The Tobacco Dependency Specialist Advisor will operationally deliver the smoking element of the NHS LTP by providing a stop smoking service for inpatients admitted to Greenparks House (Bromley), Oxleas House (Greenwich) or Woodlands and Barefoot Lodge (Bexley) based upon the Ottawa and CURE Models of Smoking Cessation. These innovative roles will provide front-facing, visible and accessible support by offering daily face-to-face onsite clinics to service users and staff in each of the mental health inpatient wards in the Acute and Crisis Directorate.

The post holder will be responsible for the following:
• Respond to all inpatient admissions ofsmokers

Clinical
• To deliver robust stop smoking interventions for inpatients, outpatients, visitors, and hospital staff in line with NICEguidance.
• To be responsible for the implementation of the NHS LTP across several wards in the hospitals.
• Deliver evidence-based stop smoking advice to people wishing to stop smoking, in accordance with local and national protocols, in groups or with one to one individual support.
• To help staff to identify partners and family members of patients and engage them in treatment.

JOB PURPOSE & KEY RESPONSIBILITIES:
• To provide specialist substance misuse care to offenders and to work as part of an integrated and multi-disciplinary health service, which may include in-reach services, reception duties, resource centre services, dual diagnosis management and primary care nursing.
• To deliver a range of specialist interventions at primary and secondary care level; to include:
o Referral management, screening assessment, triage and evidence- based interventions
o One-to-one and group-work facilitation
o Assertive outreach interventions
o Managing a mixed and challenging caseload
• To contribute to alternatives to the need for inpatient admission and where necessary assisting with early discharge from in-patient services through the implementation of substance misuse management clinical interventions and complex case management arrangements resulting in continuity of care for an offender on ordinary location.
• To perform robust assessment, screening and interventions to offenders with substance misuse concerns
• Ensure that all waiting time, assessment and interventions are delivered in a timely manner, and positively contribute to the achievement of the service’s performance targets
• Ensure integrated and collaborative working with other specialist services, including continuity of care and through the gate arrangements and liaison
• Work closely with community mental health teams (CMHTs) to ensure appropriate sharing of information, continuity of care and the Care Programme Approach (CPA) for all offenders as necessary.
• To provide comprehensive and timely reports and mental health expertise to multi agency public protection panels (MAPPA), police, probation services, solicitors and court liaison teams as directed.
• Provide supervision to junior staff and to receive supervision as directed (line management and clinical).
• To participate in resource centre services as directed, delivering group therapy and specialist activities under the direction of the team leader.
• To work in a psychologically minded way with offenders in achieving their agreed goals and quality health outcomes.
• No direct budgetary responsibility but must be aware of the financial impact of decision-making.
• Close working relationships with all prison and Serco staff, to include participation in sentence planning, resettlement, safer custody and ACCT processes.
• To develop and take on a specialist lead role as appropriate and provide peer support and training in this area.
• To contribute towards support, advice, consultation and training for other prison departments.
• The post holder will be part of skilled multidisciplinary teams comprising all healthcare staff, including strategic partners, and prison managing a single referral pathway.
• The post holder will be required to undertake training to maximise uptake of screening, health promotion and prevention activity, including vaccinations.

Oxleas NHS Foundation Trust: Bracton Campus—Pharmacy Department

Oxleas NHS Foundation Trust provides a wide range of mental health and community health services. These include acute adult and elderly mental health units and community teams based at a number of locations in Bexley, Bromley and Greenwich boroughs, and intermediate and elderly care units based at Queen Mary’s Hospital (QMH), Sidcup, and Eltham community hospital.

The Trust has dispensaries located at QMH and the Bracton campus, Dartford; both are registered with the GPhC and the QMH pharmacy store holds a wholesale dealers licence. Within Oxleas, clinical pharmacists are an integral part of the multidisciplinary team, with regular ward attendance and provision of a medicines information service which includes a patient telephone advice line.

Pharmacy also provide support to patients within the community, including to Community Mental Health teams, a specialist perinatal clinic and a home assessment Medicines Optimisation service to patients within Bromley.

Key tasks and responsibilities
• To provide clinical pharmacy services to mental health, intermediate care and elderly care inpatient units, to ensure patients receive appropriate medication on admission, have their medication and adherence reviewed during their stay, and have a clear medicines management plan on discharge.
• To provide information about the use of medicines to consultants, other prescribers, other health care professionals, patients, relatives and carers.
• To support professional leads in identifying and addressing issues around the use of medicines that are relevant to meeting CQC standards, for example advising on the safe and secure storage of medicines, and contributing to the audit and review of controlled drug processes.
• To work with the Medication Safety Officer to implement medicines safety initiatives within designated wards.
• To record information as required on clinical pharmacy activity.
• To take professional responsibility when clinically screening prescriptions to ensure the accuracy, legality, safety and efficacy of prescribed medication, including liaising with doctors and nurses to resolve any queries arising from the screening process.
• To seek advice from senior pharmacists where necessary.
• To participate in the dispensary rota as needed, working independently to liaise with doctors and nurses to resolve any queries arising from the screening process and to undertake the final accuracy check on prescriptions dispensed by other pharmacy staff.
• To participate in the medicines information rota, providing clinically sound, evaluated information on a wide variety of complex enquiries to service users and all grades of health care professionals.
• To participate in other internal rotas as required.
• To undertake any other relevant duties, as may be required by the Chief Pharmacist.

