Band 4 Senior Healthcare Support Worker
An exciting opportunity has arisen for a Band 4 Senior Health Care Support Worker to work with our Integrated Respiratory Nursing service in the borough of Barking and Dagenham and the wider BHR teams.
We are looking for an enthusiastic and motivated individual, with excellent communication skills, who is able to work autonomously, as part of the team and to be an integral member of the wider Respiratory teams in supporting people managing the complexities of respiratory management.

This role requires facilitating patient pulmonary rehab program, having a sound knowledge of the principles of respiratory/COPD management and care and a keen interest in professional development & advancing with technology.Therefore, experience of teaching and good IT skills is essential.
The post holder will be responsible for providing; supporting and developing the delivery of high quality; evidence based Respiratory clinical practice throughout the local community, in health centre clinics and patients’ homes. The successful applicant will be requiredto work closely with the lead Respiratory consultant, GPs, nursing colleagues, Physiotherapists and other members of the multidisciplinary team.
The ability to travel effectively across the boroughs is necessary.

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - Outer London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,313 to a maximum of £5,436 per annum).
· Participating in the Barking, Havering and Redbridge LTC Respiratory Service providing support to staff and patients where required in different forms, locations and helping the effective running and provision of many aspects of the service.
· Being responsible for waiting lists and waiting list management, booking of appointments, patient contact in varying forms and communication with clinical lead and other staff.
· Running reports, review systems, audit, data collections, link working (if required) and policy review where appropriate.
·
· Active in-service improvement and development.
· Able to risk assess patients on an individual basis, advise on appropriate self management, appropriately refer on to other services, able to triage patients with escalation process within competencies
· Patient and staff education within competency
