Key Result Areas & Performance: Leadership & Service Development 1. To strategically develop the pharmacy services to Cancer Services to support the needs of the division and the Trust, specifically by implementing pharmacist prescribing and advanced clinical practitioner roles. 2. To identify areas of potential cost-savings within Cancer, and to implement systems to realise relevant savings. 3. To support the Trust in monitoring drug expenditure in Cancer in order to provide accurate expenditure information, with appropriate analysis, to the appropriate division. To provide budgetary advice and financial information on medicine costs to the Director of Pharmacy and deliver on cost improvement plans. 4. To attend financial meetings when required. Communications and Relationships 1. To communicate with the multidisciplinary healthcare team (doctors, nurses, dietitians, physiotherapists, occupational therapists, ward managers, and site practitioners), to intermediate care, general practice support and community pharmacists in the provision of medicine-related information and assessment of patients treatment. 2. To liaise with external bodies including local CCGs, regional pharmacy services, and necessary pharmacy networks. 3. The post-holder will be required to communicate clearly and sensitively about patient medication issues both internally and externally. 4. To demonstrate good communication skills as the role liaises with a wide variety of people within the Trust. This will include service and general managers across all directorates, trust risk managers, directorate governance leads, multidisciplinary team members and all Pharmacy staff. 5. To provide training on medication-related issues, policies, guidelines and service developments to pharmacy colleagues, nursing and medical staff, ranging from one on one communication to more formal presentations. Job Summary The post holder will work as part of a multi-disciplinary team in a patient-facing role. Proactively manage patients with co-morbidities in the area of chronic disease management within the practice. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. Supporting the Individual: As well as the developmental support that Soar Beyond and the SMART platform provide our staff, we also have a multi-layered support system around our PCN Clinical Pharmacists. This support network includes: - Line management support from the GP Federation and the clinical leads within the organisation - PCN Level support from the PCN Clinical Director and practice leads - PCN pharmacist team peer support and oversight from our clinical pharmacist lead - Practice-based support from your hosting practice(s) on day to day case review - General support from our local medicines optimisation teams The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practices. The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. The PCN have invested in support from an organisation called Soar Beyond. Soar Beyond specialise in supporting the creation of teams of pharmacists across a network of practices. It is supported by a web platform called SMART which assesses pharmacist competency levels in a range of different disease areas and is backed up by training programmes to develop the individual where there might be gaps that the individual and the PCN would like to be filled. Currently the team is working towards competency in Diabetes management and the PCN is implementing a pharmacist led Diabetes pathway to be used across practices. Our PCN Pharmacists work to support practices in the day to day activities that are required in the busy primary care space but they also work as a PCN team to collectively tackle the PCN level targets and projects. Principal duties and responsibilities Patient facing Clinical Medication Review Undertake clinical medication reviews with patients with multimorbidity and poly-pharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests Patient facing medicines support Provide patient facing clinics for those with questions, queries and concerns about their medicines in the practice. Management of medicines at discharge from hospital To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes manage these changes without referral to a GP. Perform a clinical medication review, produce a post discharge medicines care plan including dose titration and booking of follow up tests and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Work in partnership with hospital colleagues (e.g. care of the elderly doctors and clinical pharmacists) to proactively manage patients at high risk of medicine related problems before they are discharged to ensure continuity of care. Patient facing Long term condition Manage own case load and run long-term condition clinics where responsible for prescribing as an independent prescriber (if suitably qualified) Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation). Telephone medicines support Provide a telephone help line for patients with questions, queries and concerns about their medicines. Medicine information to practice staff and patients Answers medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes. Unplanned hospital admissions Devise and implement practice searches to identify cohorts of patients most likely to be at risk of an unplanned admission and readmissions from medicines. Telephone triage Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time e.g. pathology test results, common/minor ailments, acute conditions, long term condition reviews etc. Repeat prescribing Produce and implement a practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates; make necessary changes as an independent prescriber, and ensure patients are booked in for necessary monitoring tests where required. Information management Analyse, interpret and present medicines data to highlight issues and risks to support decision making. Medicines safety Identify national and local policy and guidance that affects patient safety through the use of medicines, including MHRA alerts, product withdrawals and emerging evidence form clinical trials. Manage the process of implementing changes to medicines and guidance for practitioners.

The post holder will provide support and guidance to the recovery team within the clinical area, and will assist with the training and development of junior staff and students. The post holder will take responsibility for running operating lists in conjunction with and in the absence of a Senior Theatre Practitioner. As a member of the team, participates in establishing a positive working environment in which patients receive a high standard of care delivery. Personally deliver a high standard of care to patients and participate in assessing, planning and evaluation of individualised patient care. Participates in the care, custody and administration of medicines in accordance with Trust and local policy. Assist in developing the department philosophy and framework of care in line with unit/organisational and beliefs/values. Ensure the provision of advice, support and health information to patients and their carers in order to promote a health-based service. Ensure workload of colleagues does not jeopardise safe standards of practice and initiate action if necessary. Participate in the daily checking and maintenance of essential equipment. Transports/manages people and/or items such as equipment or specimens safely and to time consistent with legislation, policies and procedures. Maintain accurate and legible records of all clinical and legal documentation to include accident / incident reporting, with regard to HCPC/NMC statutory and Trust polices. Ensures all aspects of clinical care including interventions and investigations are performed, assessed and reported upon in a timely fashion and in a manner consistent with legislation, policy and procedures. Liaise with all members of the multi-professional team to ensure that care is assessed, planned, delivered and evaluated within a multi-professional framework, in the interestsof the patient, and that care is communicated to all team members.
