Operational

To support the delivery of an effective integrated healthcare service clinical model.

To manage a complex clinical caseload.

To support the development, implementation and maintenance of the National Standards for Health Care in Prisons for the service.

Delegate appropriate responsibility and authority to team members, whilst retaining overall responsibility and accountability on a shift-to-shift basis.

To maintain an effective case management service to ensure care and continuity of treatment is maintained throughout a patient care pathway whilst detained in prison.

To implement appropriate, effective and accredited care planning tools that are subject to the process in accordance with agreed Trust Policies.

To implement effective systems and interfaces with community and acute health services ensuring continuity of care on release or discharge. To actively promote and demonstrate working across professional and organisational boundaries.

To maintain close working partnerships with all services that may be involved with the care and management of offenders, (prison staff, Ministry of Justice, Trust & Directorate senior managers & relevant forums, CMHT staff, NHS Hospitals, Probation Services, CPS, Courts, GP's, community health services, Housing/Benefit services, social care, community multi­ agency forums etc.).

Human Resources

Responsible for day-to-day co-ordination of the Integrated Healthcare Team.

To ensure systems are in place for clinical supervision to junior team members including annual appraisal and identification of specific service, training, and personal development needs.

Governance

Ensure junior staff compliance with all Prison and Trust clinical policies and procedures as appropriate.

Ensure appropriately skilled & qualified staff comply with the administration and management of medications according to their responsibilities in accordance with all relevant policies, guidance, and procedures.

Ensure high standards of electronic record-keeping competencies and offender confidentiality in line with trust & prison policy and systems, through effective monitoring and auditing processes.

General

To liaise with other senior Trust personnel to share expertise, promote ideas and to ensure the service is working collaboratively with the Trust's other Directorates as required.

• To make clinical judgments within your clinical ability where information is highly complex or lacking or ambiguity exists. Where such judgments are outside of your clinical ability to refer the situation to a more senior pharmacist or GP for assistance.

• As part of the health and wellbeing team, identify patients at risk of admission to hospital and work to manage medicine-related risk for these patients.
• Implement improvements to patients medicines for LTCs, including de-prescribing and run LTC clinics where medicines are a large component of care.
• Develop and manage new services that are built around new medicines or NICE guidance.

Leadership, Management and Training
• To devise and implement SystmOne computer searches to identify cohorts of patients at high risk of harm from medicines.

• To provide leadership to the Healthcare Manager and GPs to ensure that practice is compliant with CQC standards where medicines are involved.
• As part of the Health and Wellbeing model, to contribute to public health campaigns, including flu vaccinations and adult immunisation programmes using specialist knowledge on immunisation. To ensure all relevant PGDs for immunisation are in place and staff authorised to administer using a PGD have received the relevant training.
• Analyse, interpret and present medicines data to highlight issues and risks to support decision making.
• Identify and provide leadership on areas of prescribing requiring improvement. Present results of audits and provide leadership on suggested changes.
• To develop and manage new services that are built around new medicines or NICE guidance, where new medicine/recommendations allow the development of a new care pathway.

• Support the Specialist Prison Service Pharmacist in providing education and training to the pharmacy team members as required.
• Identify from SystmOne patients prescribed medication that should be prescribed or initiated by Specialists/hospital doctors or subject to shared care and liaise directly with hospital colleagues to ensure prescribing and dispensing of such medicines is appropriately undertaken and adheres to the requirements of the secure environment.
• Develop, implement and maintain the prison formulary, in liaison with other OPS colleagues, and ensure this is updated onto SystmOne locally when changes are made.
• Manage the process if implementing changes to medicines resulting from MHRA alerts and product withdrawals and provide guidance for practitioners.

Clinical
• Work with the dispensary and Primary Care teams to identify patients at risk from high risk medicines to minimise such risks through medicines optimisation. Provide scheduled clinics for offenders across Rochester and West Kent to give advice and support on their prescribed medicines, OTC medicines and other medical issues they may raise.
• To put in place changes to reduce the prescribing of medicines likely to cause readmission to high-risk patient groups.
• Promote healthy lifestyles for offenders in line with local and national guidelines or promotions.
• To be an active member of the Rochester clinical governance/medicines management committee(s).
• To provide face to face clinics to help patients with questions, queries and concerns about their medicines.
• To hold clinics for patients requiring face-to-face Clinical Medication Reviews (CMRs) i.e. a review of the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their concordance with prescribed medication.
• To undertake clinical medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.
• To see patients in multi-morbidity clinics and in partnership with the Health and Wellbeing coordinator(s), implement improvements to the patient’s medicines, including deprescribing.
• Run own long term conditions clinics where responsible for prescribing as an independent prescriber for conditions where medicines have a large component (e.g. stable angina, warfarin monitoring and dose adjustment).
• To demonstrate on-going continual professional development (CPD) and be responsible for self-development and learning within the field of pharmacy or other specialty where appropriate.
• Make recommendations for and manage changes to medicines (switches) designed to save on medicines costs where a medicine or product with a lower acquisition cost is now available.
• Answer all medicine-related enquiries from GPs, other healthcare staff and patients with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes.

