We have an exciting opportunity for a Band 6 Speech and Language Therapist to join our Special Schools & ARPs team to work across 3 provisions with both primary and secondary aged CYP, in the boroughs of Hammersmith & Fulham and Westminster. The post includes the opportunity to work with a neurodiverse caseload and with CYP with a variety of profiles including supporting young people with severe learning difficulties including profound and multiple learning difficulties.
The post provides the opportunity to implement a wide range of therapeutic approaches and interventions in collaboration with education partners. Through joint planning and delivery of interventions in the classroom and through support to families you will ensure that SLT input has a functional impact for the pupils you support.

Providing a specialist service to children attending special schools with Speech/Language and communication needs (SLCN) working closely with parents/carers, and staff to enable children to develop their speech, language and communication skills and to access the curriculum
To assess the needs for, develop and implement specialist Speech and Language Therapy treatment to children referred to the SLT Education service (ARP/special schools).
To provide advice to others regarding the management and care of children with communication difficulties
To be accountable for own professional actions and recognise own professional boundaries, seeking advice from senior colleagues as appropriate
The post holder will also provide some day-to-day supervision and peer review support to less experienced colleagues and students, including second opinions when necessary.
The post holder will run appropriate in-service training independently for school and other staff.
The post holder will have well developed specialist skills in a specialist area of SLCN within the broad range of speech, language and communication needs encountered within the caseload of school aged children
The post holder will liaise with other colleagues across a range of statutory and non-statutory agencies.

B7 Paediatric Speech and Language Therapist

Chelsea and Westminster Hospital / West Middlesex Hospital & St Mary's Hospital

A new B7 post has arisen for an experienced paediatric speech and language therapist with skills in dysphagia to work in the acute service. This is a new post developed to increase VFSS capacity working closely with SLT colleagues, including at St Mary's Hospital, to increase access to existing VF services for North West London.

This role is suited to someone with an interest in gaining or consolidating skills in diagnostic interventions including VF and FEES. The post supports both hospitals to expand their capacity for VFSS and you will be required to run clinics at either site.You will be supported in the role and be given training to ensure you achieve RCSLT VF competencies. This post requires someone with a collaborative and flexible approach to working across organisations.

The postholder will also hold an inpatient and outpatient caseload. The service covers the paediatric wards at Chelsea and West Middlesex Hospital sites, including the SCBU and level 3 gastro-surgical NICU. A small outpatient service offers expert clinical advice for complex referrals and dysphagia management for infants under 6 months.

You will work closely with the MDT to deliver expert care to infants, children and their families. The team benefits from excellent working relationships with the MDT including ENT, gastro and respiratory consultants.

• To work as an autonomous practitioner to provide dysphagia assessment and intervention to a varied paediatric caseload, including infants on the neonatal unit, paediatric wards and outpatient service.



• To work with infants and children who may have long-term complex conditions, to provide specialist assessment and management plans.



• To deliver video fluoroscopy assessments for both inpatients and outpatients with dysphagia to offer expert diagnosis and management plans depending on results.



• To engage in regular supervision with the clinical lead.



• To support with clinical problem solving and provide managerial expertise for colleagues both within the Trust and within North West London.



• To participate in CPD and other developmental activities and encourage all members of your team to do likewise.



• To educate the multi-disciplinary team in the trust on elements of SLT in your designated clinical area, and raise the profile of the SLT service.



• To work closely with the other members of the multi-disciplinary team and demonstrate excellent communication skills.

B7 Physiotherapist (Medical Rehabilitation)

West Middlesex University Hospital

We are looking for an enthusiastic Physiotherapist to provide specialist skills and knowledge to support with the management of the medical rehabilitation, care of the elderly and frailty patient cohort. This role would run alongside the Team Lead Physiotherapist, to support with ensuring the department is providing an efficient and effective service as well as some day to day operational management. The post carries a clinical caseload.