JOB PURPOSE & KEY RESPONSIBILITIES:
• To provide specialist substance misuse care to offenders and to work as part of an integrated and multi-disciplinary health service, which may include in-reach services, reception duties, resource centre services, dual diagnosis management and primary care nursing.
• To deliver a range of specialist interventions at primary and secondary care level; to include:
o Referral management, screening assessment, triage and evidence- based interventions
o One-to-one and group-work facilitation
o Assertive outreach interventions
o Managing a mixed and challenging caseload
• To contribute to alternatives to the need for inpatient admission and where necessary assisting with early discharge from in-patient services through the implementation of substance misuse management clinical interventions and complex case management arrangements resulting in continuity of care for an offender on ordinary location.
• To perform robust assessment, screening and interventions to offenders with substance misuse concerns
• Ensure that all waiting time, assessment and interventions are delivered in a timely manner, and positively contribute to the achievement of the service’s performance targets
• Ensure integrated and collaborative working with other specialist services, including continuity of care and through the gate arrangements and liaison
• Work closely with community mental health teams (CMHTs) to ensure appropriate sharing of information, continuity of care and the Care Programme Approach (CPA) for all offenders as necessary.
• To provide comprehensive and timely reports and mental health expertise to multi agency public protection panels (MAPPA), police, probation services, solicitors and court liaison teams as directed.
• Provide supervision to junior staff and to receive supervision as directed (line management and clinical).
• To participate in resource centre services as directed, delivering group therapy and specialist activities under the direction of the team leader.
• To work in a psychologically minded way with offenders in achieving their agreed goals and quality health outcomes.
• No direct budgetary responsibility but must be aware of the financial impact of decision-making.
• Close working relationships with all prison and Serco staff, to include participation in sentence planning, resettlement, safer custody and ACCT processes.
• To develop and take on a specialist lead role as appropriate and provide peer support and training in this area.
• To contribute towards support, advice, consultation and training for other prison departments.
• The post holder will be part of skilled multidisciplinary teams comprising all healthcare staff, including strategic partners, and prison managing a single referral pathway.

The ICT Department is seeking a friendly and enthusiastic Helpdesk Technician to join the ICT team. The post is part of the first line technical support team providing remote support to users across the Trust.

The post holder will take and respond to support calls logged by phone, or electronically by end users. We are seeking an experienced Consultant Psychiatrist who will work on Norman ward in Green Parks House, Princess Royal University Hospital, Oxleas NHS Foundation Trust.

He/she will provide senior medical cover for18 acute inpatients admitted to Norman ward. The post-holder will be the Responsible Clinician (RC) for all detained inpatients.

The post-holder along with his/her designated 2 wte Core Trainees will be responsible for all clinical work and related administrative work including mental health act work and along with other team members ensure that patients have good quality care and that the Trust’s quality indicators are not breached. It is expected that the post holder will work very closely with the ward manager to deliver high quality and evidence based clinical care to inpatients and take a lead on clinical governance including the delivery of key performance targets, learning from incidents and service user feedback.

There are three general adult wards based at Green Parks House, Princess Royal University Hospital. These wards, Norman, Betts and Goddington admit both informal and detained patients. Norman ward has 18 beds and all wards are mixed sex wards.

The inpatient wards at Green Parks House successfully completed the College Research Unit’s accreditation programme for inpatient units in 2008. The focus of all the acute wards is on multi-disciplinary team assessment and treatment of mental illness through a range of treatments and collaborative care.

Treatment is achieved with high levels of observation and a range of treatment strategies, including pharmacological and psychological approaches, within a safe and secure environment.

RC chairs and leads a multidisciplinary review meeting of patients on the ward at least three times a week to ensure that there are enough opportunities to see each new patient at the earliest opportunity and that all patients are seen at least once a week.

We would like to recruit an enthusiastic and experienced Mental Health Nurse to our Intensive Case Management in Psychosis Team (ICMP) based at Carlton Parade in Orpington.

The team is part of the Bromley East Locality Mental Health Service and provides integrated mental health support for people experiencing psychosis or bipolar disorder. We have an exciting opportunity for 2 Senior Psychiatric Liaison Band 7 Nurses to join our Integrated Mental Health Liaison Team (WAA&OA) at the Princess Royal University Hospital in the Royal Borough of Bromley.

This post is suitable for a Registered RMN with a minimum of 1 year's recent post-registration experience in liaison mental health nursing as a B6 or B7, as well as experience of leadership and supervisory roles. Overall the person must have 3-yrs mental health experience in relevant clinical settings with transferable skills to this role.

The Liaison team is based at the Princess Royal University Hospital in the Royal Borough of Bromley, South East London.

