We have an exciting opportunity for a motivated speech and language therapist to join our Community SLT (Speech and Language Therapy) service within the Ealing borough of London, as a Band 6. We are looking for an enthusiastic SLT with experience of working with adults with neurological conditions. You will join our dynamic & friendly team working with a Band 8a SLT, other B7 SLT's,B6 & B5 SLT's and SLT Assistant's to assess, manage and treat patients in the borough of Ealing who have either swallowing or communication difficulties.
This post offers you the opportunity to develop your leadership and supervision skills. You must have experience in assessment and management of swallowing and communication disorders in adults with acquired neurological conditions including Stroke, Head Injury and progressive neurological conditions; frailty & general medical conditions. This must include highly specialist skills in dysphagia management with relevant post graduate qualifications.

• To provide specialist SLT skills to adults with a range of neurological and general medical conditions based within the Community SLT service
• To assess and treat own caseload of patients with a range a speech, language, communication, and swallowing needs.
• To hold a community and outpatient caseload
• Supervise and teach more junior members of the team including, qualified SLT’s, students and rehab assistants as well as being involved in teaching of senior SLT’s and other multidisciplinary team (MDT) staff.
• Lead on and comply with professional and departmental standards of practice within own clinical area.
• Present evidence-based projects and research including departmental audit and service developments, linking with the Trust’s Quality Improvement team.
• To develop and establish strong communication links and liaison with speech and Language therapy teams in the trust and external organisations as appropriate to the needs of people with communication and swallowing difficulties.

· The post holder will assess, differentially diagnose, set goals with and treat and manage clients with communication or swallowing disorders.
· The post holder will manage and prioritise their caseload independently.
· The post holder will provide comprehensive assessment and treatment and ensure a seamless pathway from inpatient to outpatient status.
· To work closely with all members of the multi-disciplinary team.
· To actively contribute to service development within the areas of stroke, long term neurological conditions & older people services
· To deliver training/teaching to carers, other health professionals and students
· The post holder will assist in the development and provision of Specialist Speech & Language Therapy as an autonomous SLT working within the Adult SLT service.
· The service will be provided at community outpatient clinics, within clients own homes, residential homes and nursing homes.
· The post holder will demonstrate efficacy of this service to stakeholders with appropriate liaison and relevant audit.
· The post holder will access support and supervision from SLTs, and other members of the MDT teams.
