The Clinical Coding Department is a pivotal Service within the Trust’s IM&T Division. These are corporate functions responsible for all Trust management information, reporting both externally and internally, providing vital management information to clinicians and managers which inform and determine the basis for decision-making across the Trust.

The Clinical Coding Department is responsible for the collection of clinical diagnosis codes using the ICD-10 (International Classification of Diseases) and clinical procedure codes using OPCS-4 (Classification of Interventions and Procedures), extracting the relevant information from patient case notes, discharge letters and reports.

Clinical Coding data is essential for the effective management of the Trust and directly and crucially influences the Trust’s income in line with the ‘Payment by Results’ (PBR) process. It also forms the basis for clinical audit, clinical governance reporting, bench-marking and medical research.

The department provides the full spectrum of elective and emergency surgery, including endovascular aneurysm repair, urgent carotid endarterectomy, distal bypass and hybrid open/endovascular procedures for peripheral arterial disease, and endo-venous treatment for superficial venous disease.

The on-call surgeon is freed of all elective commitments; instead, assuming the responsibilities for the care of all vascular in-patients including elective postop and new vascular emergencies. The on-call surgeon is required to do daily ward rounds including the weekends and provide specialist input to the treatment of patients admitted in the care of other specialties . Diagnostic (CT) angiograms are available 24/7 for aortic and peripheral emergencies and vascular laboratory services within the normal working time, five days a week. The junior vascular team comprises of two middle grades, one core trainee, one FY2 and 3 FY1 doctors. The general surgical on-call team provides cover for vascular patients out of hours.

The hospital has approximately 400 bed, serving a population of approximately 320,000 across Bedfordshire and surrounding areas (with a 900,000 catchment for vascular services). Its core local authority populations are Bedford Borough (170,000) and Central Bedfordshire (300,000). The hospital has over 2500 members of staff (making it the largest local employer in Bedford). The hospital enjoys strong community support with over 200 volunteers at the hospital and over £6 million being raised by the independent Bedford Hospitals Charity over the past 10 years. The post holder will be leading and working within a multi-disciplinary team planning and providing high quality care for patients with acute illness or injury within the Emergency Department.

Participate or lead on the shift handover/ safety huddle.

Supporting the band 7 in completion of the daily safety checks and ensuring all audits are completed as required.

Supporting the Band 7 in managing a small team of staff.

Supporting the band 7 shift manager leading teams to meet time critical RCEM standards.

To participate in non-clinical management processes i.e. Performance Review, sickness management recruitment and selection and Appraisal of staff both personally and of staff at Band 5 and below. To undergo, where necessary, further training in relation to the Extended Role of the Nurse and participate in the training of others, ensuring that Night staff have equal opportunity to participate in statutory training.

Be available, in the absence of the Nurse Team Managers, to deputise and take responsibility for the co-ordination of 24 hour nursing care of each patient in the department.

Support the Manager in staff rostering, ensuring adequate skill mix.

In conjunction with the Manager closely monitor bank and agency usage.

Participate in the selection of staff at Band 5 and below in conjunction with the Nurse Team Leaders and the Matron

To act up for the nurse Team Manager in their absence, ensuring senior cover within the Emergency Department at all times.

To provide support and advice to colleagues in distressing circumstances such as imparting news of terminal illness and bereavement.

Be responsible for the assessment of nursing needs and plan and implement care required by patients, in conjunction with nursing observations & patient assessment, medical requirement and with the co-operation and partnership of the patient and their relatives.

Carry out treatments such as dressings, wound closure, personal care such as washing and toileting, transfers of patients in wheelchairs, on trolleys or in beds in and around the department and hospital and assist patients with mobility as part of daily duties.

Take responsibility for ensuring the co-ordination of discharge planning of patients in conjunction with the multidisciplinary team.

Remain calm, professional and non-judgemental when faced with violent or aggressive patients and relatives, those under the influence of alcohol or drugs, Patients suffering from Mental Health Issues and those with poor personal hygiene and /or infestations on a daily basis.

To risk assess all patients and carry out manual handling as per Trust Policy.

Be able to deal with frequent exposure to unpleasant sights such as major wounds or bleeding, emaciated patients and bodily fluids.

Be able to plan own workload in a busy, noisy environment where there are frequent interruptions and unpredictable changes in demand.

To ensure communication with paramedical services to enable them to carry out their part in patient care and to create a climate in which feedback on patient progress is the norm.

To ensure that staff are aware of current changes both local and national and involve them in consultations regarding changes by contributing in regular staff meetings.

Maintain clinical competence in the use of medical devices used within the department and clinical procedures such as administration of injections.

Ensure that the National Service Framework for Paediatric Services is adhered to and that Child Protection Policies are followed.

Responsible for ensuring that all accidents/incidents are reported to the appropriate Manager and reported via the inphase reporting system. All employees are required & expected to participate in the Trust’s accident/incident reporting system and to comply with the Trust’s procedures and techniques for managing risks.

