Overview
Works under the supervision of the Nurse manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients. The RN may work in a variety of settings in the Critical Care/Telemetry Complex and roles within Riverside Health System. • Assesses, implements, and evaluates pharmaceutical care that contributes to the physiological and psychosocial outcomes and stability of the patient. Monitors, screens, and verifies all specialty medication and chemotherapy treatment plans for accurate dose, indication, route, treatment parameters, and administrative schedule. Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug, or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, failure to receive drugs, untreated indications, treatment failures, and patient adherence issues.
• Provides drug information for patients and team members. Participates patient education related to pharmaceutical care and disease state management. Monitors patient progress, reconciles data discrepancies, handles specialty pharmacy inquiries, provides departmental updates, and promotes services to RMG practices. Provide statistical data related to specialty pharmacy capture rate and leakage of prescriptions from the RMG practice sites.

Documents appropriately and timely within the electronic medical record or data analytics programs.

Develops an initial and ongoing comprehensive assessment of client needs, including the impact of the terminal diagnosis on the patients physical. functional, psychological, and environmental needs and activities of daily living.
