC rules. Ensure all data protection requirements are met when gathering, recording and storing patient data. Liaise and maintain good working relationships with all members of the practice team. Work within the multi-disciplinary team within the practice and across the wider health system. To upskill and support Families Plus staff by providing specialist mental health consultation, reflective space, training, advice, support, and liaison to staff working with children, young people and their parents/carers To be link between CAMHS and Early Help services, providing referral information, support and guidance and signposting to appropriate services. To explore and help facilitate access to community CAMHS where access issues are difficult. To assess and formulate the mental health, emotional and psychological needs of children and young people who are accessing Families Plus To provide brief intervention where indicated to children, young people and their families. To include, attending multi-agency meetings To contribute to the CAMHS triage rota.

Speech and Language Therapist – Hedgeway
Location: Pilning, Bristol, BS35 4JN
Salary: £32,358.95 - £38,398.33 (FTE £37,669 - £44,699)
Hours: 40 hours per week – Term time only

The main purpose of the role:
The purpose of the role is to plan, provide and further develop an integrated Speech and Language Therapy service to the pupils at Hedgeway School, covering communication needs
Key duties:
Run social communication groups for students, either directly or supervising groups run by education staff.
Carry out speech and language interventions as described in students’ statutory documentation.
Carry out baseline communication assessments for all students.
Write SMART targets for education plans; and assess and monitor progress against these targets, amending where necessary
Lead and deliver training to the education team in social communication
Carry out a whole-school classroom language audit and provide recommendations for SLT.
Plan and implement individual communication programmes and provide accessible written guidelines which can be delegated and delivered by others.
Regularly monitor and review outcomes for communication aims/programmes and modify as required
Attend and contribute to annual reviews, provide clear and concise written reports.
Skills and Qualifications
Successful candidate qualities:
Qualifications:
To be a registered member of RCSLT and HCPC
To hold a professional Speech and Language Therapy degree or equivalent.
To work within the RCSLT clinical guidelines and HCPC standards.
Knowledge and experience:
Knowledge of a range of therapy interventions in learning disability
Knowledge of behaviour support
Knowledge of the impact of trauma and adverse early childhood experiences on communication
Knowledge of Autism Spectrum Conditions
Have at least two years’ post-graduate experience working as a Paediatric Speech and Language Therapist.
Working with children or young people with learning disabilities
Working with children or young people with an Autistic Spectrum Condition
Have experience or knowledge of delivering provision through the EHCP process
