· The provision of high quality paediatric Occupational Therapy services to a diverse caseload of children and young people who are experiencing difficulties with daily occupations.

· Supporting junior members of the team, as well as students on placement

· To assess, fabricate and monitor the use of splints and orthoses to support Upper Limb function

· Provide assessment and advice regarding postural/seating systems

We currently have an exciting opportunity to join the Children’s Community Nursing Service in our Inner London Division We are seeking a highly motivated and enthusiastic Band 6 Children’s Community Nurse who is looking to share and develop their skills in the largest community healthcare trust in London

The post holder will work within the Children’s Community Nursing Team, visiting children in the community with a wide range of conditions, with the aim of reducing hospital admissions and attendances. The role will include nursing care, training, and supporting families, as well as deputising for Team leads, and supporting and mentoring student nurses.

The team provides a service to children and young people aged 0 – 18 years, and works across the 3 London Boroughs of Kensington and Chelsea, Hammersmith and Fulham, and Westminster. The team works closely with colleagues in Primary Care, hospitals, social services and education.

One post is for 37.5 hours per week, which will be worked flexibly over 7 days a week. This post offers a great opportunity for a children’s qualified nurse to develop and enhance skills and experience in community nursing. You will also receive regular clinical supervision and safeguarding supervision.

The Haemoglobinopathy service is seeking to recruit a full-time community haemoglobinopathy nurse with a background in children's nursing to join the service. This job description covers both children and adult nursing, but specific clinical duties will be determined by the post covered. · Be an active member of the clinical team, providing a comprehensive specialist Speech & Language Therapy service to residents of Barnet.

· To be independent with managing both communication and dysphagia caseloads.

· Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/patients and maintaining professional documentation.

· Build up and maintain key relationships with referring services/ Inpatient Rehabilitation Units/ Learning Disabilities / HEFT Service / Parkinson’s / voluntary and third sector organisations and acute stroke services and members of the multidisciplinary team.





Be responsible for gaining valid, informed consent for assessment, treatment interventions and sharing of information in compliance with legal and professional guidelines. To be able to interpret and follow these guidelines in circumstances where patients do not have capacity to provide informed consent.

Have specialist working knowledge of clinical guidelines, specific to the area you are working in and, with the guidance of your supervisor, to translate these guidelines into clinical practice.

· To assess the need for, develop and implement Speech and Language Therapy treatment to adults referred to the service with a wide range of speech, language, communication and swallowing difficulties

Perform uni-professional and multi-professional specialist assessments and re-assessments of treatment programmes using evidence-based practice and in accordance with RCSLT guidelines.

Be responsible for organising and prioritising own caseload to meet service needs and to communicate with your supervisor, team lead and/or manager, if the service needs cannot be met.

· Support the members of the team in the day-to-day operation of the service. Respiratory Specialist Nurse – Band 7 - Wandsworth

South West Division – Wandsworth Specialist Nursing

Due to promotion and progression we have ta nurse specialist roles available within our community Specialist Services team. We are looking to recruit a highly motivated and compassionate registered nurse with the skills and knowledge to fulfil this role.

The successful candidate will be expected to demonstrate a high level of expertise clinically, have excellent communication skills and the ability to work flexibly with consideration to the needs of the service.

The post holder will spend at least 80% of the time providing patient facing or related care.

To work within a multidisciplinary respiratory team to deliver community respiratory service in South West division- Wandsworth providing specialist services including hospital at home (admission avoidance and early supported discharge scheme), home oxygen assessment & review service, nurse led clinics, stable disease management and home visits to provide expert specialist care to patients with suspected or confirmed respiratory diagnosis including, but not limited to: COPD, asthma and bronchiectasis,
