The incumbent serves as the Assistant Chief of Health Information Management (ACHIM). This position reports to the Chief of Health Information Management (CHIM). Major duties and responsibilities include but are not limited to the following:

• The incumbent serves as the Assistant Chief of Health Information Management (ACHIM) in a medical facility of high complexity (level 1), with responsibility for the supervision of all HIM functions. This position is located at the Clement J Zablocki Medical Center, Milwaukee, WI and is organizationally aligned under the Medical Information Systems Division (MIS). The primary purpose of this position is to function as a Program Analyst responsible for managing the Care in the Community (CITC) budget with multi-million dollar expenditures annually.
-Incumbent will formulate and maintain CITC budget per program based on need.
-Advise staff as distribution of CITC funding.

The Community Placement Social Work Coordinator provides social work services to the Veteran population seen in Patient Aligned Care Team (PACT) clinics at the San Francisco VA Medical Center or in the Community Based Outpatient Clinics (CBOC). These services include but are not limited to placements in skilled nursing or other specialized settings, assessment of levels of care needed and available options, considering insurance benefits and family preference. Services are at the advanced practice level and include high quality case management, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the client. The Social Worker will also collaborate with the PACT Team to provide direct clinical services to patients and their families/significant others as needed. The Social Worker may follow certain patients who have been placed in specialized settings, coordinating with staff at those settings for long-term planning and re-connection with VA services.

This position is seen as the specialist in facilitating complex community placements. Advanced practice social work assessment and intervention skills assure patient safety, completion of referrals, and warm hand -offs to resource agency programs and providers in the setting of complex, high risk patient presentation and care. Volume and/or acuity require social work expertise in geriatrics including management of competency and functional decline, multiple medical and high-risk psychosocial issues. The ability to practice independently within this context is critical. Additional services consist of high quality assessment and intervention: assessment of coping in the community, including assessment of suspected abuse, especially elder abuse and issues related to capacity; interventions such as counseling to patients and families around adjustment to disability, illness and loss; crisis intervention; advance directive education; coordination of linkages to other appropriate VA or community service providers/agencies; short-term, focused case management; and consultation to multidisciplinary team members on the psychosocial aspects of individualized patient care. Services are timely, comprehensive and effectively coordinated with all other healthcare providers. Psychosocial assessment and intervention is integrated within a complex treatment plan. • Advocates with emergency responders for the delivery of the appropriate level of emergency intervention of police and other emergency responders for callers, as appropriate with the ability to understand the variety of resource options available nationally and internationally, with response teams.
• Acts as the sole point of contact for VA Suicide Prevention Officers (SPCs), community law enforcement, local emergency dispatch, hospital emergency room personnel and telecommunication companies.
• Relays real time information from the responder to emergency dispatch, law enforcement and emergency medical personnel.
• Provides updates to the supervisory team as needed on progress of ongoing rescues and alerts supervisory team immediately if there are any barriers to providing emergency services to those in crisis to include a failed rescue.
• Makes outbound calls to 911 dispatchers, emergency medical providers, and VAMCs to ensure that the Veteran was located, transported or otherwise is receiving treatment as expected.
• Contacts community medical centers and VAMCs to verify that Veterans arrived at the facility based upon the agreed upon Facility Transport Plan that is coordinated between the responder and Veteran.
• Notifies the supervisor if the Facility Transport Plan fails and initiates rescue as needed.
• Documents all contact attempts and escalates urgent or emergent contacts to the Supervisor.
• Utilizes multiple software programs and Internet search engines concurrently along with critical, creative thinking to locate imminently suicidal Veterans.
• Multitasks in high risk, high stress situations is necessary to expedite Veteran rescues.
• Locates Veterans using various Internet resources as well as VA databases.
• Receives inbound call center administrative calls and/or calls from SPCs and responds to any questions, escalating emergent situations to Supervisor.
• Relays information from local law enforcement and dispatch to VCL leadership to educate and advise leadership on changes / trends with procedure and protocol within the law enforcement community.
• Performs other duties as assigned.