The Liaison team is well established with a staff contingent of a number of experienced B6 nurses and medical staff. Due to the demands of the services, the clinical lead role is being established in the team.

A large proportion of the team’s work is based in the Emergency Department, providing frontline assessments, but the team also provides a service to all the wards and departments of the acute hospital, for patients presenting with a wide range of medical and mental health co-morbidities. The team is a very dynamic one, operating 24 hours a day, 7 days a week.

The role requires you to triage, screen and conduct rapid assessments and treatment of WAA and OA patients in the Emergency Department. You will possess outstanding clinical skills, including independent assessments, and a comprehensive knowledge of mental illness and brief therapeutic interventions. You will have a thorough understanding of clinical risk and will conduct thorough risk assessments resulting in a safe management plan. You will be competent in the completion and interpretation of mental state examinations and will be able to devise accurate formulations and management plans based on the findings of your assessment. You will be creative and innovative in your approach, and will work collaboratively with patients and their carers in the formulation of care plans. You will be able to effectively liaise with other agencies, in particular your acute trust colleagues. You will be keen to professionally develop and must possess the ability to practice safely and effectively within the relative autonomy that this role offers.

The Mental Health Liaison Team operates a 24 hour service and is it essential that all applicants are able to work shifts and unsocial hours as required.

As a member of the Integrated Mental Health Liaison Team (MHLT) you will:
• Work alongside senior team colleagues to provide clinical leadership to the team.
• Work alongside other senior colleagues to operate a Rapid Assessment and Treatment (RAT) function for patients referred from ED for specialist mental health assessment utilising the SBAR model of rapid assessment and care planning, and a modified version of the UK Mental Health Triage Scale.
• Support and develop the clinical skills of junior team members, particularly in relation to the safe and appropriate use of positive risk taking.
• Support junior team members to prioritise referrals and clinical duties effectively, in order to promote safe and effective practice and clinical responsiveness.
• Establish and maintain effective working relationships with acute trust staff.
• Maintain high standards of oral and written communication with team / trust colleagues, acute trust staff, patients and their significant others at all times.
• Provide a specialist consultation-liaison service to referring clinicians and clinical teams, assisting them in decision making in difficult and complex cases.

• Be responsible for specific clinical projects and undertake all necessary work to complete these, including facilitating meetings, liaising and/or negotiating with other staff.

• The post-holder will act as a role model in providing a service for people with mental health problems for all staff within general hospital setting.

We are seeking an experienced Consultant Psychiatrist who will work on Norman ward in Green Parks House, Princess Royal University Hospital, Oxleas NHS Foundation Trust.

He/she will provide senior medical cover for18 acute inpatients admitted to Norman ward. The post-holder will be the Responsible Clinician (RC) for all detained inpatients.

The post-holder along with his/her designated 2 wte Core Trainees will be responsible for all clinical work and related administrative work including mental health act work and along with other team members ensure that patients have good quality care and that the Trust’s quality indicators are not breached. It is expected that the post holder will work very closely with the ward manager to deliver high quality and evidence based clinical care to inpatients and take a lead on clinical governance including the delivery of key performance targets, learning from incidents and service user feedback.

There are three general adult wards based at Green Parks House, Princess Royal University Hospital. These wards, Norman, Betts and Goddington admit both informal and detained patients. Norman ward has 18 beds and all wards are mixed sex wards.

The inpatient wards at Green Parks House successfully completed the College Research Unit’s accreditation programme for inpatient units in 2008. The focus of all the acute wards is on multi-disciplinary team assessment and treatment of mental illness through a range of treatments and collaborative care.

Treatment is achieved with high levels of observation and a range of treatment strategies, including pharmacological and psychological approaches, within a safe and secure environment.

RC chairs and leads a multidisciplinary review meeting of patients on the ward at least three times a week to ensure that there are enough opportunities to see each new patient at the earliest opportunity and that all patients are seen at least once a week.

Attend clinical governance meetings where the quality of care the ward provides is reviewed at least once a month

Attend the business meeting to support and provide a senior clinical view and perspective on issues that affect the ward and have a good understanding of the directorate’s priorities in relation to patient safety, patient experience and clinical effectiveness and understand external factors influencing services such as budgetary frameworks and inspection regime e.g. CQC inspections.

Regular one on one meetings with the ward manager

Ensure effective and appropriate use of medical resources within the team to address the needs of service users.

Ensure regular and adequate supervision of trainees and non-training medical staff as may be necessary

Ensure that the views of families and carers are sought when making decisions about patient care and discharge planning

Participate in directorates and sub-directorate meetings including task and finish, ad-hoc and regular meetings to address specific issues affecting the directorate or sub-directorate that require clinical input.

Ensure that there are no statutory breaches such as allowing sections to lapse. Ensure that capacity assessments or section 58 (consent to treatment) are completed in a timely fashion. Also ensure that discharge summaries or letters are completed in a timely manner and information contained such as discharge medication is accurate.

Engage in regular service reviews and developments such as use of outcome measures such as NICE audits of their practice and use of new technology when these become available to support their work for example IPads, Docman and Video Conferencing
