Information regarding the Neurodiversity Pathway The Neurodiversity Pathway is a developing integrated pathway within CFHD. The Integrated Neurodiversity Team provides specialist assessment and intervention for children and young people and their families where the presenting need relates to neurodevelopmental conditions such as Autism Spectrum Condition, Attention Deficit Hyperactivity Disorder and Tics and Tourette Syndrome. Job Role:
• To provide clinical and administrative support to the Dental Officer to enable maintenance of the surgery. This includes preparation/sterilisation of all instruments before and after patients, reception duties, document handling, operation of dental computing system, processing radiographs, shared maintenance tasks and working in the central sterilisation area when in place.
• To be fully aware of the responsibilities of a registered healthcare professional and comply with the GDC Principles and Standards for Dental Professionals.

More detail about the role Our role can be challenging and is time sensitive in order to support patient flow through the acute hospital; but it is also rewarding and varied. You will have the opportunity to work in many different departments and wards throughout the acute site, liaising with many different teams and other specialists in order to facilitate the appropriate discharge requirements for the patients that are referred to us. We also follow patients into the community to provide acute hospital at home care. We work closely with our community colleagues and have established ourselves as a link between acute and community care in order to provide seamless transition of care between hospital and home or other receiving destination. We are currently a part of developing a Frailty pathway and are involved in many initiatives around length of stay and discharge planning and frequently receive excellent feedback from our patients as well as our professional colleagues. We require a forward thinking, dynamic, motivated and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold. Experience from both acute and community backgrounds would be preferred, but is not essential. Experience and understanding of the Continuing Healthcare Framework, Mental Capacity Act and Safeguarding would also be an advantage, but training can be provided with regards to these. More detail about the role We are a busy 24 bedded emergency surgical assessment unit caring for emergency admissions in the specialities of colorectal, urology and gynaecology including early pregnancy complications. Within the ward we have a separate ambulatory assessment unit, taking admissions from the emergency department and GP referrals and a surgical emergency outpatient clinic and our staff work within all of these areas. To work in collaboration with the Ocean View multi-disciplinary team, to develop robust discharge plans with and for service users. This will involve working with service users from admission to the ward, through their in-patient stay, and post-discharge in the community. The post holder will apply the core principles of recovery in discharge planning and preparation, supporting the multi-disciplinary team to help service users to develop and practice essential skills for independent living. The post holder will support the initial assessment and discharge action planning for service users admitted to the ward, and provide follow through support once they have been discharged. Main duties of the job The CYP IAPT Recruit to Train programme involves both academic learning, coursework and supervised clinical practice. Alongside this, the post holder will work with colleagues in a specialist CFHD pathway, building relationships and wider skills in working children and young people with complex emotional and mental health needs in the community. This will include evidence-based assessment and outcome-monitored interventions in conjunction with the training. There will also be opportunities to engage in Participation and service development as well as contributing to service delivery within the team as required. Post holders will be employees of CFHD on a substantive contract and also enrolled on a Postgraduate Diploma course with Exeter University. It is essential that applicants are able to balance a busy work and study schedule. As this is a Postgraduate level training, you will be required to study and perform at academic level and be prepared to work towards their studies outside of identified study time. Study time and/or teaching days are typically two days a week during term. As these are training positions initially, please be aware that you must both gain employment in an CFHD SMHP Recruit to Train role and be accepted onto the University Course.