The duties of this position include but are not limited to the following:
Assists the Chief, Health Information Management (CHIM) and the HIM Department in identifying and setting short and long-range goals, program objectives, and tasks to meet the mission and vision of the VAMC.

Provides subject matter expertise to Service Line Leaders, and medical center staff. Interprets and applies The Joint Commission (TJC) standards, VHA regulations and medical-legal requirements; current federal codes pertinent to health records; VHA policy memoranda, directives, handbooks and program guides pertinent to health information management; medical coding (ICD, CPT, HCPCS) and reimbursement methodologies; medical and procedural terminology; anatomy and physiology; regulations governing confidentiality of health records; release of information laws and statutes; and all policies/procedures pertaining to VHA health records.

Provides training for HIM, clinical and administrative staff on HIM topics. Designs and performs quality improvement projects regarding HIM issues. Assists the CHIM in preparing and conducting various presentations.

Keeps current on emerging issues and trends in the HIM field, including process improvement initiatives, the electronic health record (EHR), coding and computer­-assisted coding, the revenue cycle, release of information and privacy issues, records management, medical speech recognition, transcription and data entry, and health information exchange. Keeps HIM and clinical staff abreast of changing technology in electronic systems used to collect, process, code, protect and store health information. Participates in various VAMC, VISN and national committees requiring HIM subject matter expertise.

Performs audits and analyses, preparing and presenting findings and recommendations to the CHIM, Service Line Leaders and medical staff. Assures the accuracy and timeliness of the capturing of patient services charges (workload) in accordance with all applicable laws and regulations. Reviews, analyzes and reports HIM performance monitors. Advises Principal Investigators and other clinical staff on methodologies related to retrieval of health care data for specific studies. Performs data retrieval for special studies as needed.

Utilizes advanced knowledge of VHA data sources, such as VHA Support Service Center (VSSC) and the Corporate Data Warehouse (CDW), to complete analyses of VA Medical Center data. Analyzes and evaluates on a quantitative basis, the effectiveness of the HIM program and operations in meeting established goals and objectives. Analyzes program performance data to identify areas needing improvement.

May review and test HIM related software patches; test and troubleshooting EHR menu options and functional issues when HIM users report application errors; perform equipment needs analysis for new or replacement equipment and/or equipment
repair; submit work orders or Remedy tickets for correction to HIM software errors. Is the facility expert for software and computer applications related to HIM to provide technical assistance to HIM staff (e.g., coding, ROI, dictation, and medical speech recognition software).

Serves as point of contact for health record corrections to ensure documentation accurately reflects care delivered and reported. Documentation corrections may consist of retracting erroneous notes, re-titling notes, assigning notes to or removing from consult requests. Collaborates on improperly identified documents with clinicians, Clinical Applications Coordinators (CACs) and others who report problems. Reassigns, rescinds, and removes documents when they have been entered or uploaded incorrectly.

Police Operations Oversight & Improvements 25%

• Developing, modifying, and implementing programs and systems to ensure efficient execution of police operations
• Conduct site visits, tracks and assess police program operations and performance within the VISN
• Performs focused reviews of police reports to identify trends and ensure the reports are legally and technically sound
• Conducts program pre-assessments of Medical Center/Healthcare System police programs
• Provides SME level input to prepare recommendations for Police Chiefs, Medical Center and Network Directors regarding deficiencies and best practices in all areas
• Assists in the coordination, preparation, and staffing of special purpose visits from dignitaries and high-ranking government representatives
• Functions as the primary VA Law enforcement and security liaison and oversight representative of the VHA Office of the Senior Security Officer

