Plans, Formulates, and Implements National Policy and Programming: Assist with the development and sustainability of the forensic healthcare program to enhance the capacity of the Indian health system to provide high-quality multidisciplinary services and support for patients presenting for forensic healthcare and related services in collaboration with and as delegated by the Forensic Nurse Consultant. Provide recommendations to supervisors and senior leaders on forensic healthcare, projects, services, issues and trends. The incumbent serves as the Assistant Chief of Health Information Management (ACHIM). This position reports to the Chief of Health Information Management (CHIM). Major duties and responsibilities include but are not limited to the following:

The Community Placement Social Work Coordinator provides social work services to the Veteran population seen in Patient Aligned Care Team (PACT) clinics at the San Francisco VA Medical Center or in the Community Based Outpatient Clinics (CBOC). These services include but are not limited to placements in skilled nursing or other specialized settings, assessment of levels of care needed and available options, considering insurance benefits and family preference. Services are at the advanced practice level and include high quality case management, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the client. The Social Worker will also collaborate with the PACT Team to provide direct clinical services to patients and their families/significant others as needed. The Social Worker may follow certain patients who have been placed in specialized settings, coordinating with staff at those settings for long-term planning and re-connection with VA services.

This position is seen as the specialist in facilitating complex community placements. Advanced practice social work assessment and intervention skills assure patient safety, completion of referrals, and warm hand -offs to resource agency programs and providers in the setting of complex, high risk patient presentation and care. Volume and/or acuity require social work expertise in geriatrics including management of competency and functional decline, multiple medical and high-risk psychosocial issues. The ability to practice independently within this context is critical. Additional services consist of high quality assessment and intervention: assessment of coping in the community, including assessment of suspected abuse, especially elder abuse and issues related to capacity; interventions such as counseling to patients and families around adjustment to disability, illness and loss; crisis intervention; advance directive education; coordination of linkages to other appropriate VA or community service providers/agencies; short-term, focused case management; and consultation to multidisciplinary team members on the psychosocial aspects of individualized patient care. They serve as the technical expert in health record content and documentation requirements.

Incumbent serves as technical expert in health record content and documentation requirements. Incumbent is responsible for performing quantitative and qualitative reviews of health record documentation. Incumbent is responsible for ensuring that all patient care data entered in the Electronic Health Record (EHR) is accurate, timely, and completed. Adheres to established documentation requirements as outlined by The Joint Commission regulations, Veterans Health Administration (VHA) guidelines, as well as medical-legal requirements.

• Reviews records for adherence with CMS guidelines related to student, resident supervision and attending physician presence and documentation. Compiles reports to insure provider specific and service compliance. Maintains a control system to ensure completion of all inpatient and outpatient records in accordance with VA, The Joint Commission, CMS and other regulatory agencies standards.
• Participates in approved quality improvement focused reviews.
• Coordinates review activities with the Quality Improvement Program to satisfy external accreditation requirements and to satisfy current Medical Center performance measures.
• Assists with capture and reporting of health record documentation deficiencies as needed including downloading data and creating trended reports.
• Reviews health records to ensure that all records contain sufficient information to meet medical/legal requirements, to ensure continuity of patient care, and to support education and research needs. Assists clinicians with completion of delinquent/incomplete health records. The incumbent performs the quantitative and qualitative analysis on both electronic and paper-based health records.
• The incumbent assists physicians with the completion of delinquent/incomplete health records and serves as the expert resource for clinical staff regarding requirements for complete documentation.

**The salary range listed on this announcement only reflects the General Schedule incorporating Rest of U.S. basic pay. Upon selection, the salary will be adjusted to include the locality adjustment for the specific location.**Duties may include, but are not limited to:

• Works collaboratively with VISN programs, services, and interdisciplinary coordinated care team members to provide access to care.
• Routinely reviews computer medication profiles for therapeutic duplication, prescription noncompliance and allergies/drug reactions.
• Processes and submits patient requests for new prescription orders and renewals (including controlled substances) into the computer accurately, thoroughly and in a timely manner.
• Professionally handles issues escalated from team members and follows guidelines to address the issue appropriately.
• Leads the training of new staff.
• Assists with the training, orientation and re-training of employees to Pharmacy Service policies and procedures.
• Responsible for reviewing each subordinate's competencies within applicable areas of assignment on an annual basis.
• Assist in maintaining workflow patterns to ensure patient calls are answered in less than 30 seconds and that less than 5% of calls are abandoned by the patient while ensuring accurate record keeping.
• Assists other pharmacy managers and staff to identify areas of duplication of efforts and develops solutions to streamline processes, reduce waste, save costs and improve patient care.
• Completes additional duties as assigned.
