The post holder will be based within the Virtual ward Team , and working alongside the Urgent Community Response Team clinically, when required. The post holder will have clinical responsibility of the Virtual ward.

The post holder will be able to provide clinical expertise in Triage, and assessments for Virtual Ward and across the Adult Community Services Division.

The Post holder will be expected to participate in working with secondary care, to identify and assess patients suitable for community services, and to prevent avoidable hospital admissions and facilitate early supportive discharge. The work will be undertaken collaboratively with Secondary and Primary Care, and Community Services, including Urgent Care Response, Early Intervention Community Teams (EICT), Urgent Care Bureau and other services deemed in scope.

The post holder will also attend multidisciplinary team meetings (MDT’s) to provide advice and support to the Virtual Ward (Hospital @Home) Model.

This is an exciting opportunity to join the East Locality in the role of Clinic Nurse. The successful candidate will work as a Clinic nurse as part of the community nursing teams supporting non housebound patients that require clinical interventions that are deemed complex and therefore do not meet practice nurse criteria. You will need to be able to travel to the clinic sites. You will provide a holistic approach to individuals, that includes, assessment, planning and implementation of care along with review. The successful candidate will act as an autonomous practitioner providing a comprehensive clinical assessment service to patients resulting in a safe and appropriate clinical decision.

The successful candidate should have experience of wound care, with a particular interest in leg ulcer assessment, treatment and follow up care including compression therapy and lymphoedema care as well as catherisations. Prescribing or a willingness to complete nurse prescribing essential. In identifying appropriate patients for clinics you will also support with triaging of referrals.

The post holder will work as part of the integrated Multidisciplinary team under the supervision of the Clinical Team Manager. He / She will be accountable for the delivery of high quality care to patients within health centers. Working with other members of the IMT to provide care to and promote health/well being and independence for patients on the team caseload, working closely with other partners/stakeholders including GP’s, Tissue Viability and vascular clinics. You will support IMT triaging to ensure patients receive the right care at the right time in the right place.

The Band 5 Education Mental Health Practitioner will work with colleagues in the Cheshire East MHST, working closely with education settings and professionals supporting children, young people and their families.

1. When required type Child Protection Reports for the Consultant and medical team, using digital dictation, forwarding requests for investigations, (e.g. EEG, X-Rays, blood tests), filing results and correspondence.

To oversee production and collect data on CP Reports sent to to Social Care Services/Police Statements on children seen for Child Protection Medicals,liaising with solicitors and Guardian ad Litem, as requested. Some letters or reports may be of a sensitive and sometimes upsetting nature due to their content
2. To deal with sensitive and sometimes complex enquiries fromSocial Workers and The Trust Safeguarding Team to exercise tact and give suitable non clinical support in the absence of theNamed Doctor for Safeguarding Children and Young People.
