To provide the CAMHS team with a high quality, evidence based systemic psychotherapy service to children, their families, carers and professional networks, in keeping with CYP-IAPT principles.

Clinical:

1. To provide a specialist systemic assessment of referrals of a significant and complex nature to formulate and implement plans for the treatment and/or management of a child’s or adolescent’s mental health problems, based upon an appropriate conceptual framework and employing methods based upon evidence. To use routine outcome measures as laid down by the Trust.

2. To assess children, adolescents and their families as part of a risk assessment.

3. To consult to parents/carers (including foster parents) as part of a treatment plan.

4. To work in ways which are sensitive to and appropriate for the needs of families from a wide range of racial, cultural and religious backgrounds. This includes abilities to understand and offer therapeutic interventions to a variety of family forms including single parents, same sex couples and their wider family networks, accommodated children and their network of care, children with learning difficulties, and children who are on the ‘Child Protection Register’, and parents with mental health difficulties. Competence is required in undertaking systemic therapy with families using an interpreter where English is not their first language.

5. To provide specialist systemic psychotherapy with clinical autonomy drawing on a range of models and wherever possible brief interventions. To practice in a way which is inclusive and considerate of the needs of each individual in the system including family members in their varying developmental stages and current emotional state.

6. To make highly skilled evaluations and decisions about treatment options considering highly complex factors concerning historical and developmental processes that have shaped the child, family or group, and their difficulties.

7. To be responsible for implementing a range of systemic psychotherapeutic interventions for children, adolescents, and their families, including couples and groups, drawing upon different explanatory models, and maintaining several provisional hypotheses in reaching a formulation and treatment plan. Interventions may include family psychotherapy, individual work, chairing professionals, and network meetings, liaising with other agencies, observations of young people in different settings, and the use of a range of systemic models.

8. To exercise full autonomous professional responsibility for the assessment, treatment, and discharge of clients, and formulate effective care plans in collaboration with the client and family. To undertake risk assessment and risk management relevant to individual patients, including protective and risk factors present in the network of significant relationships, and to do this using the knowledge of the multi professional context that is a mental health service for the locality.

9. To work jointly and collaboratively with other team members to enhance and develop work with families by providing specialist knowledge and skills.

Following additional funding into our CAMHS Liaison and Crisis Team, we are excited to be expanding our staff team.

The crisis service provides integrated, high quality assessment, support and interventions in and out of hours. We work closely with the Adolescent teams and borough based clinical teams across the three boroughs, as well as with tier 4 (inpatient services) which are provided by the South London Partnership. We have a vacancy for a band 6 Mental Health Liaison Nurse in the older adult’s mental health liaison team. We are based in the Queen Elizabeth hospital and provide support for patients in the emergency department and on the inpatient wards.

Overview of the Post

Key Task and Responsibilities
Clinical:
1. To provide specialist psychological assessments of clients referred to CAMHS based upon the appropriate use, interpretation and integration of complex data from a variety of sources including psychological and neuropsychological tests, self-report measures, rating scales, direct and indirect structured observations and semi-structured interviews with clients, family members and others involved in the client’s care.
2. To formulate and implement plans for the formal psychological treatment and/or management of a client’s mental health problems, based upon an appropriate conceptual framework of the client’s problems, and employing methods based upon evidence of efficacy, across the full range of care settings.
3. To be responsible for implementing a range of psychological interventions for individuals, carers, families, and groups, within and across teams employed individually and in synthesis, adjusting and refining psychological formulations drawing upon different explanatory models and maintaining a number of provisional hypotheses.
4. To evaluate and make decisions about treatment options; considering both theoretical and therapeutic models and highly complex factors concerning historical and developmental processes that have shaped the individual, family or group.
5. To provide specialist psychological advice, guidance and consultation to other professionals contributing directly to clients’ formulation, diagnosis, and treatment plan.
6. To contribute directly and indirectly to a psychologically based framework of understanding and care to the benefit of all clients of the service, across all settings and agencies.
7. To communicate in a skilled and sensitive manner, information concerning the assessment, formulation, and treatment plans of clients under their care and to monitor progress during the course of both uni and multi-disciplinary care.
Leadership
1. To receive regular clinical professional supervision from a senior clinical psychologist and, where appropriate, other senior professional colleagues.
2. To continue to gain wider post-qualification experience of practitioner (clinical / counselling) psychology over and above that provided within the principal service area where the post-holder is employed.
3. To develop skills in the area of professional post-graduate teaching, training and supervision and to provide supervision to other MDT staff’s psychological work, as appropriate.
4. To provide professional and clinical supervision of assistant/graduate psychologists and, as appropriate, to contribute to the supervision of trainee clinical psychologists.
5. To contribute to the pre- and post-qualification teaching of relevant healthcare staff, as appropriate.
6. To contribute to external and internal training programmes withinCAMHS
Management responsibilities
1. To contribute to the development, evaluation and monitoring of the team’s operational policies and services, through the deployment of professional skills in research, service evaluation and audit.
2. To advise both service and professional management on those aspects of the service where psychological and/or organisational matters need addressing.
3. To manage the workloads of assistant and graduate psychologists, within the framework of the team/service’s policies and procedures.

