An exciting opportunity has arisen for a Community Nurse, working within the Unplanned Care team within Barking and Dagenham .

We are seeking an innovative, caring and dedicated registered Nurse to compliment the Barking & Dagenham Community Health and Social Care team.

The post holder will be a registered nurse responsible for patients allocated to them and will asses, manage plan and deliver care, including being professional and legally accountable

Working within the unplanned care team the post holder will triage and provide care to patients within the borough of B & D requiring ad hoc/urgent visits in line with the CHSCS SOP

The unplanned care team provide cover 7 days a week between 7am and 7pm, the post holder will be required to be flexible and ensure cover, along with colleagues during this period

The post holders will lead and manage a Community Nursing Team, geographically aligned to GP practices and be responsible for the management of a caseload ensuring the provision of a high standard of nursing care to patients and their families in the community. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients. The district nurse works without direct supervision and provides leadership to the community nursing team.

We are looking for an exercise therapist with experience of working with people or clients in either an exercise or rehabilitation environment. Experience in exercise on referral services would be an advantage.

As per of the team you would be working in clinic across Havering and Barking &Dagenham.

The role will includes working closely with our musculoskeletal physiotherapists, exercise therapist and physiotherapy apprentice to develop exercise classes and individual programmes for clients with musculoskeletal conditions to promote their recovery and encourage self-management.

We have an experienced team of musculoskeletal therapists to support you. NELFT also has a dedicated lead for AHP support level roles to support development.
