Location: Preston
Salary: £36,658 - £41,669 per annum
Hours: Full-Time, 37.5 hours per week

About the Role
In this role, you will work within a specialist neuro rehabilitation unit, delivering holistic assessments and treatment plans for individuals with complex neurological needs. You will play a pivotal role in setting and reviewing SMART goals, supporting patients, families, and staff in achieving these objectives, and facilitating timely discharge planning.
This position offers a chance to work in a cutting-edge facility, utilising emerging technologies such as VR techniques and hydrotherapy, while collaborating closely with a dynamic multidisciplinary team.

Key Responsibilities
* Conduct comprehensive assessments and develop individualised rehabilitation plans.
* Utilize advanced clinical reasoning and evidence-based practices to deliver specialised therapy.
* Perform access and home visits across Lancashire and South Cumbria to assess and facilitate graded discharges.
* Maintain professional records, manage clinical risks, and ensure compliance with outcome measures.
* Collaborate effectively with team leaders, therapy and nursing staff, and the wider MDT.
* Engage in innovative practices, such as VR and hydrotherapy, to enhance patient outcomes.

About You
We are looking for a compassionate and skilled professional with:
* A degree in Occupational Therapy and HCPC registration.
* Substantial experience in rehabilitation, ideally within neurological settings.
* Evidence of relevant postgraduate training and ongoing CPD.
* Advanced skills in patient assessment, manual treatment, and risk management.
* Excellent communication and conflict resolution abilities.
* The ability to work autonomously and prioritise effectively.