Research
1. To utilise theory, evidence-based literature and research to support evidence-based practice in individual work and work with other team members.
2. To undertake audit or research as appropriate within the service, Directorate, or multi-centred research across Trusts and to supervise research and audit projects.

4. To contribute to the evaluation, monitoring and development of the multi-disciplinary specialist CAMHS.
General

Highly Specialist OT Joint Emergency Team

The Joint Emergency Team aims to prevent inappropriate hospital admissions and facilitate early discharges from hospital by acting as the gateway to the Integrated Community Services within the borough of Greenwich. The team responds to referrals requiring intervention within 2 hours which includes the falls pick up service. The virtual ward is an integral part of this team and enables treatment and monitoring within the patients own home for a period of up to 14 days.

The team provides a multi-professional assessment of health and social care needs to support the immediate crisis, identifying specialist assessment and rehabilitation requirements. The team is able to monitor and provide intervention to support patients through an acute period of illness. The team is able to provide equipment necessary to maintain patients / clients at home.

JET is a seven day service that operates from 08:00 to 20:30, including bank holidays.

The Occupational Therapist will work as part of a multidisciplinary team, including health and social care colleagues to meet the needs of clients referred to the Joint emergency Team, ensuring the highest possible standards of practice.

We are looking to appoint a committed, highly motivated, enthusiastic and skilled Band 6 Specialist Occupational Therapist to our dynamic, integrated multi- disciplinary Rapid Response Team(RRT)/ Urgent Community Response Service (UCR).

RRT has been operational since 2013, since has gown and developed, RRT was one of the 7 accelerator sites involved in the introduction and successful roll out of the NHSE Urgent Community Response (UCR) initiative. RRT/UCR is a multi- professional team consisting of Nurses, PT's, OT's and Social Workers. The team aims to respond to crisis referrals within 2 hours and avoid unnecessary presentation to hospital and residential care.

The team provide holistic health and social care assessments for people over the age 18 years, who are registered with a Bexley GP and live in the Borough of Bexley. The team are based at QEH Woolwich and operates 7 days a week, on an 8am-8pm basis, providing urgent community response to vulnerable people in the community.

The post holder will support in the leadership of Rapid Response Team, deputising in the absence of the Band 7 lead Therapist/Nurses for the clinical management of the care of patients referred to the team. • Consultancy to other health and social care professionals regarding dementia.
• Deliver regular clinical 1:1 sessions for families affected by dementia who meet the referral criteria within the London borough of Greenwich.
• Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework for carers/families affected by dementia, including positive risk taking, managing distressed behaviours and risk minimisation strategies.
• Provide a range of bio-psychosocial interventions to promote health and wellbeing for people with dementia and their carers/families.
• Work collaboratively with other professionals to improve the identification and management of co morbidities and frailty for people with dementia and their carers/families.
• Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia.
• Provide care coordination for a clinical caseload of families with complex needs as identified within triage assessment in clinics or via home visit if required.
• Support the delivery of innovative dementia training and education to staff, to improve competence and confidence, in line with best practice and guidance.
• Develop own competence through use of the Admiral Nurse Competency Framework and portfolio development using resources provided by Dementia UK.
• Support the audit and evaluation of quality improvements and initiatives aimed at improving the delivery of dementia care within Oxleas, where appropriate to the role.

The opportunity has arisen for a 12-month Fixed Term for an experienced Looked After Children and Young People (LAC) Nurse to undertake the role of Named Nurse for Looked After Children’s Services in Greenwich.

While providing strategic, operational, and clinical leadership and professional advice on the delivery of the Looked after Children’s development plans, improving health outcomes for looked after children (LAC), the post holder will also provide line management for the Named Nurses for Looked After Children in Greenwich.

The post holder will work closely with the Associate Director, Service Manager, Operational Manager, Clinical Lead, external agencies, and key stakeholders, contributing to the planning of services to ensure all children, young people and families have access to a local, responsive and high-quality LAC service which is informed by evidence of good practice.

The post holder will work in collaboration with Children’s Social Care, Education, Health agencies, carers, keyworkers, young people and voluntary agencies, with particular reference to the delivery of the local LAC development plans.

Work closely with both the Designated Nurse within the Borough and the Designated/ Named Doctor for Looked After Children in order to promote effective partnership working and achievement of statutory requirements.
• Responsible for and to lead the provision of the Greenwich Looked After Children (LAC) at Oxleas NHS Trust
• Maintain continuity of service provision by staff recruitment, managing sickness and absence and appropriate skill mix and through forward planning formulate mitigation plans and actions

To undertake the role of SCPHN Community Practice Educator for students completing the SCPHN registration programme and for pre-registered students for the speciality they are to be competent in.

To support the service to adhere to the NMC standards for pre and post registration programmes. To support the development of and ensure the quality of Practice Supervisors.

To positively promote educational standards within Young Greenwich School Nursing through the provision of a consistent, person-centred approach to education, training and assessment.