The post holder will have good acute and community experience in a variety of acute medical presentations and pathologies, and patients present with frailty syndrome. They will work as part of the multidisciplinary team to assess and discharge patients in a timely and appropriate manner with involvement and referrals to our community services.

Clinical Responsibilities

· To offer parents of all eligible babies the Newborn Hearing Screen whilst ensuring maximum number of inpatient babies receive a hearing screen by working flexibly where necessary.

We are looking for an Ophthalmic Nurse Practitioner who has experience in intravitreal injections, POA/Biometry and pre/postoperative review of patients who have undergone cataract surgery.

An opportunity has arisen to join the Planned Care Surgery Operational team as an Assistant Service Manager in Chelsea and Westminster Hospital supporting Ophthalmology, Pain and Plastic Surgery services.

The Assistant Service Manager will work closely with nursing, administrative and clinical teams to deliver excellent performance and achieve agreed productivity targets. The Assistant Service Manager will ensure that line management their staff is undertaken professionally and that each member of staff has an appraisal and development and training plan consistent with Directorate, Division and Trust objectives

Operational Management
• Provide hands-on operational management to designated areas of the Division, ensuring the delivery of services in line with expected service standards.
• Be accountable through the General Manager, Deputy General Manager and Service Manager for the monitoring and delivery of key quality, productivity and performance targets.
• Promote excellent customer service throughout all areas of responsibility, taking remedial action to correct poor and unresponsive service
• Assist the Service Manager in ensuring that clinic templates, procedure schedules and rotas deliver care at times that meet service capacity and patient needs.
• On behalf of the Service Manager, work closely with the Service Directors and Clinical Directors to assure compliant working patterns for junior doctors, ensuring adequate medical staffing cover is in place to meet the clinical and operational needs of the service. Assist with the booking of additional medical staff as needed and maintain the relevant staffing spread sheets.
• Support the service manager with the investigation and co-ordination of complaints that are predominantly administrative/ medical related within the directorate working closely with medical staff and with the Patient Advisors. Ensure corrective action is taken in response to patient feedback
• Be responsible for effective communication systems within areas of management responsibility including regular team and departmental briefing.
• Facilitate partnership and integration between services across multiple sites e.g. in relation to aligning systems and processes, sharing best practice, improving data etc.
• Ensure effective clinic utilisation and capacity planning, identifying where there are inefficiencies or a lack of capacity and taking steps to address where productivity and access fall below targets
• Play a key role in ensuring effective systems of capturing high quality data and that all datasets are submitted to a high level of accuracy in line with required timescales.
• Identify service improvement or modernisation initiatives that will enhance the patient experience, improve the quality of care provided or enable improvements in performance; work with relevant staff and stakeholders in order to make recommendations in relation to the size and scope of the project.
• Manage small service changes or assist in wider change programmes as required, ensuring there is thorough planning, scoping, stakeholder engagement, patient communication and resource planning
• Proactively take steps to ensure compliance with internal and external governance and best practice requirements
• Identify and manage audits and service improvement projects in collaboration and with support of GMs, DGMs, SMs, Nursing and Medical teams
• Promote excellent customer service and patient experience throughout all areas of responsibility, taking remedial action to correct poor and unresponsive service.
• With support of the service manager, participate in business planning and ensure that there is enough capacity within the service to match the patient demand and needs.
• Participate in the production, implementation and revision of operational policies throughout the Directorate, instigating the changes to or implementation of policies and procedures

Staff Management
• Support Service Managers and Operational Admin Support B4 in emergency rotas, co-ordination of annual leave and study leave for clinicians, ensuring cover is always maintained and agency expenditure is minimised
• Support Operational Admin Support B4 in delivering their objectives
• Ensure close working relationships are maintained with the clinical professionals on all matter relating to patient care within the service

Financial Management
• Play an active role in identifying areas within the service where expenditure can be reduced and/or income generated (CIP)
• Be involved under the Service Manager’s/ DGMs/GM instruction in the business planning process, producing capital and revenue business cases and proposals for service development.
• Undertake detailed information analysis of specific projects/reports requiring high levels of analysis.
• Contribute to the management of performance information, taking a lead for specific projects

General

Advanced Clinical Practitioner - Orthopaedics, Band 8a

We are looking for qualified Advanced Clinical Practitioners to join our Orthopaedics team in Planned Care.

