Registered Nurse with active NMC registration
Preference to experience working in a dementia setting

To produce comprehensive handovers, maintain confidentiality and ensuring all members of the team are up to date with the changes in presentation and give instruction as to agreed ways of working.
Manage the nursing care of a group of dementia registered persons, ensuring that care is appropriate, planned and reviewed regularly in line with the home’s policies and procedures.
To give prescribed medications, in a timely manner, ensuring residents have taken them in line with the home’s policy/procedures, correct medicines administration, storage and disposal legislation and reflecting the NMC code – Preserve Safety.
To deliver compassionate and empathetic nursing care to the residents, making sure that carers are knowledgeable of the plan of care and are delivering need assessed services in line with agreed ways of working.
To promote wellbeing in line with the law (Wellbeing Act 2014) and to be compliant with RISCA regulations.
Actively promote safeguarding standards on all shifts
To produce the highest standard of nursing documentation in all clinical assessment, care plans care reviews, daily entries, and professional visitor records. To clearly record and report any changes in a resident’s physical or mental health and actioning any assessment based outcome.
To ensure progress notes are produced and accurately reflect all the care interventions provided and their outcomes.
To protect the privacy of resident’s personal information and ensure that it is available only to relevant persons in line with NMC code of practice.
To address any duties laid out in the unit diary for the day and ensuring that all matters that require forwarding are done so appropriately.
To ensure that the environment is safe for all residing there, staff and all visitors by immediately raising health and safety concerns in the appropriate way.
To refer to outside organisations when needed to maintain the best standard of holistic care possible
Be a role model for excellent infection control measures within the unit.
Manage a team of nursing assistants, senior carers, and junior staff, ensuring inclusivity and diversity.
Build and maintain professional relationships with family and visitors that are attentive, friendly, warm and informative ensuring that people are able to approach the nurse with their concerns.
Where needed provide clinical supervision for junior members of the team and provide a healthy learning support structure for any students attending
Ensure that staff are given the opportunity to learn and develop their skill set within care giving remit by supportive instruction.
Manage conflict in a professional but sensitive way
Be a role model for professionalism in all aspects of the role
Appropriately record and report any staff sickness or absence
Take an active part in arranging cover as needed to ensure that the resident care delivery is seamless.
Monitor breaks to regulate compliance and maintain a safe environment
Communicate clearly and effectively to all departments in both verbal and written formats, ensuring that information has been correctly understood.
Demonstrate good working practice with visitors both personal and professional demonstrating curtesy and respect in all communication.
Receive and make telephone calls as needed appropriately and in a timely manner – carry the unit’s mobile phone where needed in order to facilitate calls from reception and outside of the home.
Write legible 24 hour reports for the home manager
Document in such a way that the information written is accurate contemporaneous and clear
Communicate only in agreed ways ensuring that the GDPR principles are upheld and that guidelines by the IOC for social media are followed in line with the home’s policies and procedures.
Provide mental health support and intervention for residents with BPSD
Medicines management expertise, storage, disposal, dispensing and administration safely using the 5 R,s
Assessment of the unwell resident, utilising NEWS 2, taking the relevant actions in line with the management of the deteriorating patient.
All monitoring and observations needed to ensure that residents physical health is being maintained optimally
Skin health management, ANTT, wound care and dressings after training and assessment
Catheter and Stoma, insertion and removal after training and assessment
Venepuncture after training and assessment
Syringe Driver management after training and assessment