Oxleas NHS Foundation Trust provides a wide range of mental health and community health services. These include acute adult and elderly mental health units and community teams based at a number of locations in Bexley, Bromley and Greenwich boroughs, and intermediate and elderly care units based at Queen Mary’s Hospital (QMH), Sidcup, and Eltham community hospital.

The Trust has dispensaries located at QMH and the Bracton campus, Dartford; both are registered with the GPhC and the QMH pharmacy store holds a wholesale dealers licence. Within Oxleas, clinical pharmacists are an integral part of the multidisciplinary team, with regular ward attendance and provision of a medicines information service which includes a patient telephone advice line.

Pharmacy also provide support to patients within the community, including to Community Mental Health teams, a specialist perinatal clinic and a home assessment Medicines Optimisation service to patients within Bromley.

Key tasks and responsibilities
• To provide clinical pharmacy services to mental health, intermediate care and elderly care inpatient units, to ensure patients receive appropriate medication on admission, have their medication and adherence reviewed during their stay, and have a clear medicines management plan on discharge.
• To provide information about the use of medicines to consultants, other prescribers, other health care professionals, patients, relatives and carers.
• To support professional leads in identifying and addressing issues around the use of medicines that are relevant to meeting CQC standards, for example advising on the safe and secure storage of medicines, and contributing to the audit and review of controlled drug processes.
• To work with the Medication Safety Officer to implement medicines safety initiatives within designated wards.
• To record information as required on clinical pharmacy activity.
• To take professional responsibility when clinically screening prescriptions to ensure the accuracy, legality, safety and efficacy of prescribed medication, including liaising with doctors and nurses to resolve any queries arising from the screening process.
• To seek advice from senior pharmacists where necessary.
• To participate in the dispensary rota as needed, working independently to liaise with doctors and nurses to resolve any queries arising from the screening process and to undertake the final accuracy check on prescriptions dispensed by other pharmacy staff.
• To participate in the medicines information rota, providing clinically sound, evaluated information on a wide variety of complex enquiries to service users and all grades of health care professionals.
• To participate in other internal rotas as required.
• To undertake any other relevant duties, as may be required by the Chief Pharmacist.

• To put in place changes to reduce the prescribing of medicines likely to cause readmission to high-risk patient groups.
• Promote healthy lifestyles for offenders in line with local and national guidelines or promotions.
• To be an active member of the Rochester clinical governance/medicines management committee(s).
• To provide face to face clinics to help patients with questions, queries and concerns about their medicines.
• To hold clinics for patients requiring face-to-face Clinical Medication Reviews (CMRs) i.e. a review of the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their concordance with prescribed medication.
• To undertake clinical medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.
• To see patients in multi-morbidity clinics and in partnership with the Health and Wellbeing coordinator(s), implement improvements to the patient’s medicines, including deprescribing.
• Run own long term conditions clinics where responsible for prescribing as an independent prescriber for conditions where medicines have a large component (e.g. stable angina, warfarin monitoring and dose adjustment).
• To demonstrate on-going continual professional development (CPD) and be responsible for self-development and learning within the field of pharmacy or other specialty where appropriate.
• Make recommendations for and manage changes to medicines (switches) designed to save on medicines costs where a medicine or product with a lower acquisition cost is now available.
• Answer all medicine-related enquiries from GPs, other healthcare staff and patients with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes.

We would like to recruit an enthusiastic and experienced Mental Health Nurse to our Intensive Case Management in Psychosis Team (ICMP) based at Carlton Parade in Orpington.

2. Review the expiry dates and storage facilities for all such medicines and report any deficiencies to the prison service pharmacist for that prison.
3. Assist the prison service pharmacist to provide advice to prisoners and staff about the appropriate use of medicines.
4. Undertake 3 monthly controlled drugs stock checks and report any issues to the Prison Service Pharmacist for that prison.