We are very excited to be expanding our Advanced Clinical Practitioner workforce in our high achieving and friendly Trauma and Orthopaedics team. We are looking for a qualified ACP, who is looking for a new challenge and will be focused on the care of elective orthopaedics patients, both in the inpatient and outpatient settings.

With support of the senior medical team, the ACP is accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. In addition to this the ACP is also responsible for appropriately referring patients to relevant specialties for any necessary inpatient or outpatient investigations as well as facilitating safe patient discharge.



The post holder will have a Trust-wide responsibility to promote clinical excellence in the care of patients presenting to their area by providing clinical advice and support to nursing staff and other health care professionals. They will provide expert professional and independent clinical care which enables the coordination of a multi professional seamless service for patients.



The role will include developing and supporting audit and research projects and participating in up-dating and implementing departmental policies, protocols and guidelines, in line with national guidance and contemporary evidence.



The ACP will have protected time to maintain and further develop their professional competence ensuring they are working on all 4 pillars. They will have annual appraisal/PDR from medical and nursing/AHP supervisors. The ACP will also provide ongoing supervision and support, participating in the induction and training of nursing and medical staff. As a senior member of the team, the ACP will also play a pivotal role in the operational development of their specialty. They will initiate, manage and drive change within the department, innovating changes in practice for the benefit of patient care in line with current trust and local programs.



The ACP will undertake the role in accordance with Trust and Departmental guidelines but have freedom to act within broad policies and protocols, in accordance with professional responsibilities and boundaries.

Clinical Provision & Expert Practice


• Uses highly developed clinical knowledge to independently assess, diagnose, plan, implement and evaluate treatments and interventions for patients presenting to the specialty with complex undifferentiated or undefined presentations.
• Undertakes physical examination as indicated by the patient’s condition; autonomously analysing complex clinical signs and investigation results to consider differential diagnoses and diagnose conditions, initiating treatment as required.
• Performs clinical procedures appropriate to the history and physical examination of the patient, including further invasive testing and treatments requiring highly developed skills and precision, including (but not limited to) catheterisation, cannulation, peripheral arterial blood gas sampling.
• Requests investigations such as blood, urine and other laboratory tests, electrocardiographs (ECGs), ultrasound scans, X-Rays and computed tomography (CT) scans in accordance with IR(ME)R regulations and as local policy allows.
• Analyses multiple sources of data including patient history, physical examination and investigation findings when making diagnoses, clinical judgements and evaluating care provided; presenting this information, to speciality /senior medical staff for advice when scope of practice is exceeded.
• Autonomously formulates appropriate management plans for patients, formulates clinical decisions and often complex treatment plans to manage acute illness and acute episodes of chronic illness including referral, admission or discharge.
• Continuously revaluates findings and clinical response to treatment and establishes an appropriate ongoing management plan accordingly.
• Prioritises health problems and intervenes appropriately in complex, urgent and emergency situations, including initiation and leadership of resuscitation.
• Manages clinical events involving patients, often requiring unpredictable and high levels of mental and/or physical effort according to the patient’s dependency and clinical need.
• Provides clinical cover to the specialty wards according to patient and staffing needs.
• Prescribes and reviews medication (as an independent prescriber) for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence-based practice and national and local protocols and within the role’s scope of practice and legal framework.
• Integrates both pharmacological and non-pharmacological treatment in patient care/management plans

• Works in partnership with the existing senior medical, nursing and AHP teams in driving departmental development and change.