• Participates in studies, surveys, and inspections designed to determine the effectiveness and adequacy of the VHA physical security program
• Formulates methodologies and strategies for implementing physical security reviews to ensure compliance with security objectives, policies, and procedures
• Reviews the designs, plans, and specifications for new security systems and current system upgrades
• Serves as the VISN expert on the Interagency Security Committee's Risk Management Process and how it applies to the VHA facilities
• Assist in developing the VISN internal risk assessment methodology and will maintain records related to the conduct of risk assessments

• Acts as the Network level expert on law enforcement and security matters affecting facilities with the VISN
• Assist in developing VISN and local policies and implements correct procedures to alleviate security problems with access control, surveillance systems and technical equipment, and employee training programs
• Monitors field-based actions to ensure that the VA Integrated Operations center's (VAIOC) goal of reporting serious incidents within the department level established policies
• Reviews, evaluates and recommends policy on police equipment, computer software, physical security measures, and crime preventive initiatives

• Assists the VISN &VHA Senior Security Officer in regular examinations or studies of programs and staff performance to identify and report operating conditions and police misconduct that have actual or potential adverse effects upon VA objectives and reputation; reflect inadequate standards of police service employee behavior, integrity, and adequacy; indicate lack of compliance with policies and regulations; and/or involve lapses or abuses of authority
• Prepares reports based on analysis and evaluation of evidence
• Assures that each investigative file is properly secured and maintained n a current basis

Assigns codes to documented patient care encounters (outpatient and/or inpatient professional services) covering the full range of health care services provided by the VAMC. Patient encounters are often complicated and complex requiring extensive coding expertise. Applies advanced knowledge of medical terminology, anatomy & physiology, disease processes, treatment modalities, diagnostic tests, medications, procedures as well as the principles and practices of health services and the organizational structure to ensure proper code selection.

Selects and assigns codes from the current versions of the International Classification of Diseases (ICD), Current Procedural Terminology (CPT), and/or Healthcare Common Procedure Coding System (HCPCS).

Adheres to accepted coding practices, guidelines and conventions when choosing the most appropriate diagnosis, operation, procedure, ancillary, or evaluation and management (E/M) code to ensure ethical, accurate, and complete coding. Also applies codes based on guidelines specific to certain diagnoses, procedures, and other criteria used to classify patients under the Veterans Equitable Resource Allocation (VERA) program that categorizes all VA patients into specific classes representing their clinical conditions and resource needs.

Performs a comprehensive review of the patient health record to abstract medical, surgical, ancillary, demographic, social, and administrative data to ensure complete data capture. Patient health records may be paper or electronic. The abstracted data has many purposes, for example, to profile the facility services and patient population, to determine budgetary requirements, to report to accrediting and peer review organizations, to bill insurance companies and other agencies, and to support research programs.

Expertly searches the patient health record to find documentation justifying code assignment based on an expanded knowledge of the organization and structure of the patient record.

Orients and instructs new personnel and/or students from affiliated health information or medical record technology programs, at the direction of the supervisor, on unit operations, coding, abstracting, and use of an electronic health record.

Works within a team environment; supports peers in meeting goals and deadlines; flexible and handles multiple tasks; works under pressure; and copes with frequently changing projects and deadlines.

Codes inpatient professional fee services for identified inpatient admissions. Code selection is based upon strict compliance with regulatory fraud and abuse guidelines and VA specific guidance for optimum allowable reimbursement.

Codes all Operating Room procedures reported in the Surgical Package of the VistA hospital system; applies ICD and CPT coding guidelines and selects proper codes using the current code set and the encoder product suite; ensures all procedures file to the appropriate Patient Care Encounter (PCE); adds Anesthesia and Pathology codes to the PCE encounter for all billable surgical cases.

Reviews and codes assigned fee service Care in the Community outpatient encounters using the paper or electronic documentation obtained from non-VA facilities such as Community Hospitals, Emergency Rooms, military facilities, etc.

Codes diagnoses from paper forms for VA registries such as Agent Orange, Ionizing Radiation, Persian Gulf, Prisoner of War, etc.