Maintain and share up to date knowledge of current research relevant to emergency care.

Responsible for regular cleaning of equipment, reporting faults to shift manager and removing defective equipment from use. This includes participation in daily checking and cleaning or equipment/ drugs as per department huddle book.

Take responsibility for the safe storage of patients valuable and property whilst in the Emergency Department in line with trust policy.

Teaching and Learning

It is mandated that all registered healthcare professionals – nursing included, are compliant with new SSSA standards for pre registrants as practice supervisors as a bare minimum moving on to also becoming practice assessors after 12 months in the department.

To support education, training and development of self and others in the emergency department and actively encourage to share learning and best practices in Emergency Medicine.

Assist, in conjunction with the Emergency Department Matron, the Practice Development Nurse and Heads of Nursing for Emergency and Acute Care directorate, to continue to develop and set standards of care, taking into account relevant published National Guidelines and research.

To undergo, where necessary, further training in relation to the Extended Role of the Nurse and participate in the training of others.

Major Incident

To maintain an up to date knowledge of the trust Major Incident Policy and Department and Role specific plan

In the event of a major incident being declared and a call inn of staff being undertaken you should report to the Nurse in charge of ED as soon as possible.

Undertake training and remain competent in the use of decontamination equipment for Chemical, Biological, Radiological and Nuclear (CBRN) contaminated patients.

To undertake the one day HMIMMS regarding Major Incident Management so there is an understanding of expectations of band 5 role in a major incident

Undertake the Decontamination of patients from a CBRN incident as required.

Undertake training and maintain competence in scene safety and the use of Personal Protective Equipment used in the pre-hospital setting.

Specialty Doctor - 10 Basic Programmed Activities per week

The Specialty Doctor will be required to provide a middle grade level of staff for the Department.

The Specialty Doctor will attend out-patient clinics, both general and specialist, with his/her supervising consultants.

Dependant on the experience of the Specialty Doctor, he/she may be offered an independent theatre list.

There is a huge demand for surgical dermatology and experience and interest in this would be desirable although appropriate training will be given.

Assess and manage where appropriate patients with maxillofacial trauma who may attend the accident and emergency department at Lister Hospital & QE11 during normal working hours in the weekdays.

There will be administrative duties and other ad hoc attendances at clinics, theatre, etc. with other consultants as required in the absence of the regular junior members of staff.

The Specialty Doctor will work under the supervision of and be responsible to his/her respective Consultant, or the on-call Consultant and, ultimately, the Clinical Director, at Luton and Dunstable Hospital, or his successor.

The post will be based at Bedford Hospital. The appointee will be required to travel to the in-patient centre (Luton and Dunstable Hospital NHS Foundation Trust) when necessary.

In case of difficulties of any kind, the Specialty Doctor will seek the advice of the appropriate consultant. The Specialty Doctor is not expected to participate in any work beyond his/her level of expertise.

The timetable for the post is attached at Appendix A. This will change from time to time to respond to the demands of the service. The time-table will be reviewed by the

The successful candidate will participate fully in the regular Department Teaching and Audit meetings.

HOURS OF DUTY

a) 10 sessions per week. Audit and CME sessions will be undertaken on site utilising the facilities within the Department or in the Postgraduate Medical Centre. The post-holder is required to attend the audit and teaching sessions at Luton and Dunstable Hospital on alternate Friday afternoons. Please see Appendix A for the provisional job plan.

b) So far as is consistent with the proper discharge of the above duties, the appointee will undertake to deputise from time to time for absent colleagues.

c) The appointee will undertake, exceptionally, to perform additional duties in occasional emergencies and unforeseen circumstances.

d) The appointee will undertake, exceptionally, to be available for such irregular commitments, outside normally rostered duties as are essential for patient care.

EMERGENCY COVER

Patients with maxillofacial emergencies during out-of- hours including weekends will attend Luton and Dunstable NHS Trust.

The post involves a 1:8 second on-call weekend rota.

STUDY AND TRAINING

The Oral and Maxillofacial Department is recognised for postgraduate training by the Faculty of Dental Surgery.

Facilities for study leave may be available with the consent of the supervising Consultant, Clinical Director and the Director of Medical Education (Associate Medical Director).

The allocation is 30 days in a three year period as set out in the Terms and Conditions of Service for the grade. The budget for study leave is managed by the Director of Medical Education. Currently this is £3000 grant per 3 year period.

You will be required to register with the Faculty for CME training.

Although this is not formally a training grade, the Specialty Doctor is expected to join and actively participate all aspects of education and training within the Oral and Maxillofacial Surgery Department.

QUALIFICATIONS/EXPERIENCE REQUIRED FOR APPOINTMENT

The successful candidate should have completed at least four years, full-time postgraduate training, at least two of which should be in a specialty training programme in oral and maxillofacial surgery and be competent in dento-alveolar surgery and management of maxillofacial trauma.

A Membership or Fellowship in Dental Surgery is desirable. (FDS, MFDS, MJDF)
